Disagreement About Withholding Treatment on the Basis of Disability.

The parents of an infant born with arthrogryposis multiplex congenita (AMC) insist that she will not have a life worth living and ask the clinicians not to pursue life-preserving interventions and to discontinue feeding her. The neonatology team disagrees and worries that the parents are not making care choices in the patient's best interests. This challenging case is analyzed from 3 unique perspectives. First, a scholar living with AMC shares her experience navigating this diagnosis from childhood to adulthood alongside her family. A neonatologist-ethicist argues for neonatologists to inform

Withholding and withdrawing medical treatment: an emergency medicine perspective.

In emergency medicine, a significant difference rightfully persists between the withholding and withdrawal of life-sustaining medical treatment. The justification for this difference stems part from the nature of emergency medical practice and the unique manner in which clinicians apply many ethical principles. In the usual setting, the decision to withhold further medical treatment is done quietly, often without input from the patients surrogate decisionmaker, whereas withdrawal of ongoing medical treatment can be more obvious and difficult. This situation is reversed in the emergency medicin

Right Brain: Withholding treatment from a child with an epileptic encephalomyopathy.

The case of Charlie Gard, an infant who was hospitalized in England due to a mitochondrial DNA depletion syndrome that led to an epileptic encephalomyopathy, was highly publicized. Though Charlie's parents lobbied for him to receive experimental nucleoside replacement therapy as a desperate effort to save him, this request was denied, and after a lengthy legal battle, he died in late July 2017. We discuss the ethical considerations and consequences of this case.

Superimposition and withholding of edible consequences as treatment for automatically reinforced stereotypy.

The delivery and subsequent withholding of tangible consequences has been previously investigated as an intervention for stereotypic behavior. The current investigation sought to extend previous research by evaluating its effectiveness and durability as treatment for stereotypy of 2 children who had been diagnosed with autism. Nonsocial functions for stereotypic behavior were identified via functional analysis. Edible items were then delivered contingent on stereotypy and were withheld in a subsequent condition. When the superimposition procedure failed to reduce stereotypy, environmental enri

Withholding antibiotic treatment in pneumonia patients with dementia: a quantitative observational study.

BACKGROUND: Pneumonia is a life-threatening disease in nursing home patients with dementia. Physicians and families face choices about whether to withhold antibiotics when patients are expected to die soon or when treatment may be burdensome. However, little information exists on what factors influence this complex decision-making process. OBJECTIVE: To identify factors associated with decisions on whether to withhold curative antibiotic treatment in patients with dementia who have pneumonia. METHODS: We performed an observational cohort study with 3-month monitoring for cure and death. Pati

Treatment-Limiting Decisions in Neurointensive Care: Withholding or Withdrawal of Life-Sustaining Measures.

BACKGROUND: Treatment-limiting decisions (TLDs) in neurointensive care are frequently reported with no distinction between withholding and withdrawal of treatment. We investigated the proportion of patients subjected to withholding and withdrawal of life-sustaining treatment during neurointensive care, and the association with mortality. METHODS: This retrospective observational cohort study included all adult (≥ 18 years) neurocritically ill patients admitted to the neurointensive care unit (neuro-ICU), Rigshospitalet, Denmark, for a primary CNS injury during the period from July 2019 to Feb

The Influence of Hospital Policies on Clinicians' Decisions to Withhold or Withdraw Life-Sustaining Treatment.

BACKGROUND: There is considerable variation in clinicians' approaches to decisions to withhold or withdraw life-sustaining treatment (LST) across US hospitals. These differences are not explained by patient preferences alone and are likely influenced by other factors (eg, hospital policies, hospital culture, state laws, medical society guidelines). RESEARCH QUESTION: How do hospital policies influence clinician approaches to decisions to withhold or withdraw LST among patients admitted to an ICU? STUDY DESIGN AND METHODS: We conducted semistructured interviews with ICU nurses and physicians

Guidelines for Decisions to Withhold or Withdraw Life-Sustaining Treatments (LST) in Adult ICU.

OBJECTIVE: The French Society of Anaesthesia and Intensive Care Medicine (Société française d'anesthésie et de réanimation, SFAR) and the French Society of Physical Medicine and Rehabilitation (Société Française de Médecine Physique et de Réadaptation (SOFMER)) aimed at providing guidelines for decisions to withhold or withdraw life-sustaining treatments (LST) in adult ICU. DESIGN: A consensus committee of 20 experts from the SFAR and the SOFMER was convened. A formal conflict-of-interest (COI) policy was developed at the beginning of the process and enforced throughout. The entire guideline c

The legal role of medical professionals in decisions to withhold or withdraw life-sustaining treatment: Part 3 (Victoria).

This is the final article in a series of three that examines the legal role of medical professionals in decisions to withhold or withdraw life-sustaining treatment from adults who lack capacity. This article considers the position in Victoria. A review of the law in this State reveals that medical professionals play significant legal roles in these decisions. However, the law is problematic in a number of respects and this is likely to impede medical professionals' legal knowledge in this area. The article examines the level of training that medical professionals receive on issues such as refu

The impact of withholding and withdrawal life-sustaining treatment issues on patients with sepsis: a prospective, nationwide, multicenter cohort study.

Approximately half of the patients with sepsis require intensive care unit (ICU) management and their mortality rate remains high. The concept of withholding and withdrawal life-sustaining treatment (WWLST) issue was introduced to limit the suffering of critically ill patients. However, little is known about the characteristics and outcomes of WWLST in patients with sepsis. We conducted a nationwide cohort study of adult patients with sepsis prospectively collected from the Korean Sepsis Alliance Database of 20 tertiary referral or university-affiliated hospitals in South Korea between Septemb

Predictors and outcomes of withholding and withdrawal of life-sustaining treatments in intensive care units in Singapore: a multicentre observational study.

BACKGROUND: Clinical practice guidelines on limitation of life-sustaining treatments (LST) in the intensive care unit (ICU), in the form of withholding or withdrawal of LST, state that there is no ethical difference between the two. Such statements are not uniformly accepted worldwide, and there are few studies on LST limitation in Asia. This study aimed to evaluate the predictors and outcomes of withholding and withdrawal of LST in Singapore, focusing on the similarities and differences between the two approaches. METHODS: This was a multicentre observational study of patients admitted to 21

Factors Associated With Family Caregivers' Attitudes Toward Withholding Life-Sustaining Treatment in Older Adults With Dementia: A Cross-Sectional Study.

AIM: To examine factors associated with family caregivers' attitudes toward withholding life-sustaining treatments (LSTs) for older adults with dementia. DESIGN: Cross-sectional study. METHODS: A questionnaire survey assessed advance care planning (ACP) knowledge, perceptions of a good death, caregiver burden, and attitudes toward withholding LSTs using previously developed instruments. Hierarchical regression was supplemented by bootstrap confidence intervals, robust inference, and a reduced clinical sensitivity model. DATA SOURCES: Data were collected from 113 family caregivers of older a

Management of epistaxis in hereditary haemorrhagic telangiectasia (HHT) patients using pulsed dye laser and the effect of withholding treatment during the COVID pandemic.

Using a patient survey, pulsed dye laser (PDL) treatment of epistaxis for hereditary haemorrhagic telangiectasia (HHT) patients was evaluated after initial referral. Subsequently, due to the COVID pandemic, a natural experimental set-up allowed assessment of an enforced withdrawal of treatment. A total of 34 subjects were identified as undergoing PDL for HHT-related epistaxis. They were surveyed to look at the effectiveness of PDL treatment after initial referral and at the effect of delay to treatment during COVID on epistaxis and the associated quality of life. The survey also examined the c

Early decision effect of withholding and/or withdrawing life-sustaining treatment on emergency department patients' short- and long-term outcomes: a retrospective analysis.

BACKGROUND: In France, a significant proportion of end-of-life (EOL) decisions occur in emergency departments (EDs), where time constraints and limited resources may hinder optimal care. Although withholding or withdrawing life-sustaining treatments (WHWD) is common in this context, the impact of timing on patient outcomes remains unclear. The aim of this study was to assess the effect of WHWD decision timing on short- and long-term outcomes among adult emergency department patients. METHODS: We conducted a retrospective, observational study at Beaujon Hospital's ED (Paris, France) between Ja
