Work of breathing after extubation.

Recently we showed that work of breathing was higher in the immediate period after extubation as compared with spontaneous breathing through an endotracheal tube. In this study, we evaluated the glottis and trachea as potential sites of increased airway resistance after extubation. We measured breathing pattern, work of breathing, and pressure time product in eight patients during weaning from mechanical ventilation. We acquired data during pressure support ventilation and spontaneous breathing via the ventilator, with the endotracheal tube in place, and after extubation. During bronchoscopy a

Assessing effort and work of breathing.

PURPOSE OF REVIEW: Spontaneous breathing has been shown to induce both positive and negative effects on the function and on injury of lungs and diaphragm during critical illness; thus, monitoring of the breathing effort generated by the patient might be valuable for a better understanding of the mechanisms of disease and to set properly ventilation. The purpose of this review is to summarize the recent findings on the different techniques available to measure the patient's breathing effort, mainly during spontaneous assisted ventilation. RECENT FINDINGS: Although esophageal pressure measureme

[Clinical study of imposed work of breathing caused by the breathing apparatus].

OBJECTIVE: The imposed work of breathing caused by the breathing apparatus (endotracheal tube, breathing circuit tubing, and ventilator's valves) may increase patient's work of breathing. To evaluate the imposed work may be useful to determine weaning. METHODS: Work of breathing was observed by Bicore CP-100 breathing monitor at different levels pressure support (PS), T piece and after extubation, and the PS level to compensate imposed work of breathing was computed. RESULTS: When PS level decreased, patient's work of breathing increased significantly. At PS O kPa, the patient's work of brea

Subglottic Stenosis Position Affects Work of Breathing.

OBJECTIVES: Subglottic stenosis (SGS) is the most common type of laryngeal stenosis in neonates. SGS severity is currently graded based on percent area of obstruction (%AO) via the Myer-Cotton grading scale. However, patients with similar %AO can have widely different clinical courses. Computational fluid dynamics (CFD) based on patient-specific imaging can quantify the relationship between airway geometry and flow dynamics. We investigated the effect of %AO and axial position of SGS on work of breathing (WOB) in neonates using magnetic resonance imaging. METHODS: High-resolution ultrashort e

[Work of breathing in differentiation of various forms of sleep-related breathing disorders].

BACKGROUND: In contrast to the obstructive sleep apnoea syndrome (OSA) the obesity-hypoventilation syndrome (OHS) is characterised by persistent hypercapnia during the day and predominant hypoventilation during sleep. In this study we wanted to know whether work of breathing (WOB) in a sitting and supine position separates both groups. PATIENTS AND METHODS: OSA population: 20 men, 50.5 +/- 9.2 years, Body Mass Index (BMI: 54.1 +/- 6.9 kg/m2, pO2: 65.6 +/- 6.6 mmHg, pCO2: 40.6 +/- 3.1 mmHg, OHS-group: 14 patients, 13 men age: 53.1 +/- 9.3 years, BMI: 53.1 +/- 9.3 kg/m2, pO2: 51.8 +/- 10.5 mmHg

Diaphragmatic work of breathing in premature human infants.

Present methods of assessing the work of breathing in human infants do not account for the added load when intercostal muscle activity is lost and rib cage distortion occurs. We have developed a technique for assessing diaphragmatic work in this circumstance utilizing measurements of transdiaphragmatic pressure and abdominal volume displacement. Eleven preterm infants without evidence of lung disease were studied. During periods of minimal rib cage distortion, inspiratory diaphragmatic work averaged 5.9 g X cm X ml-1, increasing to an average of 12.4 g X cm X ml-1 with periods of paradoxical r

Work of breathing and respiratory drive in obesity.

Obesity, particularly severe central obesity, affects respiratory physiology both at rest and during exercise. Reductions in expiratory reserve volume, functional residual capacity, respiratory system compliance and impaired respiratory system mechanics produce a restrictive ventilatory defect. Low functional residual capacity and reductions in expiratory reserve volume increase the risk of expiratory flow limitation and airway closure during quiet breathing. Consequently, obesity may cause expiratory flow limitation and the development of intrinsic positive end expiratory pressure, especially

Increased Work of Breathing due to Tracheomalacia in Neonates.

Rationale: Dynamic collapse of the tracheal lumen (tracheomalacia) occurs frequently in premature neonates, particularly in those with common comorbidities such as bronchopulmonary dysplasia. The tracheal collapse increases the effort necessary to breathe (work of breathing [WOB]). However, quantifying the increased WOB related to tracheomalacia has previously not been possible. Therefore, it is also not currently possible to separate the impact of tracheomalacia on patient symptoms from parenchymal abnormalities. Objectives: To measure the increase in WOB due to airway motion in individual sub

Airway caliber and the work of breathing in humans.

Mechanical work rate of breathing was measured in five normal subjects during voluntary eucapnic hyperventilation at rates of approximately 10, 20, 40, 60, and 80 l/min before and after inhalation of 1 mg of ipratropium bromide, an anticholinergic agent. Chest wall recoil pressure was measured over a range of lung volumes in each subject and was used as the reference pressure in the calculation of work rate. There was little change in elastic or resistive work rate at rest when vagal tone was reduced by ipratropium. The mean work at 40, 60, and 80 l/min was 8.9, 17.2, and 34.0 cmH2O.l-1.s befo

Optimization of inspiratory work in periodic breathing in infants.

In periodic breathing, there are repeated cycles of bursts of breaths separated by pauses several seconds long. We consider the mechanics of periodic breathing in human infants using calibrated traces of tidal volume and esophageal pressure recorded during the first few days after delivery. Each cycle of periodic breathing was analyzed in terms of the inspiratory time and beginning and end inspiratory volumes for each breath, the number of breaths in the cycle, and the total observed inspiratory work. A simple model was used to characterize the mechanics of the lung during inspiration, and the

The inspiratory work of breathing during assisted mechanical ventilation.

We quantified the mechanical work of breathing in six normal subjects during assisted mechanical ventilation. Using two volume-cycled ventilators of different design, we investigated the influence of minute ventilation (VE) and machine settings of trigger sensitivity and flow during CO2-driven hyperventilation to moderate and high levels (12-24 L/min). Work estimates were derived from plots of esophageal and airway pressure against inflation volume. Peak flow and trigger sensitivity were important determinants of the energy expended, and for each combination of machine settings the work done b

Breathing He-O2 increases ventilation but does not decrease the work of breathing during exercise.

We previously observed an increase in minute ventilation (V E) with resistive unloading (He-O2 breathing) in healthy elderly subjects with normal pulmonary function. To investigate the effects of resistive unloading in elderly subjects with mild chronic airflow limitation (FEV(1)/FVC: 61 +/- 4%), we studied 10 elderly men and women 70 +/- 3 yr of age. These subjects performed graded cycle ergometry to exhaustion, once breathing room air and once breathing a He-O2 gas mixture (79% He, 21% O2). V E, pulmonary mechanics, and PET(CO2) were measured during each 1-min increment in work rate. Data we

Spontaneous breathing trial and post-extubation work of breathing in morbidly obese critically ill patients.

BACKGROUND: Predicting whether an obese critically ill patient can be successfully extubated may be specially challenging. Several weaning tests have been described but no physiological study has evaluated the weaning test that would best reflect the post-extubation inspiratory effort. METHODS: This was a physiological randomized crossover study in a medical and surgical single-center Intensive Care Unit, in patients with body mass index (BMI) >35 kg/m2 who were mechanically ventilated for more than 24 h and underwent a weaning test. After randomization, 17 patients were explored using five s

Effect of pediatric ventilation weaning technique on work of breathing.

BACKGROUND: Ventilator liberation is one of the most challenging aspects in patients with respiratory failure. Most patients are weaned through a transition from full to partial respiratory support, whereas some advocate using a continuous spontaneous ventilation (CSV). However, there is little scientific evidence supporting the practice of pediatric ventilator liberation, including the timing of onset of and the approach to weaning mode. We sought to explore differences in patient effort between a pressure controlled continuous mode of ventilation (PC-CMV) [in this cohort PC assist/control (P

Work of breathing and different levels of volume-targeted ventilation.

OBJECTIVES: The objectives of this study were to determine the impact of different volume-targeted levels on the work of breathing and to investigate whether a level that reduced the work of breathing below that experienced during ventilatory support without volume targeting could be determined. METHODS: The transdiaphragmatic pressure-time product, as an estimate of the work of breathing, was measured for 20 infants (median gestational age: 28 weeks) who were being weaned from respiratory support by using patient-triggered ventilation (either assist-control ventilation or synchronous intermi

Work of Breathing by the Campbell Diagram: Physiology and Practice.

The work of breathing (WOB) quantifies the energy cost of ventilation. This tutorial explores the fundamental physiology of WOB, its measurement via the Campbell diagram, and its bedside clinical application. The diagram visually partitions WOB into elastic (lung and chest wall) and resistive components, offering a physiological phenotype of respiratory failure. We describe how pathologies like ARDS and COPD alter the load-capacity balance: reduced compliance drastically increases elastic work (flattening the pressure-volume loop), while air flow obstruction increases resistive work (widening

Imposed Work of Breathing During High-Frequency Oscillatory Ventilation in Spontaneously Breathing Neonatal and Pediatric Models.

BACKGROUND: High-frequency oscillatory ventilation (HFOV) is used in cases of neonatal and pediatric acute respiratory failure, sometimes even as the primary ventilatory mode. Allowing patients (at least neonates) on HFOV to breathe spontaneously soon after intubation has been shown to be feasible, and this is becoming a more generally used approach for infants and small children. However, such an approach may increase the imposed work of breathing (WOB), raising the question of whether the imposed WOB varies with the use of newer-generation HFOV devices, which operate according to different f

Breathing frequency and pattern are poor predictors of work of breathing in patients receiving pressure support ventilation.

OBJECTIVE: To evaluate the relationships between directly measured work of breathing (WOB) and variables of the breathing pattern commonly used at the bedside to infer WOB for intubated, spontaneously breathing patients treated with pressure support ventilation (PSV). DESIGN: In vivo measurements of the WOB were obtained on a consecutive series of adults.
