Lumbar radiculopathy after zygapophyseal joint injection.

We report a case of lumbar radiculopathy after zygapophyseal joint injections for chronic low back pain. The management of the patient and potential causes for the radiculopathy are discussed. The case acts as a reminder that the spinal nerve roots should be considered when performing intra-articular facet joint injections and demonstrates the importance of including nerve injury in the patient consent process as a rare, but significant complication.

Percutaneous radio-frequency neurotomy for chronic cervical zygapophyseal-joint pain.

BACKGROUND: Chronic pain in the cervical zygapohyseal joints is a common problem after whiplash injury, but treatment is difficult. Percutaneous radiofrequency neurotomy can relieve the pain by denaturing the nerves innervating the painful joint, but the efficacy of this treatment has not been established. METHODS: In a randomized, double-blind trial, we compared percutaneous radio-frequency neurotomy in which multiple lesions were made and the temperature of the electrode making the lesions was raised to 80 degrees C with a control treatment using an identical procedure except that the radio

Fluoroscopic percutaneous lumbar zygapophyseal joint cyst rupture: a clinical outcome study.

BACKGROUND CONTEXT: Lumbar zygapophyseal joint (Z-joint) synovial cysts can cause low back pain (LBP), spinal stenosis, and lower extremity radiculopathy. In the literature, there are several minimally invasive techniques described with mixed results. Typical recommended treatment is surgical resection of the cyst. Currently, there is little information available concerning the efficacy and outcome with treatment of Z-joint synovial cyst by percutaneous, fluoroscopic, contrast-enhanced distention, and rupture. PURPOSE: To evaluate the therapeutic value and safety of Z-joint cyst rupture in sy

Morphometric study of the orientation of lumbar zygapophyseal joints in a South Indian population.

PURPOSE: To determine the orientation of lumbar zygapophyseal joints and prevalence of facet tropism (FT) identified by computerized tomographic (CT) scans. METHODS: In a cross-sectional study, 124 CT scans of patients with spinal injuries, seen between 2011and 2015, were retrospectively studied. A total of 566 uninjured motion segments were analysed. The right and left zygapophyseal joint angles (ZJAs) from L1-L2 to L5-S1 were measured on axial sections at uninjured levels. The absolute difference between the right and left ZJA was calculated to determine the prevalence and severity of FT at

The effects of a semiconstrained integrated artificial disc on zygapophyseal joint pressure and displacement.

STUDY DESIGN: Measurement of zygapophyseal joint pressure and displacement was performed after placement of a semiconstrained integrated artificial disc (SIAD) in a cadaver model. OBJECTIVE: To understand the likelihood of accelerated zygapophyseal joints degeneration as a result of the implant. SUMMARY OF BACKGROUND DATA: A SIAD has been developed to treat lumbar spondylosis secondary to segmental disc degeneration and spinal stenosis. The SIAD replaces the stenotic segment's disc. Previous studies have demonstrated that nonconstrained artificial disc (NAD) replacements fail to achieve thei

[Radicular syndrome and synovial cyst of the zygapophyseal joints: two case-reports].

INTRODUCTION: Multiradicular nerve root compression, and long lasting radicular syndrome occur very often. They happen particularly on degenerative spine. Huge synovial cyst of the zygapophyseal joints may account for it, expand in the epidural area and cause radicular syndrome. EXEGESIS: Two cases of huge synovial cysts spreading into the spinal channel are reported here. Diagnostic and therapeutic modalities are discussed. On degenerative spine, facet joints osteoarthritis may result in synovial cysts. Physical examination findings are radicular syndrome. CONCLUSION: Huge synovial cysts ma

The orientation of the articular facets of the zygapophyseal joints at the cervical and upper thoracic region.

Knowledge of the orientation of facet joints in the cervical and upper thoracic region is important for understanding the biomechanical properties and clinical conditions relevant to the neck. The study was undertaken on dry macerated bones from 30 adult male human vertebral columns. The orientation of the superior articular facets in relation to their inclination with the sagittal and transverse planes was examined between C3 and T3 vertebrae in each column. The linear dimensions of the superior articular facets and the width/height ratio were also calculated. The results show that all verteb

Clinical predictors of screening lumbar zygapophyseal joint blocks: development of clinical prediction rules.

BACKGROUND: Only controlled intra-articular zygapophyseal joint (ZJ) injections or medial branch blocks can diagnose ZJ-mediated low back pain. The low prevalence of ZJ pain implies that identification of clinical predictors of a positive response to a screening block is needed. PURPOSE: To estimate the predictive power of clinical findings in relation to pain reduction after screening ZJ blocks. STUDY DESIGN: As part of a wider prospective blinded study investigating diagnostic accuracy of clinical variables, a secondary analysis was carried out to seek evidence of variables potentially val

Fluoroscopically guided aspiration of a symptomatic lumbar zygapophyseal joint cyst: a case report.

Lumbar zygapophyseal joint cysts are potential pain generators in patients with axial pain and/or lower-limb radicular pain. The traditional treatment of symptomatic cysts that do not respond to conservative measures is surgical decompression. Percutaneous needle aspiration under fluoroscopic guidance is a potential minimally invasive treatment option. We present a patient with a large symptomatic L4-5 zygapophyseal joint cyst and right L5 radiculopathy who responded well to percutaneous aspiration.

Osteophytes on the zygapophyseal (facet) joints of the cervical spine (C3-C7): A skeletal study.

Previous studies have reported that osteophytes in the cervical vertebrae may cause immobility, neck stiffness, osteoarthritis, headaches, nerve entrapment syndromes, and compression of the vertebral artery. Our objective was to explore the osteophytes' expression on zygapophyseal joints C3-C7. This is a cross-sectional observational skeletal study. The study sample comprised 273 human skeletons of both sexes, aged 20-93, housed at the Natural History Museum, OH, USA. A grading system assessed the presence and severity of osteophytosis on the zygapophyseal joints. The chi-square test (SPSS 25.

Radiologic study of the influence of zygapophyseal joint orientation on spinal injuries at the thoracolumbar junction.

The change from coronal to sagittal plane orientation of the zygapophyseal (facet) joints at the thoracolumbar junction, coupled with differences in lumbar and thoracic spine mobility, may predispose the T10 to L2 segments to injury. To test for an association between the level of injury and variations in orientation of the zygapophyseal joints, CT investigations of 44 spinal injured patients were studied. Of these, 28 sustained burst/compression fractures and 16 demonstrated a rotation injury with disruption to one or both zygapophyseal joints. Injuries were examined to determine whether more

Degenerative Zygapophyseal Joint Subluxation of the Lumbar Spine: A Modified Classification and Clinical Considerations.

Degenerative lumbar zygapophyseal (facet) joint subluxation without prior trauma, fracture, spondylolysis, or spondylolisthesis is extremely uncommon and represents a distinct clinical entity. This study aims to present rare cases of advanced degenerative facet joint changes combined with unilateral or bilateral facet joint subluxation in the absence of trauma. All patients experienced chronic symptoms unresponsive to conservative treatment. MRI findings demonstrated Grade 4 degeneration per Grogan's classification, along with distinct subluxation patterns not captured by existing grading syst
