Epigenetic regulation of human gamma-glutamyl hydrolase activity in acute lymphoblastic leukemia cells.

Gamma-glutamyl hydrolase (GGH) catalyzes degradation of the active polyglutamates of natural folates and the antifolate methotrexate (MTX). We found that GGH activity is directly related to GGH messenger RNA expression in acute lymphoblastic leukemia (ALL) cells of patients with a wild-type germline GGH genotype. We identified two CpG islands (CpG1 and CpG2) in the region extending from the GGH promoter through the first exon and into intron 1 and showed that methylation of both CpG islands in the GGH promoter (seen in leukemia cells from approximately 15% of patients with nonhyperdiploid B-li

Methotrexate intracellular disposition in acute lymphoblastic leukemia: a mathematical model of gamma-glutamyl hydrolase activity.

Methotrexate (MTX) is an antifolate that is widely used for the treatment of childhood acute lymphoblastic leukemia (ALL) and a number of other malignant and nonmalignant diseases. Within cells, MTX is metabolized to more active methotrexate polyglutamates (MTXPG), and these polyglutamates are subsequently cleaved in lysosomes by gamma-glutamyl hydrolase (GGH). GGH is reported to act as either an endopeptidase or an exopeptidase, exhibiting species differences in these functions. To better define the in vivo functions of GGH in human leukemia cells, we characterized GGH activity with different

The effect of dietary zinc deficiency on pancreatic gamma-glutamyl hydrolase (EC 3.4.22.12) activity and on the absorption of pteroylpolyglutamate in rats.

The effect of dietary zinc deficiency on gamma-glutamyl hydrolase (EC 3.4.22.12) activity and on pteroylpolyglutamate absorption was investigated in rats. Enzyme activity was determined in pancreas and gut lumen washings. Pteroylpolyglutamate absorption was studied by determining the rise in plasma folate levels following pteroylpolyglutamate ingestion. Two experiments were performed; in each purified diets were given to three groups of immature male Wistar rats for approximately 2 weeks. One group was given a Zn-deficient diet ad lib. (ZD), the second was pair-fed daily with this group on a Z

Evaluation of Intracellular Metabolism of Methotrexate in Hepatocytes and Embryonic Kidney Cells based on Folylpolyglutamate Synthetase and Gamma-Glutamyl Hydrolase Expression.

BACKGROUND: Methotrexate (MTX) is a common folic acid antagonist in clinical medicine, easily inducing a common adverse side effect of liver and kidney injury. It has been found that the expression of Folylpolyglutamate Synthetase (FPGS) and gamma-Glutamyl Hydrolase (GGH) may be closely related to that of related proteins to affect the intracellular metabolism of MTX. OBJECTIVE: The relationship between FPGS/GGH and MTXPGs accumulation in liver and kidney cells was explored by adjusting the expression of FPGS and GGH in cells using UPLC-MS/MS quantitative technology. METHOD: Based on UPLC-MS

A substrate specific functional polymorphism of human gamma-glutamyl hydrolase alters catalytic activity and methotrexate polyglutamate accumulation in acute lymphoblastic leukaemia cells.

We found a significant inverse relationship between gamma-glutamyl hydrolase (GGH) activity and the accumulation of long-chain methotrexate polyglutamates (MTXPG4-7) in non-hyperdiploid B-lineage acute lymphoblastic leukaemia (ALL) cells after uniform treatment with high-dose methotrexate (HDMTX) (1 g/m i.v.). To identify genetic polymorphisms that alter the function of human GGH, we sequenced the GGH exons of genomic DNA from children with ALL, who had a 7.8-fold range of GGH activity in their ALL cells at diagnosis. A single nucleotide polymorphism (452C>T, T127I) was found among patients wi

Phosphonate and phosphinate analogues of N-acylated gamma-glutamylglutamate. potent inhibitors of glutamate carboxypeptidase II.

Phosphonate and phosphinate analogues of N-acylated gamma-glutamylglutamate were tested for the ability to inhibit glutamate carboxypeptidase II (GCP II). All of the compounds inhibit GCP II with IC(50) values in the low nanomolar range. The comparison of the results to previously reported inhibitory studies of the same compounds toward folylpoly-gamma-glutamyl synthetase (FPGS) and gamma-glutamyl hydrolase (gamma-GH) provides insight into structural and mechanistic features of each enzyme. Potential utility of these compounds as diagnostic agents and probes to understand folate or antifolate

Physiological role of gamma-glutamyl transpeptidase: Demise of the gamma-glutamyl cycle.

Gamma-glutamyl transpeptidase (GGT) is an enzyme with a plethora of names and purported functions. The enzyme has long been identified with an amino acid transport system called the gamma-glutamyl cycle which was proposed in the 1970s. The foundation of this cycle held that GGT catalyzed a transpeptidation reaction and in the process transported an extracellular amino acid into the cell as a gamma-glutamyl amino acid. However, subsequent studies showed that under physiological conditions the enzyme is a hydrolase. It cleaves the gamma-glutamyl bond of glutathione, glutathione S-conjugates and

Jejunal brush-border folate hydrolase. A novel enzyme.

Dietary folate, a vitamin required for DNA synthesis and cell regeneration, occurs as pteroylpolyglutamates that are hydrolyzed to pteroylglutamate during the process of intestinal absorption. Studies from our laboratory over the past 15 years have shown that jejunal brush-border folate hydrolase is essential and rate-limiting in folate absorption. Brush-border folate hydrolase activity and pteroylpolyglutamate hydrolysis are inhibited in disease and conditions associated with folate deficiency, including celiac and tropical sprue, the use of sulfasalazine to treat inflammatory bowel disease,

"Per aspera ad astra": the transformative contribution of glucarpidase to the improved management of delayed methotrexate elimination after high-dose therapy.

INTRODUCTION: High-dose methotrexate (HDMTX) is an important therapeutic tool for various malignancies; its use can be associated with severe treatment-emergent toxicities, particularly nephrotoxicity. These can disrupt anticancer treatment and increase morbidity and mortality. Glucarpidase (carboxypeptidase G2), a recombinant bacterial enzyme, rapidly converts toxic levels of circulating methotrexate (MTX) into nontoxic metabolites in patients with delayed MTX elimination and/or at risk of MTX toxicity. The reduction in MTX-associated toxicity and mortality can improve patients' outcomes. MTX

Pharmacokinetics and Pharmacodynamics of Glucarpidase Rescue Treatment After High-dose Methotrexate Therapy Based on Modeling and Simulation.

BACKGROUND: Model-informed approaches are important in drug development, including for dose optimization and the collection of evidence in support of efficacy. MATERIALS AND METHODS: We developed a modified Michaelis-Menten pharmacokinetics/pharmacodynamics model and used it to conduct simulations of glucarpidase at doses between 10 and 80 U/kg rescue treatment after high-dose methotrexate therapy. We carried out a dose-finding modeling and simulation study before a phase II study of glucarpidase. Monte-Carlo simulations were conducted using the deSolve package of R software (version 4.1.2).

Glucarpidase (carboxypeptidase G2): Biotechnological production, clinical application as a methotrexate antidote, and placement in targeted cancer therapy.

Patients receiving high-dose methotrexate (HDMTX) for malignancies are exposed to diverse complications, including nephrotoxicity, hepatotoxicity, mucositis, myelotoxicity, neurological symptoms, and death. Glucarpidase is a recombinant carboxypeptidase G2 (CPG2) that converts MTX into nontoxic metabolites. In this study, the role of vector type, gene optimization, orientation, and host on the expression of CPG2 is investigated. The effectiveness of various therapeutic regimens containing glucarpidase is classified and perspectives on the dose adjustment based on precision medicine are provide

Prostate-specific membrane antigen: a novel folate hydrolase in human prostatic carcinoma cells.

A novel monoclonal antibody has been developed that reacts strongly with human prostatic cancer, especially tumors of high grade. This antibody (7E11C-5) is currently in Phase 3 trials as an imaging agent for metastatic disease. We have cloned the gene that encodes the antigen that is recognized by the 7E11C-5 monoclonal antibody and have designated this unique protein prostate-specific membrane (PSM) antigen. PSM antigen is a putative class II transmembranous glycoprotein exhibiting a molecular size of Mr 94,000. Functionally, class II membrane proteins serve as transport or binding proteins

Prevention and Treatment of Acute Kidney Injury Associated with High-Dose Methotrexate.

AKI is a rare but life-threatening complication of the administration of methotrexate (MTX) at high doses (≥1 g/m 2 ) for the treatment of solid or hematological malignancies. MTX overexposure can lead to MTX-AKI and subsequent higher risk of extrakidney toxicities, morbidity, and mortality. MTX-AKI can also lead to secondary CKD requiring a reduced dose or contraindication for subsequent MTX infusions, thus worsening the cancer-related prognosis. Treatment of MTX-AKI is mainly preventive, combining alkaline hyperhydration, withdrawal of all nephrotoxic agents and drugs that modulate the metab

A Differential Electrochemical Sensor for Resolving Metabolite Cross-Reactivity Toward Selective Chemotherapy Monitoring.

Despite its widespread use, high-dose methotrexate (MTX) regimen presents a risk of life-threatening nephrotoxicity. Consequently, rigorous monitoring of MTX levels at predefined time points is highly recommended to assess toxicity and guide dosing of rescue therapy such as glucarpidase. Upon administration, glucarpidase rapidly converts MTX into its inactive metabolite 2,4-diamino-N10-methylpteroic acid (DAMPA) which displays structural similarity to MTX, causing an overestimation of the actual MTX levels. Therefore, patients receiving glucarpidase necessitate analytical methods capable of sp

Design of a dual-function agent by fusing a designed anti-VEGF-A binder and CPG-2 enzyme.

Anti-VEGF therapies are common for the treatment of cancer. Carboxypeptidase G (CPG-2) enzyme is a zinc-dependent metalloenzyme that metabolizes non-toxic synthetic 'benzoic mustard prodrugs' to cytotoxic moieties in tumor cells. In this study, we designed a dual-activity agent by combining a designed anti-VEGF- and CPG-2 enzyme to convert methotrexate (MTX). VEGF-A was docked against a set of scaffolds, and suitable inverse rotamers were made. Rosetta design was used for the interface design. The top 1200 binders were chosen by flow cytometry and displayed in yeast. The activity of CPG-2 enzy
