Аннотация

BACKGROUND: Hypospadias is an anomaly of the male urethra, in which the opening of the urethra is on the undersurface of the penis. Neourethral covering plays a surprisingly important role in the success of hypospadias repair. This study aimed to assess the outcome of using an autologous platelet-rich plasma graft as a second covering layer in distal hypospadias repair using the Snodgrass technique.

METHODS: Our study included children diagnosed with primary distal hypospadias with wide glans and healthy wide urethral plates. They were randomly classified into two groups: Group I: patients who underwent Snodgrass repair with a dartos flap. Group II: patients underwent Snodgrass repair with a dartos flap and an additional interposition layer of platelet-rich plasma then followed up for 6 months to evaluate the surgical success or presence complications.

RESULTS: We analyzed 160 cases of distal hypospadias, 80 cases in each group. Penile edema occurred in 16 patients in group I (25 %) and in 8 patients (10 %) in group II, (P = 0.077). Wound infection occurred in 10 patients in group I (12.5 %) and in two patients (2.5 %) in group II, which was statistically significant (P = 0.016). Glanular dehiscence occurred in two patients in group I (2.5 %) and one patient (1.7 %) in group II (P = 0.560). Urethrocutaneous fistula occurred in 12 patients in group I (15 %) and in 3 patients in group II (3.7 %), which was statistically significant (P = 0.015). Meatal stenosis occurred in seven patients from group I (8.7 %), while four patients from group II (5 %) developed meatal stenosis (P = 0.349).

CONCLUSIONS: Adding an autologous platelet-rich plasma graft as an interposition layer to the standard dartos flap in Snodgrass repair of distal hypospadias appears to reduce postoperative complications, including urethrocutaneous fistula and wound infection. PRP provides regenerative and protective effects that enhance tissue healing.
