Achieving correction of moderate to severe hallux valgus deformity of the 1st metatarsal by Proximal opening wedge osteotomy using a locking plate (A prospective study).

BACKGROUND: The hallux valgus deformity is among the most frequent foot deformities. Treatment of symptomatic moderate-to-severe hallux valgus deformities offers huge challenges in foot and ankle. There are many surgical procedures have been described for the correction of moderate-to-severe hallux valgus deformity. Proximal osteotomies has become popular with the recent development of low-profile plates for the opening wedge. We believe that the proximal opening wedge osteotomy technique provides excellent correction, technically easy to perform and reproducible.
PATIENTS AND METHODS: This is a prospective research involves cases with moderate to severe Hallux Valgus deformity, These patients underwent conservative therapy, involving changing the footwear and the orthosis, but there was no change and the pain continued to worsen for a year , There were 3 men and 12 women, among the 15 hallux valgus deformities , 9 occurred on the right foot and 6 occurred on the left foot, the average follow up period was 6 month.
RESULTS: 15 cases with moderate to severe Hallux Valgus deformity were involved. There is high statistically significant variance among the studied groups regarding pre-operative HVA and post- operative HVA, before surgery IMA and after surgery IMA and pre-operative pain and post- operative pain (p-value < 0.001). There were 50% dissatisfied patients and 50% satisfied patients.
CONCLUSION: Proximal opening wedge osteotomy is considered a powerful technique that can be performed with technical easy and the benefit of using a harfesting autograft from the bunion itself helped us preventing non union or delayed union. The using of a locking plates gave us good stability and early weight bearing.
