Аннотация

This letter to the editor examines recent work by Nelson et al. regarding the clinical and logistical determinants of operative timing, surgical approach, and hospital admission in 242 pediatric supracondylar humerus fractures. While acknowledging the study's insights into resource allocation, the author raises several methodological and statistical concerns. These include potential selection bias from differential missing data on weekends, the exclusion of nonoperatively managed patients which limits triage applicability, and the recoding of surgical approaches. Furthermore, the critique highlights limitations regarding multivariable model construction without correction for multiple comparisons, and notes that the evaluated outcomes reflect healthcare utilization rather than direct patient-centered benefits or long-term functional recovery. The author concludes that addressing these methodological factors, alongside multicenter validation and patient-centered outcomes, is vital before implementing such models in clinical triage.
