[Outcomes of proximal femur osteoarticular allograft reconstruction after oncologic resection in children].

Limb-sparing surgery and femoral reconstruction in children under 10 years of age is key to ensuring adequate acetabular development and reducing limb length discrepancy. This study aims to describe the long-term results of proximal femoral structural allograft reconstruction in three children with primary bone sarcomas. Three patients with a mean follow-up of 13.6 years were included. All were treated with resection of the proximal femur and reconstruction with adult donor osteoarticular graft. Two retained the graft, one of which required lengthening over native bone for length discrepancy, and the other underwent conversion to an alloprosthesis due to collapse of the femoral head. In the third case, because of a fracture, it was revised to an endoprosthesis. All patients achieved adequate acetabular development at skeletal maturity. Osteoarticular allografts are a viable option after oncological in bloc resection in patients under 10 years of age, as they facilitate acetabular development and preserve bone stock. However, they are mostly associated with variable postoperative complications.
