OBJECTIVES: Invasive meningococcal disease (IMD) declined during lockdown periods, whereas case fatality rates (CFRs) increased. These returned to baseline when restrictions were removed and IMD was re-established. We describe the characteristics and outcomes of IMD cases during the lockdown periods investigating factors associated with increased CFR.

METHODS: In England, the UK Health Security Agency conducts enhanced IMD surveillance. We compared all IMD cases and fatalities during 2015-2024 that occurred inside and outside the national lockdown periods (March 23, 2020 to July 19, 2021).

RESULTS: During the lockdown period, IMD CFR was 12.5% (15 of 120) compared with 8.5% (47 of 556) in 2019 due to meningococcal serogroup B (12.4% [11 of 89] died vs 6.8% [23 of 338] in 2019) or meningococcal serogroup Y (57.1% [four of seven] died vs 8.9% [five of 56] in 2019). All four infant MenB IMD fatalities during lockdown were dead on arrival or died in the emergency department. Nine of 11 remaining fatalities accessed health care within 1 day of onset, and two were inpatients at diagnosis. A total of 10 of 15 deaths were in people with other comorbidities.

CONCLUSION: The rapid progression of IMD and the high CFR highlights the critical importance of public health messaging to encourage early access to health care for patients with serious illnesses at all times.
