Аннотация

OBJECTIVES: To characterize stillbirths that occurred in pregnant women diagnosed with hypertensive disorders, attended at 10 referral maternity hospitals in Brazil between 2009 and 2018.

METHODS: This cross-sectional study evaluated 919 stillbirths among pregnant women with hypertensive disorders, conducted in 10 maternity hospitals in Brazil. The variables included maternal sociodemographic data, obstetric history, specific type of hypertensive disorder, characteristics of the current pregnancy, fetal death data, and information regarding the diagnostic investigation performed.

RESULTS: The majority stillbirths occurred in the antepartum period (90,9%). Regional and social disparities were observed, affecting mostly the mixed-race and Black population (59.3%) and those with a low level of education (53.7%). Regarding hypertensive diagnoses, preeclampsia was the most frequently observed form (60.1%). The predominant maternal age group in this population was between 20-34 years. Analyzing the causes of death, most were classified as resulting from maternal factors or intrauterine hypoxia, broad categories that do not always indicate the primary cause. Although investigations for infections and diabetes were commonly performed, placental histopathological analysis was conducted in only 31.9% of cases.

CONCLUSION: Hypertensive disorders accounted for a substantial share of stillbirths, yet a large proportion of cases remain without a defined cause. Investigations are still poorly and inconsistently performed, and a high rate of small-for-gestational-age fetuses was observed among stillbirths, suggesting undetected or unmanaged placental insufficiency. These findings highlight the urgent need for improved prenatal care, systematic investigation protocols, and routine placental examination and accurate death certificate reporting.
