Аннотация

INTRODUCTION: Indocyanine Green (ICG), the main tracer for fluorescent staining, can emit near-infrared fluorescence (NIRF) which can improve the intraoperative visualisation of anatomical structures. It has been recently adopted in paediatric urology and can have various applications.

OBJECTIVE: To analyse and compare the applications, key outcomes and complications of using ICG in paediatric urological operations through a systematic review and synthesis of the published literature.

STUDY DESIGN: The Preferred Reporting Item for Systematic Review and Meta-analysis (PRISMA) guidelines were followed to conduct a search of MEDLINE, PubMed, EMBASE and Cochrane databases, from their inception, up to October 2025. The protocol was registered on PROSPERO (CRD420251174642). Eligible studies included reports on patients aged 18 years or younger that underwent ICG-guided urological procedures. Studies were screened and the risk of bias was evaluated using the Newcastle-Ottawa score or JBI Critical Appraisal tools depending on study design. Studies or case series with a sample size of fewer than 5 patients, conference abstracts, letters to the editors, case reports, reviews, and expert opinions were excluded. Studies that included patients older than 18 years of age were also excluded.

RESULTS: 20 studies involving 464 children undergoing various urological procedures were included. ICG had applications in both open and minimally invasive procedures. Procedures performed under ICG guidance included testicular torsion, varicocele repair, renal cysts deroofing, pyeloplasty, complex ureteric reimplantation, hypospadias repair, bladder exstrophy, prostate utricle cyst excision, and partial, simple or radical nephrectomy. In ICG-guided varicocele repair no postoperative hydroceles were observed. In testicular torsion, ICG may aid with the evaluation of testicular viability. The most common intravenous dose of ICG was 0.3 mg/kg, while intratesticular dose ranged from 2.5 mg to 6.25 mg. No ICG-related adverse reactions were reported in any of the included studies of this review.

CONCLUSION: ICG-NIRF-guided surgery appears to be a safe and versatile technique applicable across various paediatric urological procedures. However, further prospective high-quality studies are required to better evaluate its clinical outcomes.

CLINICAL/TRANSLATIONAL APPLICABILITY OF THIS RESEARCH: These findings inform current practice by summarising current applications and dosing of ICG in paediatric urology, while emphasising the need for higher-quality outcome-focused studies.
