Use of autologous platelet gel in female pelvic organ prolapse surgery: a feasibility study.

STUDY

OBJECTIVE

We sought to evaluate whether autologous platelet gel (APG) application during anterior colporrhaphy could increase collagen content of the pubocervical fascia and thereby create a more durable repair.

DESIGN

Prospective cohort study (Canadian Task Force classification II-2).

SETTING

University hospital.

PATIENTS: Nine patients undergoing anterior colporrhaphy.

INTERVENTIONS: We applied APG to the surgical site during anterior colporrhaphy in 9 patients. We collected biopsy specimens from the anterior vaginal wall at surgery and 3 months postoperatively. Collagen content was measured and compared. We evaluated objective and subjective failure rates at 20 months postoperatively.

MEASUREMENTS AND MAIN

RESULTS

No significant increase occurred in collagen content at 3 months. Subjective failure rate was 12.5%, objective failure rate was 66.7%, and 1 (12.5%) patient required reoperation 10 months postoperatively.

CONCLUSION

The application of APG during anterior colporrhaphy does not seem to increase collagen content or durability of the surgical repair.
