Аннотация

OBJECTIVE

To summarise the best evidence of volume management of patients with coexisting cardiac and renal disorders.

DESIGN

An evidence summary, a form of rapid review.

METHODS

A systematic search was conducted in guideline websites, professional association websites and six databases included Web of Science, Embase, CIHAHL, PubMed, CNKI, SinoMed from their inception to 1 April 2024 on volume management for patients with heart and kidney comorbidity. We used the standard that the Fudan University Center for Evidence-based Nursing set based on the Joanna Briggs Institute's (JBI) methods of generating an evidence summary, which includes problem establishment, literature retrieval, literature screening, literature evaluation, evidence summary and grading and the formation of practice suggestions. Two researchers independently carried out the screening, quality appraisal, classification and extraction of the included literature and evidence.

RESULTS

This study selected 36 articles, including two systematic reviews, six randomised controlled trials, four quasi-experimental studies, six cohort studies, four cross-sectional studies, six expert consensus documents and eight guidelines. A total of 37 evidence items were summarised, covering assessment of volume management, strategies of volume management, education, selection of dialysis modality, patients' preferences and cooperation between multidisciplinary teams and patients.

CONCLUSION

Cultural and regional differences should be considered in volume management of patients with heart and kidney comorbidity. Future research should focus on investigating the impact of various volume management strategies-including dialysis-related education for healthcare providers, dialysis modalities, initiation timing, dialysis dose, treatment duration, patient-reported outcomes and interdisciplinary collaboration-on volume-related indicators and symptoms of volume overload in patients.
