Аннотация

OBJECTIVE: In patients with glioblastoma (GBM), the presence of other primary malignancies in different organs appears to be associated with a reduced survival rate compared to patients with GBM alone. This study aimed to evaluate how the presence of additional simultaneous cancers affects the survival outcomes of patients newly diagnosed with GBM.

METHODS: A cohort of 141 adults with newly diagnosed GBM was analyzed, including 19 (13.5%) with concurrent cancer (GBM.cc) and 122 (86.5%) without (GBM.only). Clinical characteristics, treatment, and survival were compared between groups. Cox proportional hazard regression was used to assess the impact of concurrent cancer, age, MGMT promoter methylation status, and sex on survival.

RESULTS: Patients with GBM.cc were older than those with GBM.only (p = .005, while median KPS and treatment approaches were comparable. All tumors were IDH-wildtype; MGMT promoter methylation was present in 56% of cases. Median OS was 16 months in GBM.cc versus 12 months in GBM.only (p = .418), and median PFI was 13 versus 8 months (p = .225). GBM.cc was associated with a reduced hazard of death (p = .012) and a trend toward longer PFI (p = .055). Increasing age independently predicted worse OS (p < .001) and shorter PFI (p = .009), whereas MGMT methylation showed a non-significant trend toward improved OS (p = .201). Sex was not an independent predictor of OS (p = .726) and was not associated with PFI (p = .703).

CONCLUSION: Concurrent cancers do not appear to adversely affect survival outcomes or probabilities compared with GBM alone, within the limits of statistical power. Age remained a strong independent predictor of both PFI and OS, regardless of concurrent cancers or sex.
