Аннотация

BACKGROUND: Sexually transmitted infections (STIs) after sexual abuse are critical problems; however, the prevalence and types of infections in this population have not yet been revealed in Japan. The aim of this study is to clarify the prevalence and the types of STIs in pediatric patients with suspected sexual abuse (SA).

METHODS: This retrospective observational study was conducted in a pediatric tertiary hospital in Tokyo, Japan, where a screening protocol has been implemented to evaluate STIs in pediatric patients with suspected SA. We included patients who were referred to the child protection team for suspected STIs or SA. Eligible patients were aged ≤18 years and visited the hospital between September 2002 and January 2023. To evaluate the positivity rate of screening tests performed for suspected SA, we conducted analyses not only in the overall cohort, but also in the SA screening group, which excluded patients who presented with an established diagnosis or symptoms of STI.

RESULTS: Eighty-one patients were included in this study. Among them, 61 patients were categorized as the SA screening group. Among the overall patients, six patients (7.4%) had diagnosed STIs. All patients had diagnosed Neisseria gonorrhoeae infection. The screening tests found one asymptomatic patient with N. gonorrhoeae in the SA screening group.

CONCLUSION: In this study, only N. gonorrhoeae was identified as an STI in patients with suspected SA. The standardized screening test may have been helpful for diagnosing asymptomatic STIs in this population.
