Аннотация

PURPOSE: Pharmacists are critical clinical partners for patients managing complex medication regimens, particularly those with chronic disease, polypharmacy, or heightened vulnerability. This commentary aims to highlight how these same principles such as careful dose titration, vigilant monitoring, and patient-centered counseling are indispensable in medical cannabis care and to illustrate the impact of pharmacist integration using New York State's tightly regulated medical cannabis program as a primary example.

METHODS: Drawing on nearly a decade of practice in a specialty pharmacy and medical cannabis, this commentary weaves together frontline clinical experience, patient cases, and real-world dispensary practice. It is informed by New York's medical cannabis patient and provider survey, selected clinical and health services studies of pharmacist-led medical cannabis models, and economic analyses of pharmacy-based dispensing. These sources are used to situate individual patient stories within broader patterns of risk, access, and system performance.

FINDINGS: Pharmacist involvement in medical cannabis through on-site consultations, comprehensive medication review, and close collaboration with certifying practitioner functions as a clinical safeguard, reducing the risk of inappropriate dosing, unmanaged drug interactions, and preventable emergency visits. Evidence from state surveys and emerging clinical literature suggests that pharmacist-led consultations improve patient confidence, support adherence, and may lower acute care utilization, particularly among older, medically complex populations. At the same time, surveys in New York and other jurisdictions reveal persistent gaps in clinician and pharmacist cannabis education, reinforcing the need for sustained, structured training.

IMPLICATIONS: As cannabis science and regulation continue to evolve, pharmacists are uniquely positioned to translate complex pharmacology into patient-centered guidance and to anchor medical cannabis programs in accountability and evidence-based care. Preserving and expanding pharmacist roles in dispensaries, investing in cannabis specific education, and incorporating pharmacist into guideline development and real-world data efforts will be essential to ensuring clinical integrity, patient safety, and healthcare system legitimacy as medical cannabis becomes further integrated into mainstream practice.
