Fresh frozen plasma transfusion within 24 h of admission associated with 28-day mortality of pediatric patients with sepsis: A single-center retrospective cohort study.
Аннотация
OBJECTIVES
To investigate association between fresh frozen plasma transfusion within 24 h of admission and 28-day mortality, in-hospital mortality and multiple organ dysfunction syndrome (MODS) in pediatric patients with sepsis.
METHODS
Children with sepsis admitted to the PICU of our hospital, from January 2023 to December 2024 were enrolled in the study. The general socioeconomic data, interventions and laboratory tests with 24 h of admission were recorded. The occurrence of death and MODS during the children's entire hospital stay was recorded. All patients were followed up with for 28 days after admission, with all-cause mortality being recorded.
RESULTS
A total of 262 cases were enrolled, included 81 children with FFP transfusion within 24 h after admitted to PICU. The cases of 28-day mortality, in-hospital mortality and MODS in the FFP transfusion group were 41 (50.6 %), 41 (50.6 %) and 60 (74.1 %), respectively. The cases of 28-day mortality, in-hospital mortality and MODS in the Non-FFP transfusion group were 6 (3.3 %), 7 (3.9 %) and 41 (22.7 %), respectively. The 28-day mortality, in-hospital mortality and MODS morbidity were significantly higher in the FFP transfusion group. FFP transfusion within 24 h was significantly associated with increased 28-day mortality (HR 6.150, 95 % CI 1.770-21.367, P = 0.004) by multivariate Cox regression analysis. 28-day survival rate was significantly higher in the non-FFP transfusion group (HR 19.931, 95 % CI: 10.350-38.404, P = 0.000) by Kaplan-Meier analysis. FFP transfusion within 24 h was significantly associated with increased in-hospital mortality (HR 9.708, 95 % CI 2.003-47.041, P = 0.005) and MODS morbidity (HR 4.351, 95 % CI 1.587-11.926, P = 0.004), respectively by multivariate logistic regression analysis.
CONCLUSIONS
FFP transfusion within 24 h of admission independently and significantly associated with increased 28-day mortality, in-hospital mortality and MODS morbidity in pediatric patients with sepsis.
