The deep plane facelift has achieved near-universal prominence in contemporary surgical discourse, yet the evidence supporting its superiority over well-executed SMAS techniques remains limited. These authors contend that SMAS lifts offer distinct anatomic and practical advantages that are increasingly relevant in contemporary practice. SMAS techniques mobilize the mobile SMAS with a greater degree of excursion than composite techniques permit, preserve rather than sacrifice the structural volume of the SMAS layer, and maintain independent control of skin and soft tissue vectors, allowing for greater surgical freedom.
