Relative hypoxemia during rhytidoplasty.

The results of blood gas analyses in 24 patients who had a rhytidoplasty under local anesthesia indicated that a moderate degree of hypoxemia can be elicited in these patients when they have had standard doses of the usual sedatives. While the acid-base abnormalities were generally corrected spontaneously, the extent of the hypoxemia can be aggravated further by the additional use of diazepam during the operation. Therefore, over-sedation of such a patient during the operation, without a secured airway, must be avoided. Additionally, we recommend deep breathing at frequent intervals, with or w

Cervicofacial rhytidoplasty in Ehlers-Danlos syndrome: hazards on healing.

Ehlers-Danlos syndrome is an uncommon autosomal-dominantly inherited connective tissue disorder marked by excessive skin and joint laxity and easy bruising and ecchymotic tendency that poses a formidable threat to achieving face lift surgery in its normal perspective. Since this surgery is often sought by these patients as a result of premature excessive skin wrinkling of the face, one has to be aware of its possibility during the preoperative examination of the patient. All the possible complications and their management are described to the patient with a case history actually presenting the

The combination of rhytidoplasty and fractional CO2 laser therapy in the treatment of facial aging.

BACKGROUND: Facial aging is a natural biological process with symptoms such as sagging tissue, accentuated lines, furrows, wrinkles, dyschromias, and loss of facial volume. Rhytidectomy alone often fails to achieve a patient's expected goals, which is why there is a high demand for adjunct laser skin treatment. METHODS: From January 2007 to February 2013, a total of 80 patients with skin types Fitzpatrick I-IV underwent large undermined cutaneous flap rhytidectomy and superficial muscular aponeurotic system (SMAS) plicature plus hyaluronic acid injection on the nasolabial folds and lips and c

Innovations in Rhytidoplasty: 480 Cases Validating the Efficacy of the Inner Hemostatic Net.

BACKGROUND: Cervicofacial rejuvenation procedures often face challenges related to soft tissue repositioning, dead space management, and long-term contour stability. The internal hemostatic net (IHN) technique offers a systematic approach to deep fixation that improves surgical outcomes and reduces complications such as hematomas or tissue displacement. OBJECTIVES: The authors of this study aim to describe a reproducible, layered suture technique for cervicofacial lifting that addresses deep neck contouring and flap stabilization through a combined submental and lateral approach, while minimi

Additive Orthodiagonal Rhytidoplasty of the Ergotrid: A Novel Treatment Technique for Severe Dynamic Lip Lines.

Dynamic lip lines pose a challenge in perioral rejuvenation despite the availability of numerous hyaluronic acid dermal filler products and techniques. In this article, the authors introduce additive orthodiagonal rhytidoplasty of the ergotrid, or ADORE, a novel technique for the treatment of severe dynamic lip lines. Based on an objective analysis of treatment outcomes, the authors' technique demonstrates the ability to confer substantial improvement in dynamic lip line severity, with a median reduction in lip line intensity of 79% (average, 70%; SD, 19%) at 1 month and 72% (average, 61%; SD,

Facial nerve in rhytidoplasty: anatomic study of its trajectory in the overlying skin and the most common sites of injury.

The anatomic study of the facial nerve is closely related to the prevention of nerve injury that may occur in facial surgeries. The purpose of this study is to analyze the most susceptible areas in the face regarding the probability of facial nerve injury, based on the demarcation of its trajectory in the overlying skin. Three hundred cadaveric hemifaces were dissected (172 male, 128 female) and the facial nerve trajectory was followed from the stylomastoids foramen to the mimic muscles. The trajectory of the facial branches was delimited in the overlying skin by six diverging lines, with the

A Composite Internal Stabilization Strategy associated with Absence of Postoperative Hematoma After Facelift: A 561-Patient Comparative Series.

BACKGROUND: Postoperative hematoma remains the most common complication following rhytidoplasty, with reported incidences ranging from 0.5% to 8%. Various preventive strategies have been proposed, including blood pressure control, drains, progressive tension sutures and quilting techniques, which have been widely adopted to reduce dead space and improve flap adherence. The authors developed an internal hemostatic network (IHN), combining established suturing principles into a composite technique designed to eliminate dead space and stabilize the facial flap. METHODS: A retrospective comparati

Application of the Auersvald hemostatic net in arteriovenous fistula revision to prevent hematoma formation.

Postoperative hematoma is a frequent complication of arteriovenous fistula (AVF) revision, threatening wound healing and access patency. The Auersvald hemostatic net is commonly used in rhytidoplasty for hematoma prevention. We report its innovative application in a high-risk patient undergoing AVF revision under supratherapeutic anticoagulation, with aneurysmal degeneration and friable tissue from light chain amyloidosis. Given the elevated bleeding risk and drain displacement concerns, the net was applied after hematoma evacuation to obliterate dead space and provide mechanical hemostasis. N

Aesthetic resection of the submandibular salivary gland.

This paper analyzes some pathologies of the submandibular salivary gland that present a special interest for facial aesthetic surgeons. Even up-to-date techniques to correct the aesthetic deformities of the submandibular and cervical regions can be disappointing when the patient presents an increasing volume or ptosis of the submandibular salivary gland. Sometimes, even though the submandibular gland has not been preoperatively identified, it may become evident postoperatively, compromising the submandibular and cervical contour because the musculocutaneous flaccidity has been corrected and th

Reconstruction of Scalp Anterior Hairline with Tissue Expansion and Skin Flap.

INTRODUCTION: Cutaneous defects involving the frontal region and anterior hairline of the scalp can result from congenital or acquired conditions. The negative esthetic impact can cause disturbances in the psychic and social sphere of the patient, causing problems in interpersonal relationships and in the body image itself. The use of skin expanders is usually effective in this region due to the bone base providing support and stability for its use. CASE REPORT: We describe the case of a 64-year-old woman submitted to reconstruction of the anterior hairline of the scalp due to scar sequelae a

Revisiting the Topographic Anatomy of the Marginal Mandibular Branch of Facial Nerve Relating to the Surgical Approach.

BACKGROUND: The marginal mandibular branch (Mbr) of the facial nerve is vulnerable to damage during rhytidoplasty, surgical reduction of the mandibular angle, parotidectomy, and excision of the submandibular gland. OBJECTIVES: The authors sought to map the Mbr and determine the relationship between the number of Mbr offshoots and the course of the Mbr. METHODS: The Mbr was examined in 29 hemifaces from 12 embalmed and 4 fresh cadavers (10 males, 6 females; mean age, 73.7 years). RESULTS: The Mbr was located ≤5 mm from the gonion (Go) in 24 of 29 hemifaces (82.8%) and ≤10 mm from the interse

Tissue sealants may reduce haematoma and complications in face-lifts: A meta-analysis of comparative studies.

UNLABELLED: The use of tissue sealants has increased among different surgical specialities. Face-lift and rhytidoplasty may cause several complications such as haematoma, ecchymosis, oedema, seroma, skin necrosis, wound dehiscence and wound infection. However, administration of tissue sealants may prevent the occurrence of some complications. We performed a meta-analysis of studies that compared tissue sealant use with controls to evaluate the outcomes. A systematic literature search was performed. The primary outcome was the incidence of haematoma. Secondary outcomes were wound drainage amoun

Discerning a smile - The intricacies of analysis of post-neck dissection asymmetry.

INTRODUCTION: Iatrogenic facial nerve palsy is distressing to the patient and clinician. The deformity is aesthetically displeasing, and can be functionality problematic for oral competence, dental lip trauma and speech. Furthermore such injuries have litigation implications. Marginal mandibular nerve (MMN) palsy causes an obvious asymmetrical smile. MMN is at particular risk during procedures such as rhytidoplasties, mandibular fracture, tumour resection and neck dissections. Cited causes for the high incidence are large anatomical variations, unreliable landmarks, an exposed neural course an

Understanding The Forehead Through The Endo-Coronal Forehead Lift.

Correction of the aged forehead must address the soft tissue redundancy while minimizing the issues of incisions, paresthesia, and hair loss. In 2006, the senior author (N.A.G.) devised the Endo-Coronal Forehead Lift. A calculated, limited coronal strip is resected based on measurements of soft tissue redundancy. The subgaleal surgical dissection is done endoscopically. The arcus marginalis is preserved, eliminating risk of eyebrow malpositioning. In addition, the resection of the calculated limited strip and endoscopic dissection restores prior anatomy while reducing scarring, numbness, and h

Endoscopic midfacial rejuvenation.

Early facial rejuvenation focused largely on the upper and lower thirds of the face. More recently, improvements in understanding of midfacial aging and anatomy have paralleled the development of endoscopic and minimally invasive surgical techniques. The midface is now understood to include both the lower lid subunit and the cheek down to the nasolabial fold. Many surgical techniques for midface rejuvenation have been used, including skin tightening with direct excision, skin-muscle flaps, isolated fat pad transposition, and subperiosteal lifting. The methods of endoscopic subperiosteal midfac

Management of the aging forehead and brow.

The appearance of the forehead and brow conveys both a state of aging as well as the dynamic status of human emotion. Maintaining a youthful and pleasant appearance of this region is often integral to facial rejuvenation. Conditions that are often addressed include transverse rhytids and glabellar frown lines, ptotic eyebrows, skin deterioration, and volume loss. Surgical treatments to improve the forehead/brow might be open, endoscopic or combined. Common nonsurgical methods used to improve the region include neuromodulation, volume replacement, ablative and nonablative skin resurfacing, as w
