OBJECTIVE: To investigate the mediating role of dyadic coping in the relationship between patient empowerment and quality of life among patients with acute ischemic stroke (AIS) and their spouses.

METHODS: A total of 283 dyads of AIS patients and their spouses were recruited between May and October 2025 using convenience sampling. A cross-sectional survey was conducted using a general information questionnaire, the dyadic coping inventory (DCI), the patient perceived empowerment scale (PPES), and the stroke-specific quality of life scale (SS-QOL). Pearson correlation analysis was used to examine associations among variables. Structural equation modeling and the Bootstrap method were employed to test the mediating effect, with empowerment as the independent variable, dyadic coping as the mediating variable, and quality of life as the dependent variable.

RESULTS: A total of 566 questionnaires (283 dyads) were distributed and all were valid, yielding a 100% effective response rate. Correlation analyses showed significant positive associations among DCI, PPES, and SS-QOL within both patients and spouses, as well as significant cross-partner correlations. In the spouse pathway, empowerment influenced quality of life indirectly through dyadic coping, with an indirect effect size of 0.666 (95% CI: 0.403-0.997), accounting for 35.5% of the total effect; the direct effect remained significant, indicating partial mediation. In contrast, the indirect effect in the patient pathway was not significant.

CONCLUSION: The pathways through which empowerment improves the life quality of AIS patients and their spouses differ. Dyadic coping plays a partial mediating role in the spouse pathway but not in the patient pathway.
