OBJECTIVE: To describe the implementation of a care pathway integrating in-person clinical consultations by the Primary Care Pharmacist (PCP) in a health center, assessingorganizational feasibility and user perception. This study represents the preliminary phase for subsequent evaluation of health outcomes through a clinical trial.
DESIGN: Descriptive, cross-sectional observational study, with prospective recruitment of chronically polymedicated patients during implementation.
SETTING: Urban health center in the Campo de Gibraltar Oeste Health Management Area (Andalusia, Spain), characterized by high care demand and social-health vulnerability.
PARTICIPANTS: Chronic patients receiving ≥10 medications. Exclusion criteria were terminal illness, severe cognitive impairment, or palliative care.
INTERVENTIONS: Implementation of a multidisciplinary network focused on Structured Medication Review conducted by the PCP through scheduled appointments. Protocols for coordination and use of Electronic Health Record (EHR) tools were established. The review included a in-person interview to assess adherence and knowledge, witheducational interventions and medication recommendations recorded on the shared REDCap® platform.
MAIN OUTCOME MEASURES: Number and type of prescription-related issues per patient, acceptance of PCP recommendations by family physicians, and patient satisfaction.
RESULTS: A total of160 polymedicated chronicpatients were attended and 1,600 prescriptions were evaluated; 3.8% showed incidents, 46.10% beingpotentially inappropriate treatments. Family physicians accepted 67% of recommendations. Satisfaction was high: 90% were very satisfied with kindness, 87% with consultation time, and 96% would recommend the intervention.
CONCLUSIONS: Integration of the PCP was feasible and well accepted. REDCap® ensured traceability and facilitated care coordination.
