INTRODUCTION: Palliative care in Kerala has become a leading model in India over recent decades. We studied the prevalence and associated factors of caregiver burden among informal caregivers of palliative care patients in Kerala, India.
METHODS: We conducted a cross-sectional study with 200 informal caregivers of palliative care patients in Pathanamthitta district in Kerala. Caregiver burden was assessed by using the Achutha Menon Centre-Caregiver Burden Inventory (AMC-CBI). We collected details of informal caregivers on socio-economic and demographic characteristics, morbidity pattern, stress and sleep. Information on patients was also collected. Multivariate analysis was done to find out the factors associated with caregiver burden.
OUTPUTS: Using AMC-CBI, 20.5% had low caregiver burden, 56.5% had moderate and 23.0% had severe caregiver burden. Stress was found to be the most significant risk factor associated with caregiver burden (OR: 11.77, 95% CI: 3.91-35.4). Male caregivers were more likely to have severe caregiver burden compared to their female counterparts (OR: 5.31, CI:1.48-19.02). Those caring for adults younger than 70 years (OR: 3.59, CI: 1.38-9.33), and with lower levels of education (OR: 3.07, CI: 1.05-8.97) experienced significantly higher caregiver burden compared to their counterparts. Disturbed sleep due to caregiving was also significantly associated with caregiver burden.
DISCUSSION: The study underscores the substantial burden on family caregivers, who face physical, emotional, and social challenges. A strengthened multifaceted approach to addressing caregiver burden in the state is warranted, especially considering the large proportion of older adults in Kerala.
