BACKGROUND: Sleep, physical activity, screen time, and dietary behaviours play an important role in childhood health. This study aimed to examine longitudinally the associations between adherence to sleep guidelines and other health behaviours and sociodemographic factors among children under 5 years.

METHODS: A secondary analysis was conducted using data extracted from two linked randomised controlled trials in Sydney, Australia: Study 1 (children from 0 to 2 years, N = 1155) and Study 2 (children from 2 to 5 years, N = 662). Data on sleep (6 months - 5 years), physical activity, screen time, and dietary behaviours (1 - 5 years) were collected. Multilevel logistic regression models were built.

RESULTS: Most children met sleep guidelines (70% - 88%). From ages 3 to 5 years, children were more likely to meet sleep guidelines among those who met physical activity (AOR (adjusted odds ratio) 1.36, 95% CI 1.01 - 1.86), screen time (AOR 1.54, 95% CI 1.07 - 2.21), and fruit consumption guidelines (AOR 1.53, 95% CI 1.03 - 2.27), and those from higher incomes families (AOR 1.41, 95% CI 1.04 - 1.90). Children of first-time mothers were less likely to meet sleep guidelines (AOR 0.66, 95% CI 0.49 - 0.90). Meeting sleep guidelines was not associated with physical activity, screen time, breastfeeding, or bottle feeding from ages 1 to 2 years.

CONCLUSIONS: Adequate sleep is associated with improved physical activity, screen time and fruit consumption, and socioeconomic advantages in preschool-aged children. Promoting children's health requires a holistic approach targeting multiple health behaviours, prioritizing support for children of low-income families and first-time mothers.
