STATEMENT OF PROBLEM: Segmental mandibulectomy followed by microvascularized free fibular flap (FFF) reconstruction and dental implant rehabilitation is a standard treatment for ameloblastoma. However, data on the length of the microvascularized FFF related to long-term peri-implant bone level change are sparse.

PURPOSE: The purpose of this retrospective clinical study was to investigate the impact of microvascularized FFF length on the peri-implant bone resorption indicated by changes in the crown-to-implant (C/I) ratio after 10 years of functional loading.

MATERIAL AND METHODS: A total of 65 patients with mandibular ameloblastoma who underwent mandibular reconstruction with microvascularized FFF and received dental implants were analyzed, with C/I ratios measured at baseline and long-term follow-up. To mimic the image distortion, panoramic radiographs were used to measure clinical C/I ratios, fibula flap length (FL), and mandibular width (MW). The FL/MW ratio was used to standardize flap length. The difference in C/I ratio from prosthesis delivery from 2015 to 2025 was calculated. Linear regression analysis was performed to evaluate the relationship between FL/MW and peri-implant bone resorption (α=.05).

RESULTS: A total of 244 implants in 65 patients were analyzed. The mean ±standard deviation FL/MW was 0.446 ±0.172, and the mean C/I change was 1.712 ±2.260. A significant positive correlation was found between FL/MW and C/I change (P=.002). The generalized estimating equations (GEE) model indicated that each unit increase in FL/MW predicted a 12.769 decrease in C/I change after being adjusted for sex.

CONCLUSIONS: A close relationship was found between the length of the microvascularized FFF and the change in peri-implant bone level. The longer microvascularized FFF reconstruction may result in less peri-implant bone resorption under long-term occlusal loading in patients with ameloblastoma who received a microvascularized FFF reconstruction.
