Аннотация

Nail involvement in pediatric psoriasis is a clinically significant yet often underrecognized feature associated with increased disease severity and risk of psoriatic arthritis. In this Letter to the Editor, I comment on the study by Tavares et al., which evaluates high-frequency ultrasound findings, including Power Doppler activity and structural alterations, in pediatric nail psoriasis. The study provides valuable insight into the potential role of imaging in detecting subclinical nail involvement through systematic correlation of clinical and sonographic findings. From a dermatologic perspective, several methodological and technical limitations warrant consideration. The absence of healthy control groups and age-specific normative data limits the ability to distinguish pathological findings from physiological vascular variability. The use of highly sensitive Doppler settings may further reduce specificity, particularly in pediatric populations with inherently increased perfusion. Additionally, the application of adult-derived classification systems and the lack of longitudinal validation restrict the clinical interpretability of the findings. While high-frequency ultrasound represents a promising noninvasive tool, its integration into routine pediatric dermatologic practice requires further validation. Future studies should prioritize standardized imaging protocols, pediatric-specific criteria, and correlation with clinically meaningful outcomes to define its true clinical utility.
