OBJECTIVE: To assess the impact of early childhood caries (ECC) and caries-related pulpal involvement on the oral health-related quality of life (OHRQoL) of Turkish preschool children aged 12 to 72 months using the Early Childhood Oral Health Impact Scale (ECOHIS).

MATERIALS AND METHODS: A total of 354 children aged 12 to 72 months with ECC and their parents/caregivers participated in this cross-sectional study. Oral examinations were performed using the decayed, missing, and filled teeth (dmft) index and the pufa index, which records pulpal involvement (P/p), ulceration (U/u), fistula (F/f), and abscess (A/a), followed by a structured interview with parents/caregivers to complete the validated Turkish version of the 13-item ECOHIS questionnaire.

RESULTS: The prevalence and mean scores of ECC (dmft) and pufa were 77.1%/5.77 ± 4.78 and 50.8%/1.54 ± 2.11, respectively. Increasing severity of ECC was significantly associated with poorer OHRQoL (P 0.001). Children with pufa > 0 had significantly higher ECOHIS scores (15.64 ± 7.69) compared to those with pufa = 0 (7.16 ± 6.67) (P 0.001). Multiple linear regression analysis revealed that pulpal involvement was the strongest predictor of OHRQoL deterioration (B = 7.656, 95% CI 5.790 to 9.523, P 0.001), explaining 28% of the variance in ECOHIS scores.

CONCLUSION: ECC and pulpal involvement adversely affected the OHRQoL of Turkish preschool children and their families. The pufa index provides a clinically meaningful complement to the dmft index in assessing the true burden of untreated caries.
