BACKGROUND: Epinephrine nasal spray (Neffy®) demonstrates pharmacodynamic effects comparable to epinephrine auto-injector (EpiPen®) and is considered safe. Neffy was approved in Japan on September 19, 2025, as an "Adjunctive treatment for anaphylactic reactions" and launched on February 12, 2026. It is expected to become a new option for initial anaphylaxis treatment.
OBJECTIVE: To investigate the acceptance of the new formulation and the background factors influencing that acceptance among pediatric patients and their parents who carry EpiPen before Neffy's distribution.
METHODS: A questionnaire was delivered online or on paper to pediatric patients carrying EpiPen and their parents/guardians who visited our hospital between November 18 and December 26, 2025.
RESULTS: Although awareness was still low before Neffy's distribution, the majority of patients and parents were interested in Neffy. Parents who were more accepting of Neffy reported higher anxiety about carrying EpiPens and had lower confidence in their use. In several cases, a discrepancy existed between the patient's and their parent's acceptance.
CONCLUSION: Moving forward, treatment selection should be based on accurate information and in consultation with both patients and their parents.
