Аннотация

This review synthesizes current perspectives on outcomes following childhood psychological trauma. When children are exposed to stressful events with similar objective parameters, they may follow distinct developmental trajectories, including resilience, post-traumatic growth, post-traumatic stress disorder (PTSD), or more severe personality disorders. Trauma is a dynamic process shaped by interactions among factors at three levels. The analysis addresses initial conditions of trauma exposure, including pre-traumatic factors such as attachment quality, neurobiological and genetic predispositions (notably polymorphisms and epigenetic regulation of the FKBP5 and NR3C1 genes), temperament, cognitive style, and macrosocial context. It also examines objective characteristics of the traumatic event, such as typology, the nonlinear nature of the dose-effect relationship, chronological parameters, the moderating role of age, and subjective primary assessment within the transactional stress model. The review further explores trauma processing mechanisms, including acute stress responses and their divergence, the progression from acute stress disorder to PTSD, peritraumatic reactions as predictors, and neurobiological mechanisms of consolidation, such as allostatic load and dysregulation of the hypothalamic-pituitary-adrenal axis. Emphasis is placed on outcomes identified in longitudinal trajectory studies, recent efforts to validate complex PTSD in children according to ICD-11, findings on post-traumatic growth, and pathways leading to borderline personality disorders. The need for an integrative, multilevel model to predict outcomes and facilitate proactive, differentiated care is substantiated.
