UNLABELLED: Testicular torsion is a urological emergency resulting from twisting of the spermatic cord, leading to acute ischemia. Although surgical detorsion is essential to restore blood flow, it is frequently accompanied by ischemia-reperfusion (IR) injury, which induces oxidative stress, inflammation, apoptosis, and potential infertility. Quercetin (Q) is a naturally occurring flavonoid with potent antioxidant and anti-inflammatory properties; however, its therapeutic application is limited by poor aqueous solubility and low bioavailability.

AIM: This study aimed to compare the protective effects of free quercetin and quercetin-loaded lipid nano capsules (QLNC) on seminiferous tubules in a rat model of testicular IR injury.

METHODS: QLNC prepared using the phase inversion method exhibited a mean particle size of 48.23 ± 2.1 nm with a high encapsulation efficiency. Adult male rats underwent testicular torsion for 1 h followed by 2 h of detorsion. Quercetin or QLNC was administered at an equivalent dose of 20 mg/kg 30 min prior to detorsion. Blood and testicular tissue were collected for biochemical, histopathological, ultrastructural, and immunohistochemical evaluations.

RESULTS: Compared to the IR and IR-Q groups, QLNC treatment markedly preserved seminiferous tubule architecture, reduced spermatogenic-cell depletion, and attenuated interstitial edema, vascular congestion, and inflammatory infiltration. This was supported by significant improvements in Johnsen's and Cosentino scores. Immunohistochemical analysis revealed a pronounced reduction in inducible nitric oxide synthase expression. Additionally, QLNC significantly restored testosterone levels and reduced oxidative stress and inflammatory biomarkers. Overall, QLNC demonstrated superior protective efficacy compared to quercetin, preserving near-normal histological and biochemical parameters.

CONCLUSION: These findings highlight QLNC as a promising nanotherapeutic strategy for mitigating testicular ischemia-reperfusion injury.
