Аннотация

Raynaud Phenomenon (RP) is a peripheral vascular disease that results in episodes of vasospasm of the microcirculation of hands and feet, which causes ischemia, pain and functional deficiency. Traditional systemic treatments of RP, such as calcium channel blockers and phosphodiesterase type-5 inhibitors, often have limitations due to perfusion-rate-based drug delivery, lack of consistency in local effect and dose-related adverse effects. Microvascular dysfunction plays a central role in determining drug distribution and therapeutic response in Raynaud's Phenomenon. These considerations have led to growing interest in localized drug delivery strategies (especially topical and transdermal nano-enabled systems). This review critically discusses pathophysiological aspects of RP that affect the delivery of drugs and focuses on endothelial dysfunction, perfusion instability, microvascular remodeling and cold-induced skin barrier changes. The systems of topical nano-strategies and transdermal nanocarrier-based systems are systematically analysed regarding their penetration depth, duration of action, dependence on systemic circulation, safety profile and translational feasibility. Although topical nano formulations provide a quick and temporary relief of symptoms, transdermal nanocarriers surpassing the limitations of transport in the bloodstream due to the ability to create sustained dermal drug delivery and targeted vascular effects. The review further addresses the permeation enhancement strategies, nanocarrier design and regulatory issues, the new role of personalized transdermal therapy in response to inter-patient variability. In general, nano-enabled transdermal delivery is a promising pathophysiology-related approach to enhancing treatment response in Raynaud's Phenomenon, as long as the design of formulations and their clinical validation are informed by disease-specific delivery constraints.
