Are the SUN Criteria Sufficient for Tubercular Uveitis? Findings From a Multinational Registry Study.

PURPOSE: The Standardization of Uveitis Nomenclature (SUN) criteria were introduced in 2021 to standardize the classification of uveitis subtypes. We evaluated the real-world applicability of these criteria for tubercular uveitis (TBU) using a computational definition mapped to the Ocular Autoimmune Systemic Inflammatory Infectious Study (OASIS) registry. METHODS: This observational retrospective cross-sectional study analyzed real-world baseline data from 372 clinician-diagnosed TBU cases within the multinational OASIS registry, all collected from 17 tertiary centers across 8 countries: Indi

Associations Between Prior Lung Diseases and Risk of Lung Cancer in Populations With No Smoking History: A Systematic Review and Meta-Analysis.

BACKGROUND: Individuals who do not smoke account for a growing proportion of lung cancer (LC) incidence, but the impact of prior lung diseases on lung cancer risk in this population remains unclear. RESEARCH QUESTION: What is the association of TB, asthma, and chronic bronchitis (CB) with lung cancer risk among individuals who do not smoke? STUDY DESIGN AND METHODS: PubMed and Embase were searched from inception through July 15, 2025, for studies examining the association between TB, asthma, or CB with lung cancer risk in populations older than 18 years who never smoked. Summary effect estim

Tuberculosis Masquerading as Behcet's Disease-Pseudo Bechet's Syndrome: A Case-Based Review of Literature.

Behcet's disease is a chronic, multisystem variable vessel vasculitis characterised by recurrent oral and genital ulcers, ocular inflammation and a wide range of systemic manifestations. Pseudo-Behcet's syndrome refers to a condition that mimics these clinical features but arises from distinct etiologies. We present a case of a 33-year-old male with year-long recurrent oral ulcers and intermittent abdominal pain, followed by scrotal ulcers, severe fatigue, weight loss and appetite loss. Chest imaging demonstrated bilateral consolidations with cavitation and right-sided pleural effusion, prompt

Management of adverse events in TB care and active TB drug safety monitoring.

Tuberculosis (TB) remains the leading cause of death from a single infectious agent, with 10.8 million cases reported in 2023. While treatment is generally effective, both drug-susceptible (DS-TB) and drug-resistant TB (DR-TB) regimens are associated with adverse events (AEs) that compromise adherence and outcomes. In this viewpoint, we highlight the most clinically relevant AEs: hepatotoxicity, cutaneous reactions and ocular toxicity in DS-TB; and linezolid-associated neuropathy, myelosuppression, QT prolongation and hepatotoxicity in DR-TB. We argue that structured safety monitoring and inte

Multidisciplinary management of thyroid eye disease (TED) in patients with latent tuberculosis or chronic hepatitis B.

BACKGROUND: Intravenous methylprednisolone (IVMP) is a first-line treatment for active moderate-severe or sight-threatening thyroid eye disease (TED), but the presence of systemic infections such as latent tuberculosis (TB) or chronic hepatitis B virus (HBV) act as relative contraindications due to the risk of reactivation. There is limited data to guide clinicians in this high-risk group. METHODS: A retrospective case series was conducted involving five TED patients with relative contraindications to immunosuppression. Each patient included had a EUGOGO severity of moderate-to-severe. Multid

Postmarketing adverse events of biologics and Janus kinase inhibitors in patients with atopic dermatitis.

BACKGROUND: New targeted drugs, including biologics and Janus kinase inhibitors (JAKi), have been approved for the treatment of atopic dermatitis (AD). Given their widespread clinical use, a comprehensive real-world study of their adverse events (AEs) is warranted. OBJECTIVES: This study aims to characterize of new targeted drugs related AEs, and to compare the AE profiles of biologics and JAKi. METHODS: Disproportionality metrics, including the reporting odds ratio, proportional reporting ratio, information component, and empirical Bayesian geometric mean, were employed to determine AE sign

Chasing shadows: case series of six posterior segment manifestations of ocular tuberculosis.

BACKGROUND: Ocular tuberculosis (OTB) is difficult to confirm microbiologically, so diagnosis often rests on characteristic posterior segment phenotypes supported by systemic immunologic evidence and therapeutic response. This case series from a high-prevalence setting illustrates the heterogeneity of posterior OTB and provides practical diagnostic anchors, treatment approaches, and complication surveillance that are directly applicable to routine uveitis care. CASE DESCRIPTION: Six immunocompetent males (28-46 years) presented with posterior disease entities that included: choroidal granulom

Longitudinal Observation by Optical Coherence Tomography in Patients Treated with Ethambutol: A Systematic Review and Meta-Analysis.

Background: The retinal changes caused by ethambutol are not clear in patients with the administration of ethambutol and without ethambutol-induced optic neuropathy (EON). The aim of this systematic review is to estimate the changes in retinal nerve fiber layer (RNFL) and ganglion cell layer and inner plexiform layer (GCIPL) thicknesses measured by optical coherence tomography (OCT) in patients with mycobacterial infection treated with ethambutol and not suffering from EON. Methods: A systematic review of articles was conducted by searching PubMed, Embase, and Web of Science until November 202

From antibiotic to peptide siderophore conjugates as modular strategies against multidrug-resistant bacteria.

SUMMARYMultidrug-resistant (MDR) bacterial infections continue to outpace therapeutic innovation, undermining the efficacy of conventional antibiotics and exposing the limitations of the current drug pipeline. In this review, we critically examine the translational viability of peptide-based antimicrobials and siderophore conjugates-two classes of therapeutics often promoted as next-generation solutions against MDR pathogens. Rather than reiterating their mechanistic promise, we dissect the structural assumptions that underpin their design and expose the clinical blind spots that have hindered

Orbital tuberculosis mimicking a vascular emergency: A case report of superior ophthalmic vein engorgement.

We report a rare case of orbital tuberculosis (O-TB) in an elderly diabetic patient that mimicked a variety of vascular, infectious, and inflammatory orbital diseases. A 73-year-old woman with uncontrolled diabetes, unilateral swelling of the left eye, proptosis, and binocular diplopia, as well as left superior ophthalmic vein (SOV) engorgement on an orbital CT scan, was referred to our center to rule out a carotid-cavernous fistula (CCF) or a cavernous sinus thrombosis (CST). An MRI, MRV, and cerebral angiography ruled out both diagnoses. Nasal endoscopy revealed septal necrosis, and biopsy d

Familial Granulomatous Uveitis with Arthritis Suggestive of Blau Syndrome: A Multigenerational Case Series from India.

PURPOSE: Blau syndrome is a rare autosomal dominant autoinflammatory disorder characterized by granulomatous dermatitis, arthritis, and uveitis resulting from gain-of-function mutations in the NOD2 gene. In India, the condition is frequently underrecognized or misdiagnosed due to significant clinical overlap with juvenile idiopathic arthritis associated uveitis and early onset sarcoidosis. Reports of familial Blau syndrome are rare. This report describes familial granulomatous uveitis suggestive of Blau syndrome, with emphasis on ocular manifestations, diagnostic challenges and treatment outco

Improving Equitable Access to Antenatal Care in China: Challenges and Potential Innovative Approaches.

Achieving equitable antenatal care (ANC) is fundamental to China's 'Healthy China 2030' agenda and its universal health coverage (UHC) commitments. Despite measurable national progress, substantial urban-rural and regional disparities in ANC access persist, driven by a complex interplay of financial, geographic, institutional, and digital barriers that disproportionately affect rural, low-income, and migrant populations. This analysis examines the current landscape of ANC in China, identifying core structural challenges including the enduring legacy of the hukou (household registration) system

Incidence, risk factors, and mortality impact of chronic kidney disease in middle-aged Finns: a 22-year population-based cohort study.

BACKGROUND: Although many studies have estimated the prevalence of chronic kidney disease (CKD) in the general population, few have assessed its cumulative incidence over a long time using both estimated glomerular filtration rate (eGFR) and albuminuria or assessed how CKD occurrence influences mortality. METHODS: This prospective, population-based cohort study included 1144 Finnish participants aged 40–65 years at baseline, who were followed from 1996 to 2019. The study assessed the prevalence of CKD stages 1–5 at baseline, 10 years, and 22 years, and examined risk factors for incident CKD, d

Mycobacterium tuberculosis-Associated Serpiginous-Like Choroiditis Mimicking Central Serous Chorioretinopathy: A Case Report.

We report a case of a 40-year-old man with no comorbidities who presented with progressive vision loss in the right eye. Fundoscopy revealed peripapillary and posterior pole serpiginous-like lesions without overt vitritis. The central optical coherence tomography revealed subretinal fluid consistent with central serous chorioretinopathy, prompting treatment with spironolactone 100 mg daily for two months, which did not improve the condition. In the meantime, a positive tuberculin skin test indicated Mycobacterium tuberculosis infection, and a diagnosis of tuberculosis serpiginous-like choroidi

A PTPRO-Related Five-Gene Blood Transcriptional Signature with Diagnostic Potential for Tuberculosis.

Background/Objectives: Tuberculosis (TB), caused by Mycobacterium tuberculosis (MTB), remains a major global health problem. Drug resistance and the limitations of sputum-based diagnostic methods highlight the need for additional host-response biomarkers. Protein tyrosine phosphatase receptor type O (PTPRO) has been implicated in inflammatory signaling and macrophage immune regulation, but its relationship with TB-related host transcriptional responses remains unclear. This study aimed to identify and preliminarily evaluate a PTPRO-related blood transcriptional signature with potential relevan
