Comparison of menstrual blood and endometrial biopsy as specimens in the diagnosis of female genital tuberculosis: a systematic review.

BACKGROUND: Female genital tuberculosis (FGTB) continues to be a significant problem worldwide. Endometrial biopsy (EB) is the most effective sample for detecting FGTB. However, biopsy is invasive and causes unwarranted suffering. Furthermore, a few studies have investigated non-invasively obtained menstrual blood (MB) as an alternative diagnostic sample for detecting FGTB. Hence, in this study, we are assessing MB as an alternative sample to EB for diagnosing FGTB. METHOD: A systematic literature search was conducted using the electronic databases PubMed, Scopus, Web of Science, and Embase un

Incidental diagnosis of genital tuberculosis in a prepubertal girl presenting with acute urinary retention: a case report.

BACKGROUND: Genital tuberculosis affecting prepubertal girls is extremely uncommon, posing a great diagnostic challenge. It is a form of extrapulmonary tuberculosis usually seen in the reproductive age group. It can lead to significant health problems, including infertility, pelvic pain, menstrual irregularities in women, or amenorrhea. Genital tuberculosis is even rarer when coupled with an imperforate hymen, thus presenting a rare and diagnostically challenging presentation. Imperforate hymen is an uncommon obstructive congenital abnormality of the female reproductive tract with an incidence

Endometrial tuberculosis increases miscarriage risk after IVF/ICSI compared with non-specific chronic endometritis.

BACKGROUND: Chronic endometritis (CE) has two principal subtypes—non-specific chronic endometritis (NSCE) and endometrial tuberculosis (ETB). Both can adversely affect embryo implantation. Although ETB is generally more severe, its low incidence has limited comparative data on assisted reproductive outcomes versus NSCE. A direct comparison of pregnancy outcomes following in vitro fertilization (IVF) or intracytoplasmic sperm injection-embryo transfer (ICSI-ET) between patients with ETB and those with NSCE would help clarify whether differential management strategies are warranted and provide m

A rare case of urinary tuberculosis in a 14-year-old: Diagnosis and surgical management.

INTRODUCTION: Urogenital tuberculosis (UG-TB) accounts for 30-40% of extrapulmonary TB (EPTB) cases and is extremely rare in the pediatric age. It is usually diagnosed in advanced stages, and its treatment mostly requires antitubercular therapy combined with surgery. CLINICAL CASE: 14-year-old patient with no previous history of lung disease, who presented recurrent UTIs (sterile pyuria) and chronic lower urinary tract symptoms. On ultrasound right hydronephrosis was seen, and a CT scan revealed vertebral lesions that led to the diagnosis of TB. Urinary symptoms improved after combined treatm

Concurrent Bladder Urothelial Carcinoma and Prostate Tuberculosis: A Case Report.

INTRODUCTION: Tuberculosis is associated with an increased risk of bladder cancer; however, the simultaneous occurrence of urogenital tuberculosis and bladder cancer is rare. Primary prostate tuberculosis is relatively uncommon in patients with urogenital tuberculosis. CASE PRESENTATION: A 68-year-old man presented with gross hematuria and irritative voiding symptoms. Cystoscopy revealed a bladder tumor. The PSA level was elevated (8.32 ng/mL), and MRI findings suggested prostate cancer. Chest radiography and CT indicated pulmonary tuberculosis, which was confirmed by sputum mycobacterial cul

A case of imported infection in China: Initially treatment-unresponsive schistosomiasis coinfection with bladder tuberculosis.

Schistosoma haematobium, endemic to sub-Saharan Africa, causes urogenital disease, differing from Schistosoma japonicum, which affects the hepatointestinal system and is the only endemic schistosome in China. A Chinese male with persistent hematuria after prolonged occupational exposure in Angola was initially attributed to S. japonicum. Following failed treatment, metagenomic sequencing confirmed S. haematobium infection, and subsequent urethral resection detected Mycobacterium tuberculosis DNA, establishing concurrent bladder tuberculosis. This case highlights the need for molecular diagnost

The many faces of extrapulmonary tuberculosis: A case series.

Extrapulmonary tuberculosis (EPTB) is the clinical entity that is gaining more recognition. Still, it may emerge in various ways, making it difficult to diagnose. Even though pulmonary tuberculosis is the most common variety, EPTB may affect many organ systems and often mimics other diseases, therefore postponing diagnosis, and treatment. Five rare types of EPTB, including testicular tuberculosis, esophageal tuberculosis, psoas abscess with spinal involvement, tuberculous mastitis, and parotid gland tuberculosis, are the main subjects of this case series. All patients who were HIV-negative wer

Porcelain bladder with ascending ureteral calcifications and seminal vesicle calcifications: CT imaging features of advanced urogenital schistosomiasis.

Urogenital schistosomiasis remains endemic in many regions of Africa and is a well-recognized cause of chronic lower urinary tract pathology. Longstanding infection may lead to bladder wall fibrosis, calcification, ureteral strictures, and hydronephrosis. We report the case of a middle-aged man presenting with chronic lower urinary tract symptoms and intermittent hematuria. Non-contrast computed tomography demonstrated circumferential bladder wall calcification consistent with a porcelain bladder, associated with bilateral ureteral mural calcifications and right-sided ureteropyelocaliceal dila

Prostatic tuberculosis incidentally discovered during transurethral resection of prostate with subsequent diagnosis of rectal and pulmonary involvement: a case report.

BACKGROUND: Tuberculosis (TB) is a globally prevalent infectious disease, including in Taiwan. Prostatic TB is a rare manifestation of genitourinary tuberculosis (GU-TB), which is the third most common extrapulmonary form of the disease. However, due to its insidious onset and non-specific symptoms, prostatic TB is often diagnosed late. CASE DESCRIPTION: We report a case of a 72-year-old male patient who presented with lower urinary tract symptoms (LUTS) and painful scrotal swelling. Following transurethral resection of the prostate (TURP), histopathological examination revealed prostatic TB.

[Tuberculosis of the male genital organs as the first manifestation of generalized tuberculosis].

Urogenital tuberculosis is difficult to diagnose in the general healthcare network because it has no pathognomonic symptoms, and there is no screening for urogenital tuberculosis, which complicates its timely diagnosis. This article presents a clinical case of generalized tuberculosis that first manifested as a lesion of the external genital organs. A high level of clinical suspicion for tuberculosis prompted special investigations using widely available methods. Real-time PCR diagnostics allowed to identify tuberculosis of the male genital organs as the first manifestation of generalized tube

MAG3 SPECT/CT Detection of Vesicocolic Fistula in Genitourinary Tuberculosis.

Enterovesical fistula is a rare complication of genitourinary tuberculosis and may be difficult to diagnose, particularly in the setting of impaired renal function. We report a 41-year-old woman with chronic kidney disease and bilateral obstructive uropathy secondary to treated genitourinary tuberculosis. 99m Tc MAG3 renography demonstrated a nonfunctioning left kidney and preserved right renal function without obstruction. Delayed images revealed abnormal tracer propagation from the urinary bladder into bowel loops. SPECT/CT localized the tracer within the colon, confirming a vesicocolic fist

Adrenal and Metabolic Hormonal Axes Shape Anti-Tuberculosis Immune Responses in Human Immunodeficiency Virus-Tuberculosis Coinfection.

BACKGROUND: Tuberculosis (TB) remains a leading cause of mortality worldwide among infectious agents, and HIV increases the risk of developing into active disease. HIV-TB coinfection impairs immune responses, while chronic inflammation and infection-associated stress activate neuroendocrine pathways that deeply impact immune homeostasis. Adrenal steroids such as cortisol, dehydroepiandrosterone (DHEA) and its metabolites, along with metabolic hormones like leptin and adiponectin, have emerged as critical regulators of immune function, although their role in TB pathogenesis, particularly in co-

A Case of Diagnostic Delay: Autoimmune Adrenalitis Mimicking Functional Gastrointestinal Disorder.

Autoimmune adrenalitis is an under-recognized cause of primary adrenal insufficiency in young individuals. Its nonspecific presentation can often mimic gastrointestinal or psychiatric conditions, leading to delayed diagnosis and life-threatening adrenal crisis. We report the case of a 23-year-old male having known treated tuberculous meningitis history who presented with recurrent episodes of vomiting, generalized weakness, and altered behavior. He was hypotensive, drowsy, and had metabolic derangements, including hyponatremia and hyperkalemia. Laboratory investigations revealed low serum cort

Transport Stress Induces Neuroimmune Dysregulation and Exacerbates Mycobacterium Tuberculosis Infection in Mice.

Transport stress (TS) significantly impacts immune function and infection, of which the cellular and molecular mechanisms remain elusive. In this study, we investigated the effects of TS on immune responses and its role in exacerbating Mycobacterium tuberculosis (Mtb) infection. TS induced anxiety-like behaviors, disrupted hypothalamic-pituitary-adrenal (HPA) axis homeostasis, altered hormone secretion patterns, and led to dysregulation of stress response genes in mice. Single-cell RNA sequencing of the spleen revealed severe immune dysfunction upon TS, characterized by inhibition of Th17 diff

A systematic review of adverse effects associated with systemic corticosteroids in the management of leprosy.

BACKGROUND: Leprosy is a chronic infectious disease caused by Mycobacterium leprae (M. leprae) and Mycobacterium lepromatosis (M. lepromatosis), primarily affecting the skin and peripheral nervous system. Leprosy is complicated by immune-mediated reactions which are risk factors for nerve damage and disability. Systemic corticosteroids are the mainstay of therapy; however, prolonged use in chronic or recurrent reactions carries significant risks but the evidence on the short- and long-term adverse effects of corticosteroids in leprosy is limited. We conducted a systematic review to evaluate co

Imaging Modalities in Tuberculosis.

Tuberculosis (TB) remains a major global health challenge, with increasing prevalence of multidrug-resistant and extrapulmonary forms complicating diagnosis and management. Imaging plays a pivotal role in the early detection, characterization, and treatment monitoring of TB, particularly when clinical or microbiological findings are inconclusive. Modalities such as chest radiography, computed tomography (CT), ultrasound, magnetic resonance imaging (MRI), and 18F-fluorodeoxyglucose positron emission tomography combined with CT (18F-fluorodeoxyglucose [18F-FDG PET-CT]) provide critical insights
