Diagnosis and treatment of bilateral primary branchiogenic carcinoma: A case of malignant transformation of branchial cleft cyst.

Branchial cleft cysts are common congenital lateral neck masses, with 95 % originating from the second branchial cleft. Although most cysts are benign, there are rare instances of malignancy. Here, we present a rare case of primary branchiogenic carcinoma originating from both sides of the neck in a 68-year-old male. Through a detailed analysis of this rare bilateral primary branchiogenic carcinoma, we present the complexity of diagnosing such rare phenomena and the limitations of existing diagnostic methods, emphasizing the need to improve diagnostic methods and the importance of further rese

Management of a Piriform Sinus Fistula With Chronic Neck Infection in an Adult.

OBJECTIVES: A pyriform sinus fistula (PSF) is a rare congenital anomaly due to failed obliteration of the third or fourth pharyngeal pouch. Diagnosis and management of PSF remains controversial. We present a case of PSF and discuss its diagnosis and management. METHODS: Case report and literature review. RESULTS: A 26-year-old female with a chronic left lateral neck abscess was diagnosed with a left PSF. She underwent surgery through a combined approach to close the openings of the PSF, internally and externally. We first placed a blunted ET tube into the internal opening of the PSF. We then

Characterisation of Kürsteiner canals of parathyroid: imparting relevance to a one-and-a-quarter-century-old concept.

AIMS: Kürsteiner canals (KC) were described at least 125 years ago as pharyngeal pouch embryological remnants of parathyroid and thymic development. While considered precursors for a subset of parathyroid cysts and salivary heterotopias (SH), they remain enigmatic. We now define a comprehensive phenotype of KC remnants and investigate their role in a spectrum of parathyroid lesions. METHODS AND RESULTS: `Sixty-two cystic and 22 non-cystic parathyroid lesions (73 patients) were retrieved from our institutional archive (2011-23) and evaluated for the presence of KC and prevalence of KC phenotyp

Complications after fine-needle aspiration cytology and core-needle biopsy in benign head & neck neoplasms.

INTRODUCTION: Benign neck masses are one of the most common causes of clinical consultation in head & neck (H&N) clinics. Fine needle aspiration cytology (FNAC) and core needle biopsy (CNB) have become among the main strategies for the diagnostic work-up of palpable and non-palpable neck lumps. Moreover, numerous studies have established the safety, high diagnostic yield, and added value of image-guided H&N biopsies, which play an important role in diagnosis, staging, and treatment planning, and can obviate the need for surgery for many patients. Nonetheless, despite the success of both techni

Minimal Invasive Approach to The Branchial Neck Cyst.

Branchial cleft cysts are congenital epithelial cysts, which arise on the lateral part of the neck. They may not present clinically until later in life. Surgical excision is a definitive treatment for branchial cleft cysts. This article presents the clinical report where the big inflamed cysts were excised under local anesthesia. It also presents guidelines for minimally invasive surgery under local anesthesia, which can be used in adequately selected cases.

Unique encounter of a branchial cleft cyst compressing the great auricular nerve in an adult: a case report.

Branchial cleft cysts, arising from developmental anomalies of the branchial apparatus, represent a rare entity in adult populations. This case report presents a unique instance of a 32-year-old female with a second branchial cleft cyst prominently involving the great auricular nerve, an undocumented finding in the existing literature. The patient initially presented with progressive left-sided neck swelling, accompanied by localized pain radiating to the auricle and headaches, without systemic symptoms. Diagnostic imaging confirmed a cystic mass adjacent to vital neck structures, necessitatin

Ectopic thymus in the neonatal period: A case report.

The thymus, a vital immune organ in humans, plays a crucial role in the differentiation and development of T cells. Its normal development commences around the fourth week of gestation, originating from the ectoderm of the third branchial cleft and the endoderm of the third parotid pouch. By the 7-8th week of gestation, the primary thymus migrates towards the central axis. However, various factors may disrupt this process, leading to congenital anomalies such as incomplete descent of thymus tissue. Persistence of residual thymus tissue can result in the formation of ectopic thymus. Although ec

[Neck lumps: Early Detection with Dental Magnetic Resonance Imaging?].

Cervical lymphadenopathy can indicate a range of diseases, including infections, inflammation, and neoplastic processes. Accurate diagnosis is crucial for identifying the underlying cause and determining appropriate treatment. Diagnostic procedures include clinical examination, ultrasound, fine needle aspiration, biopsy, and imaging techniques. While conventional imaging methods such as X-ray and computed tomography (CT) have proven useful, they exhibit limitations in sensitivity and specificity, especially for soft tissue structures. Magnetic resonance imaging (MRI) provides superior soft tis

Cavernous Lymphangioma of the Neck: An Extraordinary Case.

Cervical lymphangioma is a congenital malformation of the lymphatic system that shows a predilection for the head and neck regions. They are most common in children between the ages of three and five and are rarely present in adults. While over 90% of lymphatic malformations are congenital, they can also manifest later due to factors such as trauma, infection, tumors, or medical procedures. They are often asymptomatic, but symptoms, if present, vary depending on the location of the tumor. Imaging techniques such as ultrasonography and computed tomography can aid in accurately mapping the exten

Right-Sided Fourth Branchial Cleft Cyst in a Neonate: A Case Report and Literature Review.

Anomalies of the fourth branchial cleft are exceedingly uncommon, presenting with a diverse array of clinical manifestations. The majority of branchial cleft anomalies, approximately 95%, are of the second type, with a mere 2% attributed to the fourth type. The latter is notably more prevalent on the left side, with reports indicating an 85% incidence. Herein, we report the case of a neonate presenting with a right-sided fourth branchial cleft cyst, which manifested at birth and subsequently underwent progressive enlargement, resulting in airway and esophageal compromise. The management strate

Papillary Thyroid Carcinoma within a Second Branchial Cleft Cyst. A Diagnostic and Treatment Challenge.

INTRODUCTION: Lateral cervical cystic masses are a relatively common occurrence in the otolaryngology practice, which are often attributed to second branchial cleft cysts. However, there should always be a high suspicion for malignancy until proven otherwise. CASE PRESENTATION: We present a clinical report of a lateral cervical cyst surgical excision where histopathological examination has revealed features of a branchial cleft cyst harboring a papillary thyroid carcinoma. This led to the clinically latent thyroid primary which was treated through thyroidectomy with selective neck dissection

A Case of Rare Fourth Branchial Arch Anomaly: Presentation and Surgical Management.

This report presents a case of a six-year-old male patient with recurrent left-sided neck abscesses who presented four times over a span of two years. At each presentation, the child had developed left-sided neck swelling, pain, and fevers that required hospital admission. In the patient's most recent admission in 2020, a fourth branchial cleft anomaly was confirmed on CT, and the patient was taken to the operation theatre for ultrasound-guided aspiration and cauterisation of the sinus fistula tract under direct pharyngoscopy. This allowed for definitive management of the area and, to date, ha
