From Rescue to Risk: Challenges and Controversies in Pushing the Boundaries of Organ Utilization by Machine Perfusion.

The ongoing shortage of donor livers has prompted the use of extended criteria donors (ECD) and the development of ex-situ machine perfusion (MP) to expand the transplantable organ pool. MP has transformed liver transplantation (LT) by improving preservation, enabling better logistics, and allowing real-time viability assessment that is, a process aimed at predicting liver function after transplantation based on MP parameters. Although viability assessment has been proposed to safely expand donor pool and improve LT outcomes, it remains inconsistent. This article will summarize currently avail

Tracing the Historical Roots of Health Inequity Among People With Intellectual and Developmental Disabilities in the United States: Impact of Models, Advocacy, Legislation, and International Context.

The history of inequity for people with intellectual and developmental disabilities (IDD) in the United States is a compelling story of social perceptions of disabilities, advocacy, legislation, and international influences. Although my focus here is on health inequities, the contributing factors equally influenced where people with IDD lived, worked, and recreated. This article provides a public health perspective for viewing intertwining advances in models of disability, the power of family and self-advocacy, and US legislation to create infrastructures for community-based training, services

Factors that contribute to collision avoidance behaviours involving a single pedestrian versus a group of pedestrians in a natural environment.

Along busy walking paths many pedestrians walk in a group, thus challenging the collision avoidance behaviours of both the group and individual pedestrians. Here we sought to determine what factors contribute to these behaviours in a real-world environment. We hypothesized that pedestrian group size, spatial separation between parties, presence of an additional interferer not involved in the interaction, pedestrian distractedness, constraints to a pedestrian's mobility (i.e., holding or pushing an object), and pedestrian age would significantly influence the likelihood and extent of path devia

Dual stressors impair bioenergetics and antioxidant defenses in Etroplus suratensis.

Marine biota is confronted with the simultaneous pressures of anthropogenic drivers, such as ocean warming and engineered nanoparticles, that pose a compounded threat to aquatic ecosystems and productivity. The present study investigated the combined impacts of ocean warming and zinc oxide nanoparticle (ZnO-NP) on the eco-physiological response of Etroplus suratensis. Essential bioenergetic parameters, comprising ingestion rate, absorption rate, respiration rate, ammonia excretion, and scope for growth (SfG), were evaluated in conjunction with oxidative stress biomarkers, including superoxide

A novel behavioral paradigm using mice to study predictive postural control.

Postural control circuitry performs the essential function of maintaining balance and body position in response to perturbations that are either self-generated (e.g., reaching to pick up an object) or externally delivered (e.g., being pushed by another person). Human studies have shown that anticipation of predictable postural disturbances can modulate such responses. This indicates that postural control could involve higher-level neural structures associated with predictive functions, rather than being purely reactive. However, the underlying neural circuitry remains largely unknown. To enabl

Pulmonary Vein Stenosis Medical Therapy: Current and on the Horizon.

Pulmonary vein stenosis (PVS) is a severe vascular obstructive lesion historically associated with poor survival. As outcomes have improved in the current era, PVS has become a chronic disease requiring frequent surgeries, catheter-based interventions, and experimental therapies to prevent progression and recurrence. Despite this, PVS survival remains low and has pushed experimental research into new therapies into the forefront. In this paper, we briefly examine the pathophysiology and etiology of PVS and provide a more comprehensive review of computational and animal modeling of PVS and ther

"The weight of words": a phenomenological study of Chinese neonatologists' lived experiences of delivering bad news.

BACKGROUND: Delivering bad news in the Neonatal Intensive Care Unit (NICU) constitutes a profound ethical and emotional challenge. In China, this complexity is intensified by a defensive medical culture and strained physician-family dynamics. This study explores the lived experiences of Chinese neonatologists navigating these high-stakes interactions to uncover the phenomenon's essential structure. METHODS: A descriptive phenomenological study was conducted. Using a purposive sampling strategy, 19 neonatologists from a tertiary NICU in Western China were recruited. Data were collected through

Experiences and Perceived Influence of the Artificial Intelligence-Based Health Education Accurately Linking System (AI-HEALS) on Health Behaviors Among Patients With Type 2 Diabetes: Qualitative Study.

BACKGROUND: The management of type 2 diabetes requires sustained self-management across diet, physical activity, medication adherence, and blood glucose monitoring; however, maintaining these behaviors in daily life remains difficult for many patients. Artificial intelligence-enabled and mobile health interventions have shown promise in supporting diabetes education and self-management, but evidence on how patients actually experience and use such systems in real-world primary care remains limited. OBJECTIVE: This study aimed to explore patients' experiences with the Artificial Intelligence-B

Influence of anatomical geometry and fiber dispersion on Achilles tendon Mechanics: A finite element modelling approach.

Tendons adapt their structure and mechanical behavior in response to mechanical loading, where mechanical stimuli, such as strain, are believed to play a pivotal role. Recent studies employing finite element modeling have provided detailed insights into the distribution of strain across tendon structures under various loading conditions. However, most researchers have adopted highly simplified geometries, which are unable to capture the inherent strain heterogeneity in tendons, thereby emphasizing the need for anatomically accurate models. The current study explores how both geometry and fiber

Stochastic approximation to contrastive learning.

Contrastive learning pulls positive samples (similar examples) closer and pushes negative samples (dissimilar examples) away to learn a meaningful mapping from inputs to outputs. It is found in a broad range of applications in computer vision that range from image classification to object detection to video processing and has a similarly broad impact and relevance in natural language processing, audio and speech, graphs, recommendation systems, and multimodal learning. Although contrastive learning done right typically achieves state-of-the-art results, there are major limitations with traditi

Thermal niches within the nest contribute to the long-term development of individual differences in metabolic rate in an altricial mammal.

Early life circumstances can affect an individual's phenotype into later life. In some polytocous altricial mammals, individuals' occupancy of micro niches within the litter huddle during early postnatal life contributes to the emergence of phenotypic variation among them across development. Rabbit pups with a greater birth mass typically occupy thermally more favorable positions towards the center of the huddle, whilst their lighter littermates are pushed to the less well insulated, cooler periphery. We therefore asked whether occupying different thermal niches within the huddle during early

Trust, autonomy, and informed consent: a qualitative investigation of youth experiences accessing contraception in Canada.

We sought to understand, "What are the contraception access experiences, beliefs, attitudes, knowledge and needs of youth in Canada, from the perspectives of youth and youth service providers?" We explore person-centred contraception care and highlight the importance of fostering trust, autonomy, and informed consent. We conducted one-on-one semi-structured interviews with youth (aged 15-25) and healthcare providers (HCPs) across Canada, representing a diversity of rural and urban settings, gender, age, and ethnicities. Using social constructivist grounded theory, we identified key themes rela

Incubator baby side-shows: the history and future of neonatology.

The care of infants has a somewhat sordid history, not without its side-show elements, political intrigues, and amazing success. Abandonment of infants and outright infanticide was not uncommon in ancient times. The Roman Church, however, was instrumental in beginning to rescue abandoned infants. Over time, greater recognition was afforded newborns and young children with the advent of dispensaries and hospitals created for the purpose of administering to this population. France was a focal point for the evolution of the modern care of newborns, with development of incubators at the end of the

Axon Diameter Mapping in the Living Human Brain with Ultra-High-Gradient Diffusion MRI at 500 mT/m Gradient Strength.

Pushing the resolution limit of axon diameter mapping in the living human brain requires higher gradient strength than is currently available on most clinical MRI systems. The noninvasive quantification of axon diameter not only enables the exploration of axonal damage in a wide range of neurological disorders but also provides fundamental insights into axonal organization and conduction patterns of white matter tracts. The goal of this study was to evaluate the sensitivity of axon diameter mapping to small diameter axons using the next-generation Connectome MRI scanner (Connectome 2.0), which

[Peer support for promoting mental health: A descriptive analysis of target groups and support content].

AIM OF THE STUDY: Mental illnesses are on the rise both internationally and in Switzerland, pushing psychiatric care facilities to the limit of their capacity. As a result, there is a growing need for low-threshold psychosocial support that accompanies, stabilizes and supports those affected in their everyday lives. This is precisely where the "Psycho-Social Services" (PSD) project of the GLEICH UND ANDERS Schweiz foundation comes in. The aim of this study was to provide a descriptive, empirical analysis of the PSD project. METHODOLOGY: A descriptive analysis of the project was carried out fo

Impacts of cooperative hunting and Allee effect on the dynamics of a three-species system with sexually reproducing top predators.

This study investigates the complex dynamics of a tri-trophic predator-prey system incorporating the Allee effect in prey, cooperative hunting by mesopredators, and sexual reproduction in the apex predator. Using bifurcation analysis and numerical simulations, we examine how these factors-individually or combinedly-shape ecosystem stability, both in the presence and absence of the top predator. In the absence of the apex predator, species survival depends critically on prey's productivity and mesopredator cooperation strength: lower values lead to extinction, while moderate prey growth support

Perspectives on educating lifelong learning in postgraduate orthopaedic training: an interpretivist qualitative study.

BACKGROUND: Lifelong learning (LLL) is a core competency in medical education, reflecting the need for clinicians' knowledge and skills to remain current. While postgraduate curricula emphasise LLL, a gap remains in understanding how it is interpreted and enacted within orthopaedic training. Little is known about how trainees and trainers conceptualise LLL and how these perspectives shape their views on its development and assessment in orthopaedic practice. Addressing this is necessary to align curricular expectations with the orthopaedic clinical context. METHODS: An interpretivist qualitat

'Today, I'll manage, but not all days!' Adult refugees' meaning-making of learning and coping with trauma and other emotional distress while in language training programmes.

Background: Adverse life experiences evoke thoughts and emotions that may negatively affect adult refugees' language learning. This may significantly impact the ability to establish their lives in the resettlement country. However, little attention has been paid to adult refugee learners' own understandings of managing distress following trauma and forced migration while in language learning contexts. Objective: This qualitative study aims to explore how adult refugee learners deal with distressing memories, emotions, and thoughts, and how they make meaning of their coping and learning efforts,

Morality cut both ways: The role of cognition and emotion in attitude moralization and demoralization.

Moralized attitudes motivate social change, political participation, and charitable giving, but at the same time, hinder compromise, conformity, and acceptance of procedural solutions to conflict. Although considerable research has explored the consequences of moralized attitudes, the processes that lead to attitude moralization and demoralization are less clear. Across six studies (N = 4,464), we contrasted the moralization of attitudes about male circumcision (typically viewed as a matter of preference or convention in the United States) with the demoralization of attitudes about female circ
