Indications for flexible bronchoscopy in elderly patients (≥ 75 years) with hypoxemic respiratory failure not eligible for intensive care: Expert opinion based on a Delphi study.

BACKGROUND: The indications for flexible bronchoscopy (FB) remain poorly defined in elderly patients (≥ 75 years) with hypoxemic respiratory failure (HRF) (O₂ ≥ 5 L/min) who are not eligible for intensive care. The lack of specific data highlights the need to develop evidence-based recommendations in this setting. STUDY DESIGN AND METHODS: A Delphi study was conducted among pulmonologists from the Groupe d'Endoscopie Thoracique et Interventionnel Francophone (GETIF). The first-round questionnaire included 18 statements. Consensus was defined as ≥ 70% of experts expressing agreement or disagre

A Rare Encounter With Peripheral Dentinoma: Clinicopathological Insights.

Odontogenic tumors comprise a diverse group of lesions arising from tooth-forming tissues and often show overlapping clinical, radiographic, and microscopic features, creating diagnostic challenges. Peripheral dentinoma is an exceptionally rare dentin/dentinoid producing odontogenic tumor characterized by dysplastic dentin or dentinoid formation, with odontogenic epithelial and ectomesenchymal components that may vary according to the histologic pattern. We report a case of peripheral dentinoma in a 24-year-old male patient in the lower right posterior tooth region. Radiographic examination re

Biomaterial characteristics and biofluid composition differentially modulate protein adsorption and macrophage phenotype on common surgical polymers with proteome-transcriptome concordance.

Dysregulated macrophage responses to common surgical polymers contribute to implantable medical device failure. While biomaterial characteristics are known to influence macrophage behavior, the contribution of the local biological milieu and the adsorbed proteins remains poorly understood. Here, we investigate how biomaterial physicochemical properties and biofluid composition differentially modulate adsorbed proteomes and macrophage transcriptomes on common surgical polymers as well as explore the link between the latter two. We first quantify the stiffness, roughness, wettability and protein

Retained Colorectal Foreign Body After Suspected Chemical Incapacitation: A Surgical and Forensic Challenge.

Retained rectal foreign bodies are an uncommon but clinically significant surgical emergency. We report the case of a 74-year-old male with multiple medical comorbidities who presented with progressive rectal pain and obstipation several days after an episode of suspected drug-facilitated assault. Plain radiography identified a large metallic aerosol can, and attempts to retrieve it under general anesthesia using surgical clamps and a magnetic probe failed. Intraoperative fluoroscopy demonstrated that the can had migrated proximal to the rectosigmoid junction into the left colon, precluding tr

Deep brain stimulation as neuroimmune-metabolic modulation in Parkinson's disease and beyond: tissue response, glial remodeling, and disease modification.

Deep brain stimulation (DBS) is an established symptomatic therapy for Parkinson's disease (PD), yet its biological effects extend beyond modulation of abnormal basal ganglia circuitry. This review conceptualizes DBS in PD as a neuroimmune-metabolic intervention acting across the electrode-tissue interface, glial networks, peripheral immunity, and activity-dependent metabolic systems. PD is characterized by convergent immune and metabolic vulnerability. These processes create a disease environment in which electrical stimulation may interact with inflammation, excitotoxicity, bioenergetic stre

Bezoar ileus in patients with previous abdominal surgery: an observational comparison of conservative and surgical management.

BACKGROUND: Evidence on the management of bezoar ileus in patients who have already undergone abdominal surgery is limited. We reviewed conservative and surgical pathways in clinically selected patients, with particular attention to patient selection criteria and how each pathway was defined. METHODS: We reviewed 80 patients with CT-confirmed bezoar ileus and previous abdominal surgery who were treated between January 2010 and December 2021. Twenty-six completed conservative treatment, and 54 underwent surgery. In practice, conservative treatment was used for clinically stable patients withou

Monolithic tissue sealant membrane with covalently‑anchored NHS‑activated gelatin adhesive moieties on flexible PLCL substrate: preparation, characterization and biological performance.

Clinically available tissue sealant membranes are limited by high swelling, poor flexibility, insufficient adhesion and high cost, restricting their use in complex surgeries. Herein, a novel monolithic embedded tissue sealant membrane (IIIb) was constructed via UV‑initiated copolymerization. In this optimized formulation, NHS‑modified gelatin adhesive moieties were covalently anchored onto one side of flexible PLCL‑containing substrate, rather than simple physical lamination of two independent films (control sample IIIa prepared by method a). Although exhibiting a two‑layer‑like appearance, th

Endoscopic techniques for removal of intraorbital foreign bodies - a clinical series.

BACKGROUND: Management of intraorbital foreign bodies (IOFBs) is challenging due to the confined space and proximity of neurovascular and ocular structures. The increasing adoption of endoscopic techniques has expanded minimally invasive treatment options. We report operative corridors, indications, and outcomes of various endoscopic approaches in a consecutive series. METHOD: We analyzed 16 patients with IOFBs treated between 2015 and 2023 at a tertiary university center. All procedures were performed using exclusively endoscopic techniques. Eleven IOFBs were metallic and five organic. Surgi

Giant Mixed-Material Gastric Bezoar With a Synchronous Jejunal Bezoar in a Young Woman With Pica and Autism Spectrum Disorder: A Case Report.

Bezoars are conglomerates of indigestible material retained within the gastrointestinal tract and may produce obstruction, ulceration, bleeding, or perforation. Mixed-material bezoars containing nonfood foreign bodies are uncommon and may be particularly challenging in patients with persistent pica. We report a 21-year-old woman with autism spectrum disorder (ASD), mild intellectual disability, previously diagnosed pica, and a seizure disorder treated with carbamazepine who presented after nearly one month of recurrent fever, colicky abdominal pain, progressive distension, early satiety, const

Swallowed dental prosthesis causing late oesophageal obstruction and tracheo-oesophageal fistula in a young patient.

A 25-year-old man presented to the emergency department after a rugby accident having swallowed a dental plate. Initial assessment at the time of ingestion, including a chest radiograph, was unremarkable and the plate was presumed to have been expectorated. Five years later he presented with progressive dysphagia, recurrent food bolus obstruction, and haemoptysis. Computed tomography of the thorax performed 5 years later revealed circumferential oesophageal thickening and a large inflammatory mass around a retained radio-opaque foreign body at the level of the carina, with an associated trache

A retrospective study of flexible bronchoscopic retrieval of airway foreign bodies in adults.

Unlike in children, foreign body aspiration in adults is an uncommon occurrence. The clinical presentation is often nonspecific, with a myriad of symptoms and radiological abnormalities. These cases can mimic chronic lung conditions like asthma and chronic obstructive pulmonary disease, thereby delaying definitive diagnosis and timely interventions. We conducted a retrospective analysis of the bronchoscopy suite database (January 2022 to December 2025) to identify cases subjected to flexible bronchoscopy for suspicion of a foreign body.  Data on demographics, clinical symptoms, radiological fi

Monitoring metabolic health in the adolescent population of Rarotonga: a multi-year cross-sectional study from 2016 to 2023.

BACKGROUND: The Cook Islands, a small Pacific nation, carries a high burden of obesity (75%) and non-communicable disease (NCD) among adults. Primary prevention before adulthood, including school-based strategies, is a priority. Local evidence is crucial but challenging in resource-limited, low-population settings. This study presents data on the prevalence of risk factors associated with metabolic syndrome (MetS) among adolescents in Rarotonga, the main island of the Cook Islands. METHODS: A multi-year cross-sectional survey of NCD risk factors in Year 9 students (median age: 13.8 years) bet

Functionalized bioresorbable biliary stents: materials design, coating engineering, and clinical translation.

Biliary strictures remain a major therapeutic challenge because conventional plastic and self-expandable metal stents are limited by occlusion, repeat intervention, migration, and tumor ingrowth. Bioresorbable biliary stents offer temporary mechanical scaffolding and, when functionalized, can also provide local therapeutic delivery without permanent foreign-body retention. This review focuses on the engineering decisions that determine whether these devices can move beyond descriptive promise toward indication-specific translation. We compare polymeric and emerging metallic biodegradable platf

Occult Penetrating Trauma: A STEMI Mimic Hiding in Plain Sight.

BACKGROUND: We report a case of acute pericarditis caused by an occult penetrating foreign body manifesting as an ST-segment elevation myocardial infarction mimic. CASE SUMMARY: The patient initially presented with chest pain, increased cardiac troponin I, and ST-segment elevation. Following emergency intravenous thrombolysis and urgent coronary angiography, an alternative etiology was suspected. Subsequent verification of the medical history and imaging examinations ultimately identified the culprit: a nail. The patient received prompt treatment, and the pericardial foreign body was successf

A Fish Bone Penetrating the Esophagus Into the Left Atrium.

BACKGROUND: Fish bone migration into the left atrium is extremely rare and carries high mortality. CASE SUMMARY: A 68-year-old woman presented with chest pain, fever, and dyspnea 5 days after swallowing a fish bone; she had attempted to dislodge it with a rice ball. Contrast-enhanced computed tomography showed a 2.9-cm linear foreign body penetrating the mid-esophagus into the left atrium. Transthoracic echocardiography revealed a 2.3-cm echogenic mass in the left atrium without pericardial effusion. Emergency surgery under cardiopulmonary bypass extracted the fish bone, debrided a large vege

Adult esophageal foreign body and food impaction: a video-based guide to endoscopic and nonendoscopic management.

Esophageal foreign body (FB) ingestion and food impaction represent critical and increasingly common emergencies in adult populations, often requiring prompt diagnosis and intervention to prevent severe adverse events. This review explores the epidemiology, risk factors, and clinical management of esophageal FB and food bolus impactions, with an emphasis on endoscopic retrieval strategies. Although conservative and pharmacologic treatments, including glucagon, effervescent agents, and muscle relaxants, are occasionally used, they exhibit limited efficacy and should not delay endoscopic interve

Case Report: A case of severe chronic infection of an intraperitoneally placed composite mesh treated with complete mesh explantation and primary tissue repair using the anterior component separation technique.

INTRODUCTION: The introduction of synthetic mesh has fundamentally changed the management of ventral hernias by substantially reducing recurrence rates compared with primary suture repair. However, mesh implantation carries an inherent risk of foreign body-related complications, most notably mesh infection. Although uncommon, mesh infection following ventral hernia repair is a potentially devastating complication that often requires complex surgical management. Treatment strategies range from attempts at mesh preservation to complete mesh explantation followed by abdominal wall reconstruction
