Pathogenic Mechanisms of Two Novel Variants in the GJB1 Gene.

BACKGROUND AND AIMS: Charcot-Marie-Tooth disease X-linked Type 1 (CMTX1), caused by GJB1 mutations, presents diagnostic challenges due to phenotypic heterogeneity and novel variants. This study reviews clinical data from 2021-2024 and investigates the mechanisms of two novel GJB1 variants. METHODS: We retrospectively analyzed eight male patients, evaluating clinical signs, electrophysiology, and sural nerve biopsies. Pathogenicity of novel variants was assessed via bioinformatics, ChimeraX molecular modeling, and HeLa cell models focusing on protein localization and channel function (scratch-

Scratch Collapse Test and Patient-Reported Outcomes After Lower Extremity Nerve Decompression Surgery.

INTRODUCTION: Entrapment neuropathies of the lower extremity are uncommon, frequently misdiagnosed or diagnosed late, and have no well-established diagnostic protocol. The scratch collapse test (SCT) has been described for carpal and cubital tunnel syndrome and, in the lower extremity, has been evaluated only for common peroneal nerve compression; whether it can localize uncommon or multiple sites of lower extremity nerve compression, and how it compares with Tinel's sign at those sites, is unknown. This study evaluates the feasibility of the SCT for localizing the site of nerve compression an

EAN-ESCMID guidelines on the diagnosis and management of encephalitis in adults caused by infection.

BACKGROUND: Encephalitis is difficult to diagnose and manage, posing a challenge to clinicians. Since the last European guidelines were published in 2010, novel causes have emerged, diagnostic processes have evolved and treatments assessed which need to be reflected in updated guidelines. OBJECTIVE: We aimed to produce recommendations for the diagnosis and management of infectious encephalitis in adults based on current evidence to equip clinicians to improve patient outcomes. METHODS: These guidelines were developed jointly with the support of two European specialist medical organisations.

Borrelia miyamotoi exceeds Borrelia burgdorferi sensu lato in human exposure despite the opposite in ticks: an integrated serological, entomological and molecular study in southeastern Kazakhstan.

Lyme borreliosis (LB) and hard tick-borne relapsing fever caused by Borrelia miyamotoi are emerging tick-borne infections, yet in Kazakhstan routine LB diagnosis relies on single-tier enzyme immunoassay (EIA) without confirmatory testing, and B. miyamotoi is absent from national diagnostic protocols. In 2024-2026, 1300 participants were enrolled in the Ixodes persulcatus-endemic Almaty and Jetisu regions and the non-endemic Zhambyl and Turkistan regions, across diverse clinical and tick-exposure groups. All sera were screened for IgM and IgG to both pathogens by multi-antigen protein arrays, w

Metabolic and oxidative stress biomarker responses to enhanced levels of natural radioactivity in three edible bivalve species.

Despite the known contribution of industrially derived natural radioactivity, also known as Naturally Occurring Radioactive Material (NORM), to animal and human radiation dose via seafood consumption, its ecotoxicological effects on marine organisms remain poorly understood. In particular, the physiological responses of bivalve molluscs to enhanced natural radioactivity have not been studied under controlled conditions. We conducted laboratory exposure experiments on three edible bivalve species, Cresstoderma edule, Mytilus edulis and Magallana gigas, selected for their ecological relevance an

The prognostic value of HER2 in gastroesophageal adenocarcinoma - a meta-analysis.

BACKGROUND: HER2 amplification and overexpression are predictive markers for HER2-targeted therapies in gastroesophageal adenocarcinoma. However, unlike in breast cancer, the prognostic significance of HER2 status in this disease remains uncertain, with conflicting evidence across studies. METHODS: We conducted a systematic review and meta-analysis of studies reporting hazard ratios (HRs) for overall survival (OS) based on HER2 status, identified through PubMed, Scopus, and Web of Science. Eligible studies included HER2-positive and HER2-negative patients with gastroesophageal adenocarcinoma,

Systematic screen of PKR reveals genetic variants that broadly evade divergent viral pseudosubstrate inhibitors.

Evolutionary arms races can arise at the contact surfaces between host and viral proteins, producing dynamic spaces in which genetic variants are continually pursued even while protein function must be maintained. A striking case is protein kinase R (PKR), an innate immune protein that detects viral replication and halts protein synthesis by phosphorylating the initiation factor eIF2α. PKR is targeted by many viral antagonists, including pseudosubstrate inhibitors encoded by poxviruses and ranaviruses that mimic eIF2α. We previously found that the eIF2α-binding surface of human PKR is highly m

Case Report: dupilumab for severe pruritus in pediatric recessive dystrophic epidermolysis bullosa-a longitudinal clinical assessment and literature review.

Recessive dystrophic epidermolysis bullosa (RDEB) results from biallelic COL7A1 variants and causes trauma-induced blistering, scarring, and progressive extracutaneous complications. Severe pruritus may intensify skin injury through an itch-scratch cycle. We report a 7-year-old, 25-kg girl with infantile-onset generalized blistering, severe pruritus, mucosal and nail involvement, scarring, and digital contractures. Histopathology showed a subepidermal blister. Sequencing identified heterozygous COL7A1 c.5605G>C (p.Gly1869Arg) and c.5532+5G>A, inherited from different parents and therefore occu

One Health genomic surveillance of ticks found on community-dwelling non-human 'farm' animals to identify microbes associated with tick borne illnesses in Eastern Ontario.

Companion animals, such as dogs and cats, and wildlife are recognized as sentinel species in One Health surveillance due to periodic exposure to ticks and infection by tick-borne pathogens (TBPs). Surveillance of this type is a strategy that captures a broad range of genetic, immunological, and behavioral factors that affect tick and disease dynamics. In contrast, non-human animals (hereafter referred to as 'animals') that are traditionally farmed for their meat and other products have been largely overlooked as sentinel species. Community-dwelling farm sanctuary residents, such as cattle, pig

Ephrin Signaling Patterns Sensory Neurons During Tissue Homeostasis in Planarians.

Eph and ephrin genes encode receptor-ligand pairs that mediate contact-dependent cell signaling and are essential for nervous system development. However, less is known about the role of Ephrin signaling during adult tissue homeostasis and regeneration. Here, we investigated the role of Ephrin signaling in neural patterning in the planarian Schmidtea mediterranea . We discovered that RNAi against the Eph receptor EphR1 led to striking ectopic expression of the mechanosensory neuron markers pkd1L-2 and hmcn-1-L, without obvious disruption of the overall architecture of the central nervous syste

A case of seal bite injury.

In Japan, seal habitats are limited, and no medical cases of seal bites have been reported. Therefore, we report a case of a seal bite. A 22-year-old female aquarium employee was bitten by a seal that reacted to a dog that was barking during feeding. She washed the wound at the scene for approximately 10 min before presenting to the hospital. A bite wound with tooth marks extending from the back of the right hand to the wrist and partially penetrating the subcutaneous tissue was observed. The wound was cleaned and disinfected, and the bite site was left open. Tetanus toxoid and amoxicillin/cla

How crocodiles lost a forebrain circuit.

Why do brain circuits differ among species of the same or different classes of vertebrates? Answers to this question are critical in determining brain organization and evolution. Investigations in mammals and in certain model systems have been the main focus of these studies. Among the areas that have commonly been investigated are cell death, synaptic pruning, and synapse organizing proteins. Whether these same or even other factors sculpt similar circuits in other vertebrates such as reptiles is unknown. The present study examined the development of a well defined circuit in crocodiles and c

Low-nanomolar chetomin in human skin cells: Preserved viability and wound closure, reduced basal IL-6 and IL-1β release, and preliminary tolerability in Galleria mellonella.

Chetomin is a disulfide-bridged epipolythiodioxopiperazine produced by Chaetomium endophytes and is studied mainly as an inhibitor of the HIF-1α-p300 interaction in tumour models. Its behaviour in non-transformed skin cells at sub-toxic concentrations has not been reported. We exposed HaCaT keratinocytes and HFF-1 fibroblasts to 2.5-20 nM chetomin and recorded viability (MTT and PrestoBlue, 24 and 72 h), scratch wound closure, basal IL-6 and IL-1β release (ELISA), collagen I and III immunostaining, real-time impedance over 87.7 h, and survival of Galleria mellonella larvae. Viability remained
