Pre-hospital time and risk of perforation in patients with acute appendicitis in 112 countries (AlliGatOr): an international, prospective, observational cohort study.

BACKGROUND: Acute appendicitis is the most common surgical emergency worldwide. Concern that delayed treatment might increase perforation can lead to unnecessary surgery and high rates of negative appendicectomy, where a normal, non-inflamed appendix is removed. However, whether perforation is related to pre-hospital time (symptom onset to first surgical assessment) has not been explored in contemporary global data. METHODS: We conducted an international, multicentre, prospective, observational cohort study (AlliGatOr) of patients undergoing emergency appendicectomy for suspected acute append

Lymph node-targeted, mKRAS-specific amphiphile vaccine in pancreatic cancer: the randomized phase 2 AMPLIFY-7P trial.

Camupepimut (ELI-002 7P) drives mutated KRAS (mKRAS)-specific T cell responses that correlated with disease-free survival (DFS) and overall survival (OS) in patients with mKRAS pancreatic adenocarcinoma (PDAC) in the phase 1 AMPLIFY-201 trial. In the phase 2 AMPLIFY-7P trial, patients with stage I-III KRAS-mutated PDAC without recurrence within 6 months of standard therapy were randomized 2:1 to camupepimut (n = 96) or standard-of-care/observation (n = 48). Stratification balanced nodal status. The primary DFS endpoint was not met (median 23.8 versus 21.2 months; HR 0.85, P = 0.5329). However,

Morphological, Radiographic and Three-Dimensional Scanning Characterization of the Skull and Mandible of the American Crocodile (Crocodylus acutus).

The American crocodile (Crocodylus acutus) is a widely distributed Neotropical species for which detailed osteological descriptions of the skull and mandible remain scarce. This study provides an integrative characterization of the cranial and mandibular complex using gross anatomy, descriptive cranial morphometry, radiography, and three-dimensional (3D) scanning. Four skulls and mandibles from post mortem specimens were prepared following standard osteological procedures, and descriptive cranial measurements were recorded. Radiographic projection allowed visualization of bone margins, cranial

Climatic niche conservatism shapes the macroevolution, extinction, and survivorship of crocodile-line archosaurs.

Crocodylians are iconic thermophiles. Over 250 million years, crocodile-line archosaurs endured hyperthermals, climatic and range shifts, and two mass extinctions. Whether this niche conservatism reflects a deep evolutionary constraint or recent adaptation to novel climates remains unclear. We integrate present-day occurrences with a time-scaled phylogeny of >400 fossil and extant species. Niche models calibrated on living crocodylians were projected across 245 million years and compared with paleophylogeographic reconstructions. Macroevolutionary model fitting shows that temperature and preci

Chamber-Specific Morphometry and Quantitative Cardiac Histology in the Broad-Snouted Caiman (Caiman latirostris).

Quantitative cardiac morphology remains poorly documented in crocodilians. Hearts from four commercially reared male Caiman latirostris were examined by gross morphometry, histology, stereology, reticulin staining and Masson's trichrome. Relative heart mass averaged 0.125% of body mass. The left ventricular wall was numerically thicker than the right. Cardiomyocyte volume density and cross-sectional area varied among chambers, whereas collagen-positive area was greater in the right ventricle. Pulmonary arterial walls were markedly thinner than the systemic aortae, and the left pulmonary arteri

Epidemiology of voluntarily reported dog and cat bite incidents in a large-scale, multi-site veterinary network in the UK and Ireland.

INTRODUCTION: Dog and cat bites are an occupational risk within veterinary practise. Despite this, there is minimal research exploring the occurrence and details surrounding such incidents. METHODS: The aim of this study was to use a large veterinary group's existing incident and near miss reporting system in the UK to explore the incidence of dog and cat bite reporting and surrounding details of such incidents. RESULTS: Dog and cat bite incidents were identified from January 2024 to October 2025 covering all clinics in the UK and Ireland (>1,000). A total of 2,327 dog (1,239; 53.24%) and ca

[What should you request to the laboratory for a patient bitten by a tick ?].

Ticks are a growing concern for the public, owing to their abundance and a longer activity season. In Switzerland, more than 99% of ticks removed from humans are of the species Ixodes ricinus. This article reviews the tick-borne diseases documented in the country : Lyme borreliosis, tick-borne encephalitis, tularaemia, neoehrlichiosis, anaplasmosis, babesiosis, Borrelia miyamotoi relapsing fever, and rickettsiosis. Carriage of a microorganism by a tick does not imply infection, just as isolated seropositivity does not imply disease. Testing should be guided by pretest probability and disease s

The Epidemiological Characteristics of 55 Multiple-Victim Incidents Involving Bites from a Single Dog - Shandong Province, China, 2025.

WHAT IS ALREADY KNOWN ABOUT THIS TOPIC?: Rabies is a zoonotic infectious disease caused by rabies virus infection and was classified as a Class B notifiable infectious disease in China. Events involving multiple victims bitten by a single dog represent a distinct category in rabies epidemiological surveillance and risk assessment. These incidents indicate high-probability point-source exposure to potentially rabid animals and thus pose urgent public health threats. WHAT IS ADDED BY THIS REPORT?: This study analyzed the epidemiological characteristics of events involving multiple victims bitte

Rabies and rabies prophylaxis in Egypt: a nationwide cross-sectional study that calls for action.

BACKGROUND: Rabies is a fatal but preventable viral disease that poses a global health threat, especially in developing countries. Despite the high incidence of animal bites reported annually in Egypt, limited data exist regarding the knowledge, attitudes, and practices (KAP) of the general population toward rabies and rabies prophylaxis. METHODS: We conducted a nationwide cross-sectional study using a structured interview-based survey targeting individuals aged 18 years or older who attended outpatient clinics at nine university hospitals across Egypt. The survey assessed participants' KAP t

Delay in initiation of rabies post-exposure vaccination and influencing factors in Gondar zones of Ethiopia.

Rabies is a viral zoonotic disease of warm-blooded animals, which is preventable through proper administration of post-exposure prophylaxis (PEP). In controlling rabies, delays in initiating post exposure vaccination (PEV) are a challenge in endemic regions. A study was conducted to assess the delay in initiation of PEV and identify associated factors in Ethiopia. Data was collected by face-to-face interviews from 260 dog bite victims or their guardians between January and June, 2025 using a questionnaire. Post exposure vaccination was considered delayed if it was initiated after 48 hours of t

Molecular insights into how maternal investment shapes embryonic development for improving conservation breeding of the Chinese alligator (Alligator sinensis).

The Chinese alligator (Alligator sinensis) remains critically endangered in the wild due to severe habitat loss and historical hunting. Although ex situ breeding has achieved demographic success, the low recruitment of reintroduced populations highlights the importance of improving offspring quality. Egg weight, a direct measure of maternal investment, is known to affect hatchling phenotype, but its molecular effects on embryogenesis remain poorly understood. Here, large-sample analyses confirmed that heavy eggs had higher hatching success and produced heavier hatchlings than light eggs. We fu

Borrelia miyamotoi exceeds Borrelia burgdorferi sensu lato in human exposure despite the opposite in ticks: an integrated serological, entomological and molecular study in southeastern Kazakhstan.

Lyme borreliosis (LB) and hard tick-borne relapsing fever caused by Borrelia miyamotoi are emerging tick-borne infections, yet in Kazakhstan routine LB diagnosis relies on single-tier enzyme immunoassay (EIA) without confirmatory testing, and B. miyamotoi is absent from national diagnostic protocols. In 2024-2026, 1300 participants were enrolled in the Ixodes persulcatus-endemic Almaty and Jetisu regions and the non-endemic Zhambyl and Turkistan regions, across diverse clinical and tick-exposure groups. All sera were screened for IgM and IgG to both pathogens by multi-antigen protein arrays, w

The prognostic value of HER2 in gastroesophageal adenocarcinoma - a meta-analysis.

BACKGROUND: HER2 amplification and overexpression are predictive markers for HER2-targeted therapies in gastroesophageal adenocarcinoma. However, unlike in breast cancer, the prognostic significance of HER2 status in this disease remains uncertain, with conflicting evidence across studies. METHODS: We conducted a systematic review and meta-analysis of studies reporting hazard ratios (HRs) for overall survival (OS) based on HER2 status, identified through PubMed, Scopus, and Web of Science. Eligible studies included HER2-positive and HER2-negative patients with gastroesophageal adenocarcinoma,

One Health genomic surveillance of ticks found on community-dwelling non-human 'farm' animals to identify microbes associated with tick borne illnesses in Eastern Ontario.

Companion animals, such as dogs and cats, and wildlife are recognized as sentinel species in One Health surveillance due to periodic exposure to ticks and infection by tick-borne pathogens (TBPs). Surveillance of this type is a strategy that captures a broad range of genetic, immunological, and behavioral factors that affect tick and disease dynamics. In contrast, non-human animals (hereafter referred to as 'animals') that are traditionally farmed for their meat and other products have been largely overlooked as sentinel species. Community-dwelling farm sanctuary residents, such as cattle, pig

A case of seal bite injury.

In Japan, seal habitats are limited, and no medical cases of seal bites have been reported. Therefore, we report a case of a seal bite. A 22-year-old female aquarium employee was bitten by a seal that reacted to a dog that was barking during feeding. She washed the wound at the scene for approximately 10 min before presenting to the hospital. A bite wound with tooth marks extending from the back of the right hand to the wrist and partially penetrating the subcutaneous tissue was observed. The wound was cleaned and disinfected, and the bite site was left open. Tetanus toxoid and amoxicillin/cla

How crocodiles lost a forebrain circuit.

Why do brain circuits differ among species of the same or different classes of vertebrates? Answers to this question are critical in determining brain organization and evolution. Investigations in mammals and in certain model systems have been the main focus of these studies. Among the areas that have commonly been investigated are cell death, synaptic pruning, and synapse organizing proteins. Whether these same or even other factors sculpt similar circuits in other vertebrates such as reptiles is unknown. The present study examined the development of a well defined circuit in crocodiles and c

Rare Migrated Fishbone Foreign Body in the Nasopharynx.

Although impaction of the pharyngoesophageal fishbone represents a common otolaryngological emergency, migration of an ingested fish bone into the nasopharynx is an extremely rare clinical occurrence. We present a 57-year-old woman who developed acute odynophagia one hour after fish ingestion, accompanied by a single choking episode, in the absence of nausea or vomiting. The oropharyngeal examination at the bedside failed to identify any foreign body. Given the patient's refusal to computed tomography (CT), she was discharged for outpatient surveillance. Twenty-four hours later, she returned w
