ECPR eligible refractory out of hospital cardiac arrests - A post-hoc analysis of the EVIDENCE randomised controlled trial and Registry.

BACKGROUND: The use of Extracorporeal cardiopulmonary resuscitation (ECPR) for refractory out-of-hospital cardiac arrest (OHCA) is increasing. The EVIDENCE study of refractory OHCA transportation did not report a survival benefit of expedited transportation to hospital, likely in part owing to a very limited number of patients receiving ECPR. The reasons for this, the total potential numbers of ECPR patients and the effects of different system changes would assist in trial and clinical service planning. METHODS: A pre-specified, post-hoc analysis was performed of the randomised controlled tri

Mapping eligibility criteria in oncology target trial emulations using real-world data: a scoping review.

BACKGROUND: Target trial emulations often face challenges in accurately identifying the intended target population within real-world data (RWD) because eligibility criteria cannot always be directly mapped to available variables. This scoping review aimed to systematically characterize the proportion of eligibility criteria from oncology target trials that are successfully mapped to their emulated counterparts and to describe the approaches used for this mapping. METHODS: We searched MEDLINE for peer-reviewed studies published between January 1, 2016, and April 23, 2025, that were designed to

Applying the Target Study Conceptual Model to Measure Racial and Ethnic Disparities in Hypertension Treatment Intensification.

BACKGROUND: This tutorial paper demonstrates the application of a conceptual model to measure racial and ethnic disparities in treatment intensification among adults with hypertension consistent with the Institute of Medicine (IOM) definition. The IOM defines disparity as differences in health care quality that are not due to access-related factors, clinical needs, preferences, and appropriateness of intervention (referred to as allowable covariates). METHODS: We used a conceptual model called the Target Study to estimate annual disparities in the probability of treatment intensification betw

Deprescribing in Older People: A Clinical Practice Guideline Summary.

INTRODUCTION: Older people face higher risks of medicine-related harm due to polypharmacy and the use of potentially inappropriate medicines. Current treatment guidelines rarely specify when to stop medicines, leading to medicines often being continued indefinitely without a clear deprescribing plan. While deprescribing guidelines exist for some medicine classes, limited guidance is a major barrier to deprescribing. These new guidelines address this gap by providing structured recommendations that complement more detailed drug-specific deprescribing guidance, disease-specific therapeutic guide

Professional identity formation (PIF) - What has NKLM 2.0 to say about it? A workshop report.

OBJECTIVE: Medical education should support the development of a professional identity. However, it is still unclear what exactly is meant by professional identity formation (PIF), how it proceeds, and how it can be specifically supported. Against this background, this workshop report describes an attempt by a group of experts to identify and evaluate learning objectives in NKLM 2.0 that could be relevant to PIF. The aims of this project were, on the one hand, to determine the extent to which PIF has been incorporated into the NKLM and, on the other hand, to analyze how PIF can be understood o

A Retrospective, Multicenter Analysis Within the National Network Genomic Medicine Lung Cancer in Germany to Detect RET Fusions as a Possible Mechanism of Resistance in Patients With EGFR Mutations.

BACKGROUND: Resistance to third-generation EGFR tyrosine kinase inhibitors (TKIs) such as osimertinib remains a major challenge in the treatment of EGFR-mutated non-small cell lung cancer (NSCLC). While on-target and bypass mechanisms such as MET amplification are well-characterized, oncogenic fusions-particularly RET fusions-are emerging as relevant resistance mechanisms in a subset of patients. The feasibility of dual inhibition strategies and personalized monitoring through liquid biopsy remains underexplored in real-world clinical practice. MATERIALS AND METHODS: This retrospective, multi

Strengthening the foundations of shared decision-making in oncology: Systematic review and formal representation of theoretical models.

OBJECTIVES: Shared decision-making (SDM) is central to person-centred care, and particularly critical in oncology. Yet, its clinical implementation remains inconsistent. A strong theoretical foundation is essential for developing and sustaining effective SDM practices. Formal representation of theories enhances clarity and testability, enabling the accumulation of reliable evidence to support implementation. We systematically reviewed SDM models in oncology and formally mapped the constructs proposed, their definitions and operationalization, and the hypothesized relationships among them. MET

Evaluating evidential strength of statistically nonsignificant meta-analyses with likelihood ratios: a meta-epidemiological study.

OBJECTIVES: This study aimed to determine the proportion of statistically nonsignificant meta-analyses that provide evidence favoring the null hypothesis of no difference, evidence favoring the alternative hypothesis, or inconclusive findings using likelihood ratios. STUDY DESIGN AND SETTING: In this meta-epidemiological study, Cochrane systematic reviews published in 2025 were screened. Eligible meta-analyses pooled data from two or more primary studies, reported a pooled effect estimate with a 95% CI including the value of no difference, and had a P value between 0.05 and 0.20. Development, Feasibility, Acceptability, and Usability of an Artificial Intelligence-Powered Chatbot (Suzy) to Support Patients in Substance Use Disorder Recovery: Multiphase Study.

BACKGROUND: Substance use disorder (SUD) remains a major public health crisis in the United States, with significant challenges in treatment access, retention, and workforce capacity. SUD care teams, including addiction medicine physicians and peer recovery coaches (PRCs), support patients receiving SUD treatment but face heavy workloads and burnout. Artificial intelligence (AI) innovations, particularly large language model (LLM)-based chatbots, may extend PRC support and provide patients with on-demand recovery support between clinic visits and PRC contacts. However, evidence on their develo

Addressing social determinants of health and equity in early childhood: a qualitative document analysis of national policies in Ecuador.

BACKGROUND: Early childhood health is a major policy priority in Ecuador, yet systematic analyses of relevant policies are limited. This study analyses how Ecuadorian national-level policies (2000-2024) have addressed social determinants of health (SDH) in early childhood and promoted health equity. We map the development and scope of the past and present national policy documents across sectors to identify gaps, strengths and priorities within the policy landscape. METHODS: Following a systematic search, we conducted a qualitative analysis of 29 policy documents using the READ approach (read

The use of variable selection in clinical prediction modeling for binary outcomes: a systematic review.

OBJECTIVES: Clinical prediction models are valuable tools that can support medical decision-making. Concise and interpretable models are more likely to be adopted in clinical practice; therefore, appropriate selection of predictor variables is often considered essential in model development. Typically, researchers specify a list of candidate predictors based on literature reviews and expert knowledge. Data-driven variable selection methods are then often used to further reduce the number of variables in the final model. However, many commonly used approaches, such as univariable selection, hav

Safety and efficacy of ivabradine in the treatment of patients with acute myocardial infarction: a real-world propensity score matching study.

AIMS: Ivabradine, a selective If channel inhibitor, reduces heart rate without impairing myocardial contractility. Evidence regarding its safety and efficacy in the acute phase of acute myocardial infarction (AMI) remains limited and conflicting. METHODS AND RESULTS: We analysed data from 136 033 patients with AMI admitted to 82 hospitals in Tianjin, China, between 2010 and 2024. Primary outcomes were in-hospital mortality and 1-year cardiac mortality; secondary outcomes included 1-year all-cause mortality and other adverse events. Propensity score matching (PSM) was used to balance baseline

Sternum-worn vibrotactile device in Parkinson's disease: a randomised, double-blind, placebo-controlled pilot trial.

Effective, non-invasive treatments for motor and non-motor symptoms in Parkinson's disease (PD) remain limited, and wearable vibrotactile stimulation may help by enhancing sensorimotor integration. This multi-centre, double-blind, pilot randomised controlled trial evaluated the usability, safety and tolerability of the CUE1+ sternum-worn vibrotactile device, with exploratory clinical outcomes compared with a sham device. Fifty adults with idiopathic PD, stable on antiparkinsonian medication, were randomised to active CUE1+ (n = 25) or sham (n = 25) for 12-weeks (8 h/day), with ON-medication as

Learning to reflect together: a video-facilitated reflective inquiry and linguistic analysis of collaborative clinical reasoning in intensive care.

BACKGROUND: Collaborative clinical reasoning (CCR) is essential to safe and effective practice in intensive care units, where decisions are shaped by uncertainty, high cognitive load, and interprofessional dynamics. Although CCR is widely recognised as a collective cognitive process, little is known about how clinicians learn to reason together or how reflective educational interventions may influence the language and social organisation through which CCR is enacted. METHODS: This exploratory mixed-methods Groundwork study examined the association between a video-facilitated reflective (VFR)

Pain at the end of life in patients with cancer: a population-based study on prevalence, relief, and the role of pain assessment.

BACKGROUND: Pain is common in advanced cancer, and its assessment is recognized as crucial for effective management. However, real-world evidence on pain prevalence, relief, and the impact of structured pain assessment across cancer types at the end of life remains limited. METHODS: We analyzed data from 215,317 patients who died from cancer reported to the Swedish Register of Palliative Care (2011-2023). Data are based on validated end-of-life questionnaires completed by healthcare providers after the patient's death. Patient characteristics and provider-reported pain outcomes (prevalence of

Disparities in the Receipt of Palliative-intent Treatment Across Disaggregated Hispanic Subgroups With Stage IV Gastric Cancer in the United States.

OBJECTIVES: Gastric cancer affects over 30,000 Americans annually and has a 5-year survival rate of roughly 40%. Palliative-intent interventions are critical for symptom relief and quality of life, yet access remains inequitable. We examined disparities in receipt of palliative-intent care among disaggregated Hispanic subgroups with gastric cancer. METHODS: Using the National Cancer Database (2004 to 2021), we identified adults (≥18 y) with AJCC stage IV gastric cancer. The primary outcome was receipt of palliative-intent intervention, defined as treatment for symptom alleviation rather than

Brain states recur across diverse narrative contexts during longitudinal viewing.

What does the brain do during the continuous, varied experience of watching a story unfold? One account holds that the brain traverses a finite repertoire of recurring states, but whether that repertoire is a stable property of the individual or is reshaped by each new experience has not been tested across diverse naturalistic content within the same person. We characterized the dynamic brain-state repertoire in six individuals who watched the television series Friends across its six seasons during fMRI (up to ~146 episodes, ~54 hours per person). For each individual we fit a sticky hierarchic
