Cardiovascular Health at Midlife and Alzheimer Disease Biomarkers.

BACKGROUND: Cardiovascular health factors are associated with cognitive decline and risk of dementia, including Alzheimer disease (AD); however, this has been mostly studied in late life. We investigated whether vascular and lifestyle factors are associated with AD plasma and imaging biomarkers in midlife. METHODS: We investigated 1,406 participants from the Coronary Artery Risk Development in Young Adults (CARDIA) study with information on vascular and lifestyle factors framed from the American Heart Association (AHA) "life's essential 8" (LE8) guidelines for cardiovascular health at early m

An Unusual Case of Situs Inversus in a Premature Neonate.

Situs inversus, also known as a mirror image, is the transposition of all major organs in the body from left to right. Situs inversus totalis is a rare condition characterized by the complete inversion and mirroring of organs, including the cardia, lungs, and all abdominal contents. It follows an autosomal recessive pattern of inheritance, with an incidence of approximately one in 8,000 live births. Early prognosis is important due to the risk of complications associated with this syndrome. In this case report, we will examine a premature infant with isolated situs inversus, which was discover

[Gastrointestinal bleeding after mitral valve implantation. Case report].

Performing surgical interventions on the mitral valve is one of the main high-tech treatment methods that requires further anticoagulation therapy. However, patients at high risk of bleeding have traditionally become a group requiring a personalized approach and, in many cases, the guidance of a multidisciplinary team of specialists of various profiles. In this clinical case, it is noteworthy that the 72-year-old patient underwent a full amount of preoperative preparation, and no contraindications to surgical treatment were identified. During esophagogastroduodenoscopy, an endoscopic picture o

Proteogenomic Analysis of Coronary Artery Calcification in Human Populations.

BACKGROUND: Joint use of multiple molecular layers can be useful to prioritize targets for mechanistic studies. Application of this approach to coronary disease in large populations is an emerging field. METHODS: We used reported circulating proteomic data (Somascan aptamer-based) from ≈3000 individuals in the CARDIA study (Coronary Artery Risk Development in Young Adults), measuring association with prevalent and 10-year incident coronary artery calcium (CAC) score. We used a multiparametric approach to prioritize circulating protein-CAC associations via genomics of circulating protein level

A network meta-analysis of comparative effectiveness and safety of pulsed field, radiofrequency, and cryoballoon ablation for atrial fibrillation.

BACKGROUND: Pulsed field ablation (PFA) has emerged as a non-thermal alternative to radiofrequency ablation (RFA) and cryoballoon ablation (CBA) for atrial fibrillation (AF). Despite several individual comparisons, integrated comparison across 3 modalities remains limited. OBJECTIVE: To compare the efficacy, safety, and procedural characteristics of PFA, RFA, and CBA for AF ablation. METHODS: We performed frequentist network meta-analysis (using pooled odds ratios [ORs] and mean differences[MDs]) of studies comparing PFA, RFA, and CBA. Outcomes included recurrence of atrial arrhythmias, AF r

Transcatheter Aortic Valve Replacement Outcomes in Patients With Chronic Inflammatory Systemic Diseases.

The clinical impact of performing transcatheter aortic valve replacement (TAVR) in patients with chronic inflammatory systemic diseases (CIDs) is not well reported. Hence, we designed this study to evaluate whether coexistence of CIDs in TAVR patients leads to worse clinical outcomes. We retrospectively studied TAVR patients at our institution between November 21, 2011 and March 29, 2024. Patients diagnosed with chronic inflammatory diseases such as rheumatoid arthritis, systemic lupus erythematosus (SLE), scleroderma, Sjögren's syndrome, inflammatory bowel disease, or other related diseases w

Redo-TAVR (TAV-in-TAV) Best Practices Part 1: Short-in-Short and Short-in-Tall: A Heart and Valve Collaboratory Document.

Transcatheter aortic valve replacement (TAVR) has created a growing population of patients who require redo-TAVR (TAV-in-TAV) for structural valve deterioration. Short-frame balloon-expandable valves implanted within failed short- or tall-frame transcatheter valves ("short-in-short" and "short-in-tall") pose specific challenges, including coronary risk assessment, valve sizing, anchoring, and preservation of coronary access. Under the auspices of the Heart and Valve Collaboratory, an international multidisciplinary panel of experts reviewed bench and clinical data and synthesized contemporary

Redo-TAVR (TAV-in-TAV) Best Practices Part 2: Tall-in-Short and Tall-in-Tall: A Heart and Valve Collaboratory Document.

Transcatheter aortic valve replacement (TAVR) has created a growing population of patients who require redo-TAVR (TAV-in-TAV) for structural valve deterioration. Tall-frame self-expanding valves implanted within failed short- or tall-frame transcatheter valves ("tall-in-short" and "tall-in-tall") present distinct challenges related to anchoring, constrained expansion, valve alignment, and the risk for coronary obstruction or sinus sequestration driven by the resulting neoskirt plane. Under the auspices of the Heart and Valve Collaboratory, an international multidisciplinary panel reviewed benc

First-in-Human Mechanical Leaflet Modification of Native Aortic Stenosis During TAVR.

BACKGROUND: Coronary obstruction is a life-threatening complication of transcatheter aortic valve replacement (TAVR). Mechanical leaflet modification (LM) has been developed to mitigate this risk in patients with degenerated bioprostheses. However, their use in native valves has not previously been reported. CASE SUMMARY: We report 2 cases of mechanical LM performed in patients with severe native aortic stenosis at high risk for coronary obstruction during TAVR. In both cases, successful LM was achieved using a dedicated device (ShortCut, Pi-Cardia), followed by TAVR. Imaging validated leafle

Seven-Year Valve Durability With Transcatheter or Surgical Aortic Valve Replacement: An Ad Hoc Analysis of the PARTNER 3 Randomized Clinical Trial.

IMPORTANCE: As transcatheter aortic valve replacement (TAVR) is considered for younger and lower-risk populations, the durability of bioprosthetic valves is increasingly important. Limited data exist on long-term (7 years and beyond) valve durability. OBJECTIVE: To report 7-year valve durability outcomes for low-risk patients with symptomatic severe aortic stenosis treated with TAVR vs surgery. DESIGN, SETTING, AND PARTICIPANTS: Between March 2016 and October 2017, a total of 1000 patients were enrolled at 71 centers in the US and Canada and randomized to undergo TAVR vs surgery. The patient

Sex Differences in Left Ventricular Remodeling for Risk Stratification of Patients With Aortic Regurgitation.

IMPORTANCE: Left ventricular (LV) dilatation is an established prognosticator in aortic regurgitation (AR). Current guidelines recommend aortic valve surgery (AVS) using LV end-systolic diameter index (LVESDi) with a uniform threshold, irrespective of sex. While LV end-systolic volume index (LVESVi) may better characterize LV remodeling, it was only recently included in European guideline recommendations, with a threshold of 45 mL/m2 for both men and women. OBJECTIVE: To assess sex differences in LV remodeling using linear and volumetric dimensions and their association with outcomes in AR.

Pulmonary Artery Banding: an Effective in-vivo Acute Model of Functional Tricuspid Regurgitation for Transcatheter Interventions.

Tricuspid regurgitation (TR) is a highly morbid and often untreated valvular heart disease. New devices are under development to address this unmet need, necessitating valid models to test their efficacy. Aim of this study was to assess feasibility and reliability of pulmonary artery banding (PAB) as a pathological acute model of functional TR. Eight pigs underwent right thoracotomy, with an umbilical tape placed around the main pulmonary trunk, followed by controlled reduction of the pulmonary artery lumen via a tourniquet system. No animals died during the procedure. After PAB, right ventric
