Comparative clinical features of antibiotic-associated Kounis syndrome and non-Kounis allergic coronary events: a disproportionality analysis using U.S. Food and Drug Administration Adverse Event Reporting System.

BACKGROUND: Kounis syndrome (KS) is an acute coronary syndrome (ACS) triggered by allergic reactions and is frequently associated with antibiotic exposure. However, patient characteristics of antibiotic-associated KS cases are not well described. Some cases with KS-consistent symptoms may instead be reported as antibiotic-associated allergic coronary events (non-KS) without proper diagnosis. Characterizing these non-KS events may improve the accurate diagnosis of KS. This study explores the clinical features of antibiotic-associated KS and non-KS allergic coronary events in the FDA Adverse Eve

Penicillin reintroduction using the PEN-FAST score reduces infectious complications in maxillofacial surgery.

CONTEXT: Self-reported penicillin allergy frequently limits optimal antibiotic prescribing in maxillofacial surgery. The PEN-FAST score (Penicillin allergy, within the last five years, Anaphylaxis/angioedema, Severe cutaneous adverse reaction, Treatment required for allergy episode) is a validated clinical decision rule used to identify patients at low risk of true penicillin allergy and to guide safe penicillin reintroduction. METHODS: We conducted a prospective, interventional, single-center study including adult patients with a self-reported penicillin allergy undergoing maxillofacial surg

Climatic determinants of antibiotic use in Southern Thailand: Retrospective longitudinal study during 2021 to 2023.

BACKGROUND: Antibiotic consumption is influenced by multiple factors, including environmental conditions. In tropical regions, climate variability may contribute to seasonal patterns of infectious diseases and prescribing behaviors. This study examined the association between climatic variables and antibiotic use in a tertiary hospital in southern Thailand. METHODS: We analyzed 7,696 prescriptions of tetracyclines (n = 971), beta-lactams and penicillins (n = 4,208), and macrolides (n = 2,517) dispensed from 2021 to 2023. Defined daily doses (DDDs) were modeled using generalized additive model

Practical and clinical significance of the pharmacist-led antimicrobial stewardship.

Antimicrobial resistance (AMR) undermines clinical care. Thus, ensuring appropriate antimicrobial therapy is crucial in global and national AMR countermeasures. This narrative review aimed to summarize pharmacist-led antimicrobial stewardship interventions in Japan and discuss their practical and clinical significance across inpatient, outpatient, and community settings. In inpatient care, antimicrobial stewardship programs (ASPs), usually embedded within antimicrobial stewardship teams and supported by national policies and reimbursement incentives, have consistently reduced broad-spectrum an

Case-Control Study on Risk Factors of CRE Infection in the ICU.

INTRODUCTION: In recent years, the increasing prevalence of carbapenem-resistant Enterobacteriaceae (CRE) poses a serious threat to clinical anti-infection therapy. This study aims to identify risk factors for CRE infection among intensive care unit (ICU) patients. METHODOLOGY: An unmatched case-control study based on a hospital was performed. Data on the general condition of patients, disease scores, invasive procedures, and the use of antibiotics before infection were collected. Univariate and multivariate Logistic regression were performed to analyze the risk factors for CRE infection. RE

Temocillin for non-urinary tract infections caused by AmpC-producing Enterobacterales: Is it a feasible option?

BACKGROUND: Infections due to AmpC-producing Enterobacterales represent an increasing therapeutic challenge. Third-generation cephalosporins may select resistant mutants, inducing clinicians to use cefepime or carbapenems and thereby contributing to carbapenem resistance. While temocillin is a narrow-spectrum beta-lactam stable against AmpC enzymes, evidence of its impact in non-urinary tract infections remains limited. METHODS: We conducted a retrospective study of adult patients treated with temocillin for non-urinary tract infections caused by AmpC-producing Enterobacterales between 2016 a

A structured proactive penicillin allergy delabeling intervention in adult patients in three non-university hospitals.

BACKGROUND: Over 90% of patients with penicillin allergy do not have a true penicillin allergy. This allergy label can lead to inappropriate antibiotic prescription, a higher risk of treatment failure and toxicity, and higher rates of infections with multidrug-resistant organisms. Penicillin allergy delabeling strategies are increasingly reported and should be included in antimicrobial stewardship programs. OBJECTIVES AND METHODS: Development and phased implementation of a structured and proactive screening tool and decision algorithm for delabeling presumed penicillin allergy in the preopera

Feasibility and impact of pharmacist-led penicillin allergy delabelling using the PEN-FAST scoring tool in a Canadian tertiary care hospital.

BACKGROUND: Penicillins are the most frequently prescribed antibiotics for a broad range of infections. In North America, up to 15% of hospitalized patients report a penicillin allergy, but research has shown that 98% of these patients can tolerate penicillins. Removing inaccurate allergy labels is an essential component of antimicrobial stewardship. While allergy delabelling used to be complex or require an allergist referral, the emergence of new tools, such as the PEN-FAST score, facilitates direct delabeling of low-risk patients. OBJECTIVES: The primary objective of this study was to tria

[THE ALL-UNION RESEARCH INSTITUTE OF PENICILLIN (1947-1952): THE EXTRACTED PAGE OF HISTORY. REPORT III: THE STALIN AWARD "FOR DEVELOPMENT AND IMPLEMENTATION INTO INDUSTRY THE METHOD OF OBTAINING MEDICAL PREPARATION"].

The article, on the basis of analysis of archival materials, reveals contribution of the All-Union Research Institute for Penicillin and Other Antibiotics to development of modern technology for production of penicillin. The factors that allowed the Institute to improve key stage of production process in short period of time, despite deficiency of manpower and other resources were studied. The causes of exclusion of information about the Institute and its work on penicillin from the text of the Decree on the Stalin Prize of 1950 and from the information space in general are considered.

Phenotypic and genomic characterization of Castellaniella ginsengisoli, an emerging pathogen associated with disease in birds.

The environmental bacterium Castellaniella revealed its clinical relevance from case reports, causing fatal infections in animals. The lack of phenotypic and genomic characterization may have historically contributed to the scattered identification of Castellaniella in the field and in the laboratory. This pioneer study addressed this knowledge gap by performing phenotypic and genomic characterization of C. ginsengisoli isolated from 22 clinical cases of chickens. We observed that C. ginsengisoli isolates exhibited three colony morphologies on blood agar and presented flagella under transmissi

Functional characterization of a novel class A carbapenemase CAE-1 in carbapenem-resistant Pseudomonas aeruginosa clinical isolates.

Two carbapenem-resistant Pseudomonas aeruginosa (CRPA) isolates were collected, which lacked known carbapenemases but produced a β-lactamase CAE-1. CAE-1 was recently found in Comamonas aquatica and conferred resistance to penicillins and cephalosporins. In this study, we aim to know whether CAE-1 was responsible for the carbapenem resistance in the CRPA isolates and its enzyme hydrolyzing characteristics. Both CRPA clinical isolates exhibited minimal inhibitory concentrations (MICs) of 8 µg/mL for imipenem and meropenem, with a positive result in the modified Hodge test for meropenem. The bla

Development and validation of a predictive model for profiling antibiotic resistance phenotypes of Acinetobacter baumannii strains via clinical MALDI-TOF mass spectra: A multicenter study.

Acinetobacter baumannii is a significant pathogen responsible for healthcare-acquired infections (HAIs), posing challenges due to its rising resistance to multiple antibiotics. Traditional diagnostic methods like bacterial cultures and antibiotic susceptibility testing (AST) are slow, necessitating the development of faster alternatives. This study aimed to create a predictive model for antibiotic resistance phenotypes of A. baumannii using clinical MALDI-TOF mass spectrometry (MS) spectra combined with machine learning techniques. A total of 3644 A. baumannii strains were analysed for resista
