Rapunzel Syndrome in a Child With Sensory Feeding Difficulties and Attention-Deficit Hyperactivity Disorder.

Rapunzel syndrome is a rare form of trichobezoar characterized by the extension of a gastric hair mass beyond the pylorus into the small intestine. It is most commonly reported in adolescents with underlying psychiatric illness. We describe a diagnostically challenging case in a younger child with neurodevelopmental vulnerability and sensory feeding difficulties. A 10-year-old girl with attention-deficit hyperactivity disorder, sensory aversion to food textures, and heterozygous chromosomal deletions presented with progressive weight loss, anxiety around eating, and intermittent abdominal pain

Pylorus-spanning diffuse gastric lipomatosis as an uncommon cause of intermittent gastric outlet obstruction: a case report.

INTRODUCTION AND IMPORTANCE: Gastric lipomas are uncommon; diffuse gastric lipomatosis is rarer, and pyloric involvement may cause bleeding or intermittent gastric outlet obstruction. We report a pylorus-spanning lipomatosis treated definitively by laparoscopic distal gastrectomy. PRESENTATION OF CASE: A 75-year-old man presented with melaena and post-prandial epigastric discomfort. Endoscopy revealed a broad-based submucosal mass at the pyloric canal intermittently prolapsing into the duodenal bulb with a small healing ulcer. computed tomography (CT) showed an 8.1 × 2.7 × 3.8 cm multiseptate

Jejunal perforation caused by transanastomotic pancreaticojejunostomy internal stent following pancreaticoduodenectomy: a case report.

Mortality and morbidity following pancreaticoduodenectomy (PD) have significantly decreased due to multimodal treatment approach and refinement in surgical techniques. Despite lacking proven benefits, surgeons continue using pancreaticojejunostomy (PJ) transanastomotic stent to mitigate post-operative pancreatic fistula (POPF). We share a morbid case of jejunal perforation caused by PJ stent. A 69 years male presented with features of gastric outlet obstruction secondary to carcinoma second part of duodenum. After proper optimization, he underwent pylorus resecting PD with child's reconstructi

Laparoscopic management of congenital pyloric Atresia: A case series and surgical perspective.

BACKGROUND: Congenital pyloric atresia (CPA) is an extremely rare neonatal gastric outlet obstruction (approximately 1 in 100,000 live births) that requires prompt surgical correction. It can occur in isolation or in association with junctional epidermolysis bullosa (JEB), and historically it has been managed with open surgery. Laparoscopic repair has only been reported in a few cases, so its safety and efficacy remain unclear. METHODS: We retrospectively reviewed five paediatric patients with Type I CPA (pyloric web) who were treated laparoscopically at three centers between 2020 and 2024. P

The Diagnostic Challenge of Rapunzel Syndrome: Multimodal Imaging Findings in a Child.

We report the case of a school-aged patient with attention-deficit/hyperactivity disorder who presented with a palpable epigastric mass. The initial abdominal ultrasonography indicated the presence of a heterogeneous space-occupying lesion in the upper abdomen. Subsequent computed tomography revealed a large intragastric mass with a mottled air-containing density, an imaging feature characteristic of a bezoar. Esophagogastroduodenoscopy confirmed a massive trichobezoar extending beyond the pylorus into the duodenum, consistent with Rapunzel syndrome. Although the endoscopic removal proved unsu

Type 2 diabetes risk alleles in peptidyl-glycine alpha-amidating monooxygenase influence GLP-1 levels and response to GLP-1 receptor agonists.

BACKGROUND: Type 2 diabetes (T2D) is a leading cause of morbidity and mortality worldwide. Despite the availability of multiple glucose-lowering agents, only half of individuals with T2D achieve the recommended HbA1c target of < 7.0%. Precision medicine approaches that leverage patient-specific markers offer a promising strategy to improve therapeutic outcomes. The PAM gene encodes the sole enzyme responsible for amidating bioactive hormones, including GLP-1, and harbors two hypomorphic T2D-risk alleles (p.D563G and p.S539W); however, whether PAM regulates GLP-1, a key amidated incretin hormon

Large Gastric Pyloric Gland Adenoma Mimicking Malignancy on FDG PET/CT.

A 51-year-old woman presented with intermittent upper abdominal pain for two years and unintentional weight loss recently. Gastroscopy revealed a large cardia mass protruding into the gastric lumen. CT examination suggested malignant gastric tumor. Subsequently, ¹⁸F-FDG PET/CT demonstrated heterogeneous intense uptake in the gastric mass. Pathologic examination identified a gastric pyloric gland adenoma by immunohistochemistry. The lesion was resected locally, and the patient recovered well.

Pyloric Metrics and Distensibility Plateau by Functional Luminal Imaging Probe in Pediatric Patients: A Pilot Study.

BACKGROUND & AIMS: Pyloric dysfunction contributes to chronic nausea, vomiting, and abdominal pain in adults and children. The Functional Luminal Imaging Probe (FLIP) has been used in adults to characterize pyloric distensibility as a marker of muscular hypertonicity and to predict symptomatic improvement to intrapyloric botulinum toxin injection (IPBI). FLIP parameters that identify pyloric dysfunction in children are unknown. In this retrospective cohort study, we aimed to characterize the relationship of pyloric FLIP parameters to clinical symptoms, gastric emptying (GE), and antroduodenal

Secondary Mesenteroaxial Gastric Volvulus in an Elderly Female: A Rare Presentation of an Occult Diaphragmatic Hernia.

Gastric volvulus is a surgical emergency characterized by rotation of the stomach greater than 180°, potentially leading to ischemia and perforation. Mesenteroaxial volvulus associated with a late-presenting diaphragmatic hernia in adults is an uncommon clinical entity. A 70-year-old woman with diabetes presented with acute abdominal distension and lactic acidosis. Nasogastric decompression yielded 3 L of coffee-ground aspirate, and computed tomography (CT) identified a mesenteroaxial gastric volvulus. Emergency laparotomy revealed a left-sided diaphragmatic hernia containing the gastric antru

Bouveret Syndrome Presenting as Duodenal Bulb Stenosis and Gastric Outlet Obstruction: A Case Report.

Bouveret syndrome, or gallstone obstruction of the duodenum, is a rare cause of gastric outlet obstruction, where gallstones migrate through a bilioenteric fistula and obstruct the pylorus or duodenum. Here, we present a case of a 61-year-old man who presented with intractable nausea and vomiting. Initial imaging, such as computed tomography of the abdomen and pelvis, showed gastric distention without an obvious obstructing lesion, and a subsequent upper endoscopy revealed a cholecystoduodenal fistula filled with gallstones, posterior to a stenotic region in the duodenal bulb. The calculi were

Case Report: Scar-Like Alterations of the Gastric Mucosa Observed Under Endoscopy in Collagenous Gastritis.

Collagenous gastritis (CG) is an uncommon disorder of the digestive system, characterized by marked thickening of the subepithelial collagen band and infiltration of inflammatory cells within the lamina propria. The etiology and pathogenesis of CG remain not fully understood, and its diagnosis is contingent upon histopathological evaluation, contributing to its status as an under-recognized condition. This article presents a case involving a 35-year-old male patient of Chinese descent, whose primary clinical manifestations included upper abdominal bloating and discomfort. Endoscopic assessment

Diabetic Gastroparesis in Evolution.

Diabetic gastroparesis (DGp) is a chronic and often debilitating complication of diabetes mellitus, characterized by delayed gastric emptying in the absence of mechanical obstruction. Over the past 6 decades, our understanding of DGp has evolved considerably. Clinically, DGp presents with symptoms such as nausea, vomiting, early satiety, bloating, and abdominal pain, which are not always proportional to the degree of gastric emptying delay, underscoring the nature of the disorder. To prepare this review, we conducted a targeted narrative synthesis of the literature, performing structured searc

Giant Morgagni hernia with multivisceral involvement: a case report of successful surgical repair in an elderly patient.

Morgagni hernia (MH) is a rare congenital diaphragmatic defect, representing 2%-4% of adult diaphragmatic hernias. It occurs through the sternocostal triangle, usually on the right side, and may remain undiagnosed because of nonspecific or absent symptoms. We report, a 75-year-old woman with hypertension and dyslipidemia presented with 3 days of progressive upper abdominal pain, dysphagia, nausea, vomiting, and poor oral intake. Examination revealed decreased breath sounds and bowel sounds over the right lung base. Chest radiography demonstrated right diaphragmatic elevation with a colonic air

A Rare Case of Pancreatic Neuroendocrine Tumor with Intraductal Extension in the Dorsal Duct of a Pancreas Divisum.

Background and Clinical Significance: Pancreatic neuroendocrine tumors (pNETs) rarely exhibit intraductal growth, a pattern that may mimic intraductal papillary mucinous neoplasms (IPMNs) or pancreatic ductal adenocarcinoma (PDAC). Preoperative recognition is challenging, particularly when associated with anatomic variants such as pancreas divisum. Case Presentation: A 63-year-old man with a history of pancreatic duct dilation presented with pruritus, weight loss, and lymphadenopathy. Cross-sectional imaging revealed a cephalopancreatic mass with upstream ductal dilatation. EUS demonstrated a

Combined Preoperative Risk Score Including sCD40, Leukocytes, and BMI Predicts Pancreas-Specific Complications After Pancreatic Cancer Surgery.

BACKGROUND: Pancreatic ductal adenocarcinoma (PDAC) has a poor prognosis, with a 5-year survival rate of 13%. Surgical resection followed by adjuvant chemotherapy remains the only curative approach. However, complications such as postoperative pancreatic fistula (POPF) and postpancreatectomy hemorrhage (PPH) often delay or prevent further treatment. Reliable preoperative biomarkers for predicting these complications are lacking. This study investigated soluble CD40 (sCD40) as a potential predictive marker for pancreas-specific complications after pancreatoduodenectomy (PD) in patients with PDA
