Comparison of Endoscopic and Intraoperative Approaches in the Management of Delayed Gastric Conduit Emptying After Minimally Invasive Esophagectomy: A Single-Center Retrospective Analysis.

Introduction: As multimodal therapy for esophageal cancer advances, addressing immediate and long-term functional outcomes following surgery has become more important. Despite surgical advancements, delayed gastric conduit emptying (DGCE) remains a primary cause of functional impairment after esophageal cancer resection. The literature addressing pylorus management following minimally invasive esophagectomy (MIE) is scarce. The effects of pyloric drainage with pyloromyotomy or postoperative approaches such as intrapyloric Botox injection or dilatation on the incidence and course of DGCE were t

Intraoperative endoscopic pylorus dilatation during minimally invasive Ivor Lewis oesophagectomy to prevent delayed gastric conduit emptying (DGCE): protocol for the WIDE randomised controlled trial.

INTRODUCTION: Oesophageal cancer has a poor prognosis and oesophagectomy has increasingly adopted minimally invasive techniques, but delayed gastric conduit emptying (DGCE) still occurs in ~10-50% of patients, depending on the used diagnostic criteria and definitions. DGCE can cause serious complications, such as aspiration pneumonia, and it prolongs hospital stay and the overall morbidity of patients. Surgical pyloric drainage procedures (pyloroplasty or pyloromyotomy) have not clearly improved outcomes. Endoscopic pyloric balloon dilatation (EPBD) effectively treats established DGCE, and eme

Maropitant Citrate Administration Significantly Decreases the Rate of Peristalsis in the Stomach and Jejunum and Does Not Significantly Alter Intestinal Diameter or Intestinal Wall Thickness in Healthy Adult Dogs.

Maropitant citrate (Cerenia) is a neurokinin-1 (NK-1) receptor antagonist labeled for the treatment of vomiting in dogs and cats. Anecdotal observation at the authors' institution suggests that patients receiving maropitant citrate occasionally display sonographic and radiographic signs of diffuse functional ileus. The goals of this prospective one-group pretest, posttest study were to determine if administration of maropitant citrate caused changes in ultrasonographic wall thickness and peristaltic rate at the pylorus, duodenum, or jejunum and/or a radiographic change in intestinal diameter o

A Rare Case of Idiopathic Rapid Gastric Emptying.

Idiopathic rapid gastric emptying (IRGE), also called idiopathic dumping syndrome, is a rare gastrointestinal (GI) motility disorder marked by an abnormally accelerated gastric emptying with no history of gastric surgery, diabetes mellitus, or other structural anomalies of the stomach or small intestine. This disorder has a similar presentation to classic dumping syndrome. Yet, it happens in patients with an intact stomach and pylorus, with no evidence of previous gastric surgeries. Due to its similarity with other endocrine hypoglycemic syndromes and functional GI disorders, IRGE remains misd

Duodeno-jejunal intussusception: an unusual manifestation of ampullary adenocarcinoma.

Duodeno-jejunal intussusception is highly unusual because of the retroperitoneal fixation of the duodenum, with only a limited number of cases documented in the literature. Furthermore, intussusception caused by a malignant ampullary lead point has not been reported previously. We present the case of a 58-year-old man who presented with obstructive jaundice for 8 months with no features of intestinal obstruction. Computed tomography imaging revealed dilation and deformation of the pancreaticobiliary tree due to duodenal intussusception. Intraoperative findings confirmed duodeno-jejunal intussu

Gastric Antral Perforation Caused by a Percutaneous Endoscopic Gastrojejunostomy Tube: A Case Report.

INTRODUCTION: Percutaneous endoscopic gastrojejunostomy (PEG-J) is used when gastric feeding via percutaneous endoscopic gastrostomy (PEG) is poorly tolerated due to gastroesophageal reflux or delayed gastric emptying. Gastrointestinal perforation related to PEG-J has been reported mainly in infants; gastric perforation in adults is extremely rare. We report a case of gastric antral perforation caused by the tube tip 12 days after exchange from a PEG tube to a PEG-J tube. CASE PRESENTATION: A 49-year-old man with severe dysphagia after brainstem hemorrhage underwent PEG placement using the in

Risk factors of reoperation after laparoscopic pancreaticoduodenectomy: insight from a learning curve analysis.

Reoperation is a serious adverse event after laparoscopic pancreatoduodenectomy (LPD). This study aims to identify risk factors of reoperation after LPD and evaluated the impact of the learning curve. Patients undergoing LPD from February 2014 to May 2024 were retrospectively reviewed. A logistic regression analysis was performed to identify predictors of unplanned reoperations within 90 days. Learning curve analysis was performed to compare surgical outcomes across different stages. Of the 445 patients undergoing LPD, 33 (7.4%) required unplanned reoperation within 90 days. Univariable and mu

The nonlinear correlation between serum albumin to creatinine ratio and length of stay in patients undergoing pylorus-preserving pancreaticoduodenectomy: A retrospective study.

The serum albumin to creatinine ratio (sACR) has emerged as a potential biomarker for predicting postoperative outcomes, yet its relationship with length of stay (LOS) in patients undergoing pylorus-preserving pancreaticoduodenectomy (PPPD) remains underexplored. This study aimed to investigate the nonlinear correlation between sACR and LOS in this patient population. A secondary analysis was conducted on retrospective data from 809 patients who underwent PPPD. LOS was dichotomized into short (≤ 41 days) and long (> 41 days) based on the median value. sACR was calculated as the ratio of serum

When can neuronal activity-dependent homeostatic plasticity maintain circuit-level properties?

Neural circuits are remarkably robust to perturbations that threaten their function. Activity-dependent homeostatic plasticity (ADHP) is a stabilizing mechanism that supports robustness by tuning neuronal ion conductances to combat chronic over- or under-activity. Its restorative capacity has been demonstrated in the pyloric circuit of the crustacean stomatogastric ganglion, whose neurons must burst in a specific order to coordinate digestive muscles. After disruption by physical and pharmacological manipulations, this circuit reliably recovers not only the activity levels of constituent neuro

Trichobezoar with Rapunzel syndrome: a case report of a 23-year-old female with trichophagia.

INTRODUCTION AND IMPORTANCE: Trichobezoars (hair concretions) are uncommon and are typically found in young women with psychiatric issues, who often deny eating their own hair (trichophagia). Continuous ingestion over time can cause hair to lodge in the stomach along with mucus and food. In some cases, the trichobezoar can extend past the pylorus into the jejunum, ileum, or even the colon, a condition known as Rapunzel syndrome. Only a few cases have been reported from Sudan. CLINICAL PRESENTATION: We reported a case of Trichobezoar with Rapunzel syndrome in a 23-year-old female with a histor

Successful surgical intervention of Bouveret's syndrome at a district general hospital: a case report and literature review.

INTRODUCTION: Bouveret's syndrome is a rare etiology of gastric outlet obstruction, comprising 3% of gallstone ileus (GI) cases. GI occurs with cholecystoenteric fistulating disease resulting in gallstone impaction within the gastrointestinal tract. There is a lack of unified guidance in GI management for surgeons. This, combined with its unpredictable symptomatology at presentation, results in delayed diagnoses and high mortality rates, despite advancements in surgical care. PRESENTATION OF CASE: A 71-year-old male with a 10-day history of abdominal pain and vomiting. Imaging revealed gallst

Pancreatic cancer mimicking ectopic pancreas origin: a rare case after neonatal pyloromyotomy.

Pyloromyotomy is a technique for relieving infantile hypertrophic pyloric stenosis and is performed almost safely without severe complications during long-term outcomes. To the best of our knowledge, this is the first reported case of pancreatic cancer arising from the pancreatic parenchyma embedded within the pyloric ring after neonatal pyloromyotomy. A 39-year-old woman with a history of infantile hypertrophic pyloric stenosis underwent a pyloromyotomy during the neonatal period and presented with progressive nausea and vomiting. Imaging studies suggested the presence of ectopic pancreatic o

Operative management for refractory chyle leakage after pancreatectomy: A case report and literature review.

Post-pancreatectomy chyle leak is a common complication typically managed conservatively. However, rare ISGPS grade C leaks that produce high output can lead to significant morbidity and often resist non-operative treatments. We report two cases of such refractory leaks successfully addressed through surgical intervention. We describe two complex cases of postoperative chyle leakage. The first case involves a 66-year-old female who underwent pylorus-preserving pancreaticoduodenectomy with portal vein resection. The second case is a 67-year-old female who underwent completion total pancreatecto

Rapunzel Syndrome Presenting as Acute Pancreatitis With Biliary Ductal Dilatation in a Child: A Rare Mechanical Etiology of Pediatric Pancreatitis.

Rapunzel syndrome is a rare manifestation of trichobezoar in which a gastric hair mass extends beyond the pylorus into the small intestine. It typically presents with abdominal pain, nausea, vomiting, early satiety, weight loss, or bowel obstruction. Presentation of acute pancreatitis is uncommon and may delay diagnosis. We report a previously healthy nine-year-old girl who presented with progressive epigastric pain, nausea, vomiting, decreased oral intake, and constipation. Laboratory evaluation demonstrated markedly elevated lipase levels consistent with acute pancreatitis. Abdominal ultraso
