Emerging Therapeutic Strategies for Graves' Disease: Current Advances and Future Perspectives.

Graves' Disease (GD) is the leading cause of autoimmune hyperthyroidism and is still considered a challenging clinical condition. Antithyroid drugs, radioactive iodine, and thyroidectomy have long been used as main treatments. These approaches are generally effective in restoring euthyroidism and controlling thyroid function, even if they cannot target the underlying autoimmune mechanisms implicated in the disease. As a result, relapses after medical therapy remain frequent, whereas definitive treatments often lead to permanent hypothyroidism and the need for a lifelong thyroid hormone replace

Increased Risk of Thyrotoxicosis After COVID-19 in Adolescents and Young Adults During the Omicron-Predominant Period: A Nationwide Matched Study.

OBJECTIVE: Thyroid dysfunction has been increasingly reported after the coronavirus disease 2019 (COVID-19) pandemic. To date, no population-based study has examined this association using more than 10 million matched pairs. We compared the risk of documented thyroid outcomes after COVID-19 with that after influenza during the Omicron-predominant period. METHODS: We conducted a nationwide retrospective cohort study using the Japanese National Database of Health Insurance Claims, including patients with COVID-19 between January 2022 and March 2023, and those with influenza between September 20

Adverse drug reactions and associated factors in patients taking levothyroxine and antithyroid drugs at Northwest hospitals, Ethiopia: A cross-sectional study.

BACKGROUND: Thyroid disease is a type of endocrinologic disease that should be treated early to avoid thyroid disease-related comorbidities, especially cardiovascular complications secondary to hyperthyroidism, which requires medication therapy to achieve an euthyroid state. However, patients are exposed to medication-related adverse drug reactions (ADRs) while taking drugs to treat thyroid disease. ADRs are unpredictable and unintended medication effects on the patient. OBJECTIVE: This study aimed to determine the prevalence and severity of ADRs and the contributing factors of medications us

Radioiodine or thyroidectomy in children and adolescents with Graves' disease: a structured literature review of efficacy and safety.

Graves' disease (GD) is the most common cause of hyperthyroidism in children and adolescents. When remission is not achieved with antithyroid drugs, definitive treatment with radioiodine therapy (RAI) or thyroidectomy is recommended. We conducted a structured literature review of studies indexed in PubMed/MEDLINE to evaluate the efficacy and safety of these approaches in the paediatric population. Twenty-one studies including 9,490 patients were analysed. RAI achieved high remission rates (85-96%), with hypothyroidism representing the expected therapeutic outcome. Thyroidectomy provided defini

Biopsy-proven Systemic Lupus Erythematosus and Antineutrophil Cytoplasmic Antibody-associated Vasculitis Overlap Syndrome Developed during Propylthiouracil Therapy: A Case-based Review.

A 31-year-old woman who had been receiving propylthiouracil (PTU) treatment for Graves' disease was referred to our hospital for the evaluation of hematuria and proteinuria. Laboratory tests showed positivity for myeloperoxidase-antineutrophil cytoplasmic antibody (ANCA), proteinase 3-ANCA, and anti-double-stranded DNA antibodies. A kidney biopsy revealed immunocomplex-mediated mesangial proliferative glomerulonephritis, consistent with lupus nephritis (LN) diagnosis. Additionally, tubulitis, peritubular capillaritis without immune complex deposition, and medullary angiitis were observed. We h

From storm to stability: The role of plasmapheresis in the stabilization of patients with high-risk thyrotoxicosis.

INTRODUCTION: Thyrotoxicosis in high-risk patients is a severe condition requiring prompt management, although conventional therapeutic approaches often present limitations. Therapeutic plasmapheresis has emerged as a potential option to achieve rapid clinical stabilization and facilitate a safe transition to definitive treatment by effectively reducing circulating thyroid hormone levels. The objective of this study was to evaluate the safety and efficacy profile of plasmapheresis in patients with high-risk thyrotoxicosis. MATERIALS AND METHODS: We conducted a retrospective multicenter study

Finite-time synchronization of quaternion-valued fuzzy memristive neural networks by an non-separation method via event-triggered control.

In this paper, the event-triggered control method is applied to study a class of generalized delayed quaternion-valued fuzzy Memristive Neural Networks. By constructing a novel Lyapunov function and designing an appropriate event-triggered controller, new event-triggered conditions and synchronization criteria are established. To handle the inherent complexity of quaternion operations, a nonlinear scaling technique is employed to compare distinct quaternion components, while an improved quaternion norm together with a symbolic function enables a holistic analysis of the quaternion-valued fuzzy

Impact of Minimal Dose Strategy Before Antithyroid Drug Discontinuation on Relapse Risk in Graves' Disease.

BACKGROUND: Although antithyroid drugs (ATDs) are widely used as the first-line therapy for Graves' disease, relapse after treatment discontinuation remains common. While a prolonged low-dose ATD therapy has been associated with improved remission rates, the impact of the minimal maintenance dose before discontinuation on relapse risk remains unclear. METHOD: We conducted a retrospective cohort study using electronic medical records from a thyroid specialty hospital in Japan. Patients newly diagnosed with Graves' disease between 2008 and 2024 who had discontinued methimazole (MMI) after recei

Comparative Efficacy of Radioiodine Therapy With Adjunctive Thionamides vs Either Treatment Alone in the Management of Graves Disease: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

CONTEXT: Antithyroid drugs (ATDs) and radioactive iodine (RAI) are the main nonsurgical modalities for Graves disease, but there is no agreement on the optimal approach for utilizing these 2 alternatives, either individually or in combination. OBJECTIVE: This systematic review and meta-analysis of randomized controlled trials (RCTs) aimed to evaluate the success (hypothyroidism/euthyroid) and failure (hyperthyroidism/relapse) of adjunctive (pre-RAI/post-RAI/both) and monotherapy with ATDs in Graves disease. METHODS: PubMed, CENTRAL, Scopus, and Web of Science were searched to December 2023 f

Beneath the Cure Lies the Risk: Carbimazole-Induced Angioedema in Graves' Disease.

Carbimazole is a widely used antithyroid medication for the treatment of hyperthyroidism and is generally well tolerated. Most adverse effects are mild and limited to cutaneous reactions. However, angioedema is a rare but potentially serious hypersensitivity response that may occur shortly after drug initiation. We describe a patient who developed acute facial angioedema soon after starting carbimazole therapy. The patient presented with sudden periorbital and lip swelling accompanied by mild difficulty in swallowing, without any respiratory compromise. There was no history of allergies, autoi

Iodide for the management of Graves' disease in pregnancy.

There is considerable variation among European, American, and Japanese guidelines regarding the management of Graves’ disease (GD) in early pregnancy. In contrast to European and the American Thyroid Association’s recommendations, the Japanese Thyroid Association includes potassium iodide (KI) as an alternative treatment option, reflecting long-standing clinical experience in iodine-sufficient populations. This review summarizes the historical background, pharmacologic mechanisms, and clinical evidence related to KI therapy in pregnant women with GD, with particular emphasis on data derived fr

Cellulose-based hydrogel SERS sensor for label-free detection and identification of antithyroid drugs.

Antithyroid drugs, while essential for the management of hyperthyroidism, can induce adverse reactions detrimental to patient health. Therefore, developing a rapid, quantitative method for monitoring therapeutic drug levels in blood is critical for the effective diagnosis and treatment of thyroid disorders. In this study, we fabricated a three-dimensional (3D) porous Ag NPs/C-CNF/PNIPAM hydrogel as a sensor for surface-enhanced Raman spectroscopy (SERS). Specifically, silver nanoparticles (Ag NPs) were stabilized within a poly(N-isopropylacrylamide) (PNIPAM) network using carboxylated cellulos

Aescin and rutin mitigate hyperthyroidism-induced multisystem dysfunction via antioxidant and anti-inflammatory mechanisms in a rat model.

Hyperthyroidism triggers systemic complications, including oxidative stress, dyslipidemia, hepatic injury, insulin resistance, and cardiovascular dysfunction. Conventional treatments such as Propylthiouracil (PTU) often exhibit adverse effects, highlighting the need for safer alternatives. We evaluated the therapeutic efficacy of the natural bioactives aescin and rutin, alone and in combination with PTU, in L-thyroxine–induced hyperthyroid rats. Comprehensive analyses included thyroid hormone levels, metabolic and hepatic biomarkers, oxidative stress indices, and histopathological/immunohistoc

Delayed Diagnosis of Thyroid Storm Presenting With Predominant Gastrointestinal Symptoms.

Thyroid storm is a rare but life-threatening endocrine emergency. Although mortality predictors have been described, diagnostic delay remains under-recognized, particularly in atypical presentations. A previously healthy man in his 20s presented with persistent nausea, vomiting, diarrhea, and 5-kg weight loss over one month. Abdominal computed tomography suggested superior mesenteric artery (SMA) syndrome. Despite supportive treatment, symptoms fluctuated and subsequently worsened with hemodynamic instability. Thyroid function tests performed during readmission revealed markedly elevated free

HTLV-1-associated uveitis mimicking thyroid-associated ophthalmopathy during antithyroid therapy: a diagnostic pitfall and management challenge.

SUMMARY: We report the case of a woman in her 70s, a known HTLV-1 carrier with recent-onset Graves' disease, who developed HTLV-1-associated uveitis (HAU) four weeks after the initiation of methimazole therapy. The diagnosis was established based on characteristic ophthalmologic findings and exclusion of other causes. Her uveitis resolved completely following local steroid injection; however, subsequent management of Graves' disease proved exceptionally challenging. Both methimazole and propylthiouracil eventually had to be discontinued because of safety concerns, including severe adverse reac

Drug cross-reactivity between methimazole and propylthiouracil causing recurrent pancreatitis in thyroid storm.

SUMMARY: Drug-induced pancreatitis represents 1.1% of acute pancreatitis cases. To date, eight cases of antithyroid drug (ATD)-induced pancreatitis have been reported, including seven associated with methimazole (MMI) and one with carbimazole; however, no cases related to propylthiouracil (PTU) have been reported in the literature. We report a patient with thyroid storm who developed acute pancreatitis during MMI therapy, with recurrence following PTU substitution, suggesting cross-reactivity between antithyroid agents. The patient demonstrated elevated serum immunoglobulin G4 (IgG4) concentra
