Knowledge, attitude, and contraceptive use among married women attending a maternity hospital in Kabul, Afghanistan: a cross-sectional study.

BACKGROUND: Family planning is a foundational component of reproductive health that empowers individuals to make informed decisions about childbearing. In Afghanistan, where maternal mortality remains critically high and contraceptive prevalence is low, understanding family planning dynamics is essential for improving reproductive outcomes. This study assessed knowledge, attitudes, and contraceptive use among married women attending a major maternity hospital in Kabul. METHODS: A cross-sectional analytical study was conducted among 356 married women attending Rabia Balkhi Maternity Hospital i

Cardiovascular and thromboembolic risks of sex hormone therapy: from contraception to transgender care.

The risk of venous thromboembolism is increased with combined oral contraceptives, rises with the dose of ethinylestradiol, and is higher with desogestrel, gestodene, drospirenone or cyproterone acetate than with levonorgestrel; the determinant is not the generation of the pill but the residual androgen-receptor activity of the progestin, which modulates the hepatic effect of the estrogen. Age over 35 years, obesity, smoking and a family history of thrombosis amplify it. Pills based on a native estrogen have a more limited hepatic impact and rank below most ethinylestradiol combinations in con

Comparative efficacy and safety of oral pharmacological and complementary interventions for premenstrual syndrome: a protocol for a systematic review and a network meta-analysis.

BACKGROUND: Premenstrual syndrome (PMS) affects around 48% of women of reproductive age worldwide, interfering with psychological and physical well-being as well as functioning. A multitude of interventions exists for PMS, including oral pharmacological interventions, such as antidepressants, combined oral contraceptives, or alternative and complementary interventions. While the use of pharmacological interventions for PMS is linked to multiple side effects, evidence is limited for complementary interventions. Further, placebo effects are high in PMS intervention studies, affecting the estimat

Diagnosis and management of polycystic ovary syndrome/polyendocrine metabolic ovarian syndrome: unmet needs and areas of clinical confidence among European endocrinologists.

BACKGROUND: Polycystic ovary syndrome (PCOS) or polyendocrine metabolic ovarian syndrome (PMOS) is a common, heterogeneous endocrine disorder whose diagnosis and management remain variable across clinical settings. Despite contemporary international guidance, several diagnostic definitions and therapeutic decisions are not consistently applied. OBJECTIVE: To identify persistent unmet needs and areas of clinical confidence in PCOS/PMOS care, using the 2023 survey of the European Society of Endocrinology PCOS Special Interest Group as the principal data source. METHODS: This manuscript present

The Use of the Drospirenone 4 mg-Only Pill in Overweight and Obese Women with Polyendocrine Metabolic Ovarian Syndrome: A Retrospective Cohort Study.

Objective: Polycystic ovary syndrome is frequently associated with hyperandrogenemia-related hirsutism, which creates notable cosmetic concerns for many affected women. Although multiple antiandrogenic treatments exist, many patients remain dissatisfied, and combined oral contraceptives are often contraindicated due to metabolic and cardiovascular risk factors. This study evaluates whether a drospirenone-only pill improves hormonal profiles and hirsutism in PMOS compared to cyclic dydrogesterone. Methods: This retrospective cohort study assessed hyperandrogenemia and hirsutism using the modifi

Risk of ischemic and hemorrhagic stroke in users of progestogen-only contraceptives: A Danish Registry Study.

OBJECTIVE: We studied the risk of ischemic (IS) and hemorrhagic stroke (ICH) in users of progestogen-only contraceptives (POCs): levonorgestrel-releasing intrauterine device (LNG-IUD), progestogen-only pill (POP), implant and injection. STUDY DESIGN: In this nationwide cohort study using Danish registry data for all women aged 18-49 years, 2004-2021, we estimated incidence rate ratios (IRRs) for IS and ICH associated with POC use. Comparisons were made with non-use of hormonal contraception and with combined hormonal contraceptives (CHC) with ethinylestradiol containing equivalent progestogen

The effects of intrauterine devices on anti-Mullerian hormone levels-a mini-review.

Intrauterine devices (IUDs) are contraceptive devices utilized by many women, especially those who struggle with the use of oral contraceptives. As the age of childbearing rises, it is crucial to have reliable assessments of ovarian reserve, such as anti-Müllerian hormone (AMH) levels. In efforts to provide clinicians with optimal knowledge in the evaluation of fertility, this review investigates the relationship between IUDs and AMH levels. A detailed search of PubMed articles was conducted with six relevant articles discovered. Hormonal IUDs have been shown to either decrease or have no sign

Elevated fasting glucose and triglycerides in combined oral contraceptive users compared to naturally cycling females.

The effects of oral contraceptive pills (OCPs) on metabolism and body composition remain unclear. This study aimed to compare fasting glucose and lipid metabolism between naturally cycling (NAT) females, second-(OCP2), and third-generation (OCP3) OCP users during low-(LH) and high-hormone (HH) phases. Forty-nine healthy females (age: 21 ± 2 years, BMI: 23.5 ± 3.8 kg/m2) were assessed. Participants were assessed in two randomized fasting blood draws during: (1) LH phase (NAT: early follicular, OCP: placebo pill week), and (2) HH phase (NAT: mid-luteal, OCP: third week of active pill use) to ass

THE Environmental Impact of Contraceptive Methods - A Systematic Review.

Contraception contributes to healthcare's global environmental footprint through pharmaceutical emissions and disposable waste. Steroid hormones used in hormonal contraception are widely detected in aquatic environments, raising concerns regarding their ecotoxicological risks. This review synthesizes environmental impacts of registered (non-)hormonal contraceptives, aiming to compare their environmental sustainability and estimate comparative ecotoxicological risk profiles of hormonal methods. A systematic literature search of Medline, Embase, Web of Science Core Collection, and Scopus was con

Oral contraceptive use and anterior cruciate ligament injury or reconstruction in women: A systematic review and meta-analysis of very low-certainty evidence.

PURPOSE: To determine whether oral contraceptive use is associated with anterior cruciate ligament injury or reconstruction in women and to examine whether associations vary according to age, contraceptive formulation, duration of use and outcome definition. METHODS: PubMed, Scopus, Embase and Latin American and Caribbean Health Sciences Literature (LILACS) were searched through 31 July 2026 (LILACS rerun with multilingual terms on 16 September 2026). Comparative observational studies were meta-analysed. The primary analysis pooled adjusted or matched odds ratios (ORs) for reconstruction or o

Revisiting the relation between oral contraceptive use and attentional engagement in everyday life: The role of formulation.

Oral contraceptives (OCs) are composed of artificial estrogen (ethinyl-estradiol) and/or progesterone (progestin), which can attach to receptors in the brain and potentially influence cognition. One prior study suggested that OC use was associated with increased mind-wandering - implying that OC use was associated with detriments to attention. In contrast, two subsequent well-powered conceptual replications of this initial finding revealed little evidence that OC use was related to increases in inattentiveness (including mind-wandering). However, these later studies collapsed across different

Oral Contraceptives, Concussion History, and Contact-Sport Exposure: Multimodal MRI and Brain Injury Biomarkers in Female Athletes.

Neuroimaging and blood-based biomarkers are increasingly utilized to identify long-term effects of concussion and contact-sport exposure (CSE), but female athletes are underrepresented in this work, despite potential sex-related differences. Early research indicates that widely used oral contraceptives (OCs) influence brain structure and function and concussion-related biomarkers. This study investigated whether current oral contraceptive (OC) use moderates associations of concussion history and CSE with magnetic resonance imaging (MRI) measures and brain injury biomarkers in female athletes.

Hormonal Options for Perimenopause: A Clinician Toolkit.

Perimenopause is a clinically heterogeneous stage of reproductive aging with unique physiology distinct from menopause. It is variably defined, lacks specific guidelines or practice recommendations, and can be challenging for clinicians to manage. Patients may present with myriad symptoms, including mood and cognitive changes, abnormal uterine bleeding, sleep disruption, and vasomotor symptoms, as well as sexual concerns. Managing such complaints while taking into consideration contraceptive needs and evolving cardiometabolic risk can be nuanced. Hormonal contraceptives are commonly prescribed

Hormonal contraception and breast tenderness: A Danish cross-sectional study.

BACKGROUND: Breast tenderness is an acknowledged side effect of hormonal contraception, reflecting hormonal effects on breast tissue, but evidence on variation across contraceptive types, doses and formulations remains limited. METHODS: We conducted a cross-sectional study among Safe Choice survey respondents aged 18-39 years. Associations between hormonal contraceptive use and self-reported breast tenderness were assessed using logistic regression. RESULTS: We included 112,558 women (median age 26), of whom 17,478 had experienced breast tenderness (15.5 %). Combined oral contraceptives were

Mitochondrial metabolic plasticity in cancer: hybrid bioenergetic states and translational targeting in surgical oncology.

Mitochondrial metabolism is increasingly recognized as an active determinant of tumor growth and cancer-cell adaptation rather than solely a source of cellular energy. Mitochondrial metabolism supports tumor anabolism, while mitochondrial DNA (mtDNA) alterations can affect respiratory chain function, redox homeostasis, and adaptation to changing tumor environments. At the same time, tumor metabolism is heterogeneous and flexible and cannot be explained by a uniform Warburg model. Tumor cells may use glycolysis, oxidative phosphorylation (OXPHOS), or hybrid glycolysis/OXPHOS states, with the re
