Identification and management of psychiatric medication-induced nutrient depletion.

PURPOSE: This article reviews the evidence for drug-nutrient (DN) interactions associated with the chronic use of commonly prescribed psychiatric medications and suggests management strategies. SUMMARY: Nutrient deficiencies due to DN interactions pose a risk for poorer patient outcomes. However, DN interaction studies and guidelines on preventing and managing DN abnormalities are lacking, with limited information for healthcare providers and patients to use as a resource. DN interactions can be caused by chronic use of psychiatric medications. Carbamazepine (CBZ), valproic acid (VPA), tricyc

Targeting Brain Plasticity: Vagal Nerve Stimulation as a Therapy for Autism-Like Symptoms in a Valproic Acid Mouse Model.

Autism spectrum disorder (ASD) is a multifactorial neurodevelopmental condition defined by social deficits, stereotypical or repetitive behaviors, and anxiety. This study evaluates the therapeutic potential of transauricular vagal nerve stimulation (tVNS) in a valproic acid (VPA)-induced mouse model of ASD. The study comprised three groups: the control + sham (saline-treated offsprings receiving sham stimulation), the autistic + sham (VPA-treated offspring receiving sham stimulation), and the autistic + tVNS (VPA-treated offsprings receiving tVNS). Male C57BL/6 mice exposed to VPA on embryonic

Capability, opportunity and motivation for shared decision-making about valproate as an antiseizure medication treatment for epilepsy in women with pregnancy potential: A qualitative study of patient perspectives.

OBJECTIVES: Valproate is a highly effective antiseizure medication but carries significant teratogenic and neurodevelopmental risks to offspring if used during pregnancy. A shared decision-making (SDM) approach is recommended to guide clinician/patient discussions on valproate suitability for women with pregnancy potential. This study applied the Capability, Opportunity, Motivation-Behaviour (COM-B) theoretical framework to explore barriers and facilitators to SDM in valproate prescribing from the perspectives of women with epilepsy who have pregnancy potential. DESIGN: Qualitative study usin

Valproate Releases the Latent Differentiation Capacity of Neuroblastoma by Targeting SP1/CDK4 Signaling.

Differentiation therapy represents a standard therapeutic regimen for high-risk neuroblastoma (HR-NB), demonstrating significant efficacy in suppressing tumor recurrence and improving patient survival rates. Retinoic acid (RA) remains the sole clinically approved differentiation-inducing agent for HR-NB, yet its therapeutic application is substantially limited by inherent drug resistance. Here, we found that valproate (VPA) exhibited dose-dependent inhibition of tumorigenicity in NB cells SH-SY5Y, SK-N-BE and retinoic acid (RA)-resistant SK-N-AS, along with the induction of G1-phase arrest and

Bayesian-Based Pharmacokinetic Framework Integrated with Therapeutic Drug Monitoring for Assessing Adherence to Antiseizure Medications: A Clinical Trial Simulation Study.

BACKGROUND: Adherence to antiseizure medications (ASMs) is a cornerstone of effective epilepsy management. However, current consensus guidelines for assessing medication adherence via therapeutic drug monitoring (TDM) may neglect individual patient characteristics, thereby compromising the accuracy of adherence assessments. OBJECTIVE: This study proposed an innovative Bayesian-based pharmacokinetic (PK) framework integrated with TDM data to address the above limitations, with a focus on 14 widely prescribed ASMs, including brivaracetam, carbamazepine, clobazam, eslicarbazepine acetate, lacosa

Circadian Clock Dysfunction Exacerbate Autistic-Like Behaviour and Wnt/β-Catenin Signalling Dysregulation in ASD Mice and Treatment of Melatonin.

Between 50% and 80% of children diagnosed with Autism Spectrum Disorder (ASD) are estimated to experience sleep disturbances, highlighting the importance of exploring the role of the circadian clock in ASD development. Previous studies have identified a potential link between Bmal1 deficiency and ASD in mouse models. In this study, we first characterise the expression patterns of circadian proteins. Subsequent behavioural tests and western blot analyses revealed that mice exposed to valproic acid (VPA) displayed autistic-like behaviours, along with altered circadian protein expression and disr

Valproate causes inositol depletion in yeast by decreasing levels of phosphatidic acid and increasing Opi1-mediated repression of INO1 expression.

Valproic acid (VPA) is a widely prescribed mood stabilizer used in the pharmacological management of bipolar disorder (BD), a psychiatric illness that affects 2% of the world's population. Although VPA has been in use for four decades, the therapeutic mechanism of action has not been determined. Inositol depletion is a proposed mechanism, but how VPA depletes inositol is not understood. Using the yeast model, in which the inositol biosynthetic pathway has been well characterized, we show that VPA supplementation leads to decreased levels of phosphatidic acid (PA), including decreased PA specie

Polycystic ovary syndrome in women with bipolar affective disorder or epilepsy exposed to valproic acid: a nationwide 16-year cohort study.

BACKGROUND: Both bipolar disorder (BD) and epilepsy (ES) have been linked to polycystic ovary syndrome (PCOS) that is one of the most common endocrine disorders in women of reproductive age. The antiseizures medication valproate is widely used in the treatment of both disorders but has been suspected to increase the risk of PCOS. Previous studies have been limited by small sample sizes and heterogeneous definitions. We aimed to investigate the association between valproate exposure and incident PCOS in females with BS and ES. METHODS: We conducted a register-based cohort study including all f

Development of cerebral lateralization in children exposed to fetal antiseizure medications: A prospective cohort study at age 6 years.

OBJECTIVE: We previously reported in a prior cohort that fetal exposure to antiseizure medications (ASMs), especially valproate, was associated with reduced right-handedness and poorer verbal versus nonverbal performance in children at age 6 years. Given changes in prescribing practices in epilepsy over the past 2 decades, we enrolled a new cohort of the Maternal Outcomes and Neurodevelopmental Effects of Antiepileptic Drugs (MONEAD) study to determine whether fetal exposure to newer generation ASMs is associated with altered cerebral lateralization in children at age 6 years and to assess exp

Possible therapeutic repositioning of valproic acid: From epileptic seizures to acute kidney injury.

AIMS: Valproic acid is an inhibitor of histone deacetylases (HDACs) and is used for the treatment of epileptic seizures. The drug (sodium valproate) has antihypertensive effects, and HDACs modulate renal Na+-transporting ATPases. The aim was to investigate whether valproate could be repositioned as an adjunctive drug for the prevention of acute kidney injury (AKI). METHODS: AKI due to ischemia followed by reperfusion (I/R) was induced by clamping of the renal arteries in rats that had received valproate or a vehicle for 20 days. Plasma creatinine and urea concentrations, and glomerular filtra

Does Altered Membrane Glycosylation Contribute to Neurodevelopmental Dysfunction in Autism Spectrum Disorder?

Neuronal development relies on cell-surface glycoconjugates that function as complex bioinformational codes. Recently, altered glycosylation has emerged as a central mechanistic theme in the pathophysiology of autism spectrum disorder (ASD). Critically, the brain maintains a distinctively restricted glycan profile through strict biosynthetic regulation, creating a specialized landscape highly susceptible to homeostatic perturbation. This "membrane-centric vulnerability" spans both glycoproteins and glycolipids; however, evidence remains fragmented, obscuring their pathogenic interplay. To brid
