Chronic Diazepam Reveals Excessive Homeostatic Gain in SOD1G93A Mouse Spinal Motoneurons.

Motoneurons are under strong pressure to maintain stable motor output throughout an individual life, through homeostatic regulation of their electrical properties. Dysregulated spinal motoneuron excitability has long been implicated in the pathogenesis of amyotrophic lateral sclerosis (ALS). Recent work in SOD1G93A mice suggests that the homeostatic response of motoneurons becomes dysregulated as cellular processes are disrupted by the disease, causing fluctuations in motoneuron electrical properties. Yet, few studies directly test whether ALS motoneurons respond differently than wild-type mot

Variations in S-100Β and Neuron-Specific Enolase Levels During Functional Endoscopic Sinus Surgery Under Moderately Controlled Hypotension Using Four Distinct Anesthetic Protocols: A Randomized Controlled Study.

Background and Objectives: Controlled hypotension during functional endoscopic sinus surgery (FESS) improves surgical field visibility but may pose a risk of subclinical cerebral hypoperfusion. Serum S100Β and neuron-specific enolase (NSE) are established biomarkers of glial and neuronal injury and may reflect perioperative neuroprotection associated with different anesthetic regimens. This study evaluated the effect of four anesthetic protocols on perioperative brain biomarker release during FESS. Materials and Methods: In this single-center, randomized, controlled trial, 88 adult patients (A

Effect of evening dexmedetomidine nasal spray on postoperative sleep disturbance in patients undergoing laparoscopic cholecystectomy: protocol for a multicentre, prospective, randomised, double-blind, controlled trial.

BACKGROUND: Postoperative sleep disturbance (PSD) is a common complication following laparoscopic cholecystectomy (LC). It is associated with delirium, cognitive decline and delayed recovery. Effective preventive strategies are currently lacking. Dexmedetomidine, a highly selective α₂-adrenergic receptor agonist, offered in a nasal spray formulation with high bioavailability and convenient administration, may present a novel approach for PSD prevention. OBJECTIVES: The primary objective is to evaluate the effect of evening dexmedetomidine nasal spray on the incidence of PSD on the first posto

Factors associated with rapid pediatric acute seizure emergency treatment: Quality Improvement in Time to Treat Status Epilepticus baseline cohort.

OBJECTIVE: Status epilepticus (SE) treatment is more effective when benzodiazepines (BZDs) are given soon after SE diagnosis. The Quality Improvement in Time to Treat Status Epilepticus (QuITT-SE) trial is a multicenter, randomized, stepped-wedge effectiveness-implementation hybrid study aimed at improving time to SE treatment. We report the QuITT-SE baseline cohort and test the hypothesis that acute seizure emergency episodes treated promptly differ in clinical and demographic features from those with delayed treatment. METHODS: Data were summarized from 371 consecutive acute seizures episod

Evaluation of Oculomotor Function Following Sleep Improvement With Lemborexant in Older Adults With Insomnia.

OBJECTIVE: Insomnia impairs cognitive and brain function, and its treatment improves outcomes. Eye movements are objective biomarkers of neurocognitive state and are sensitive to sleep loss, but whether treatment improves oculomotor function remains unclear. We examined associations between sleep improvement and eye movement changes after hypnotic intervention. METHODS: In this open-label, single-arm study, 31 Japanese adults aged ≥ 50 years with insomnia and preserved cognition received nightly lemborexant (5-10 mg) and were assessed at baseline, week 4, and week 12. Eye movements were measu

Economic impact of switching to licensed ready-to-administer injectable anaesthetic and critical care medicines in the National Health Service: a model-based evaluation of prefilled syringes.

BACKGROUND: Licensed, ready-to-administer injectable medicines can reduce medication errors, minimise waste, and streamline perioperative workflows, but higher acquisition costs have limited uptake in England. This study evaluated the economic impact of switching selected anaesthetic and critical care medicines from conventional ampoules and vials to licensed prefilled syringes within NHS practice. METHODS: An economic model compared current mixed-use practice with a hypothetical switch to 100% licensed ready-to-administer products for eight medicines: epinephrine 1 mg in 10 ml, ephedrine 30

Effect of 24-hour sedation during mechanical ventilation on hematological, biochemical, and coagulation parameters in bitches: A randomized clinical study.

The effects of prolonged sedation on hematological, biochemical, and coagulation variables have not been previously investigated in veterinary medicine. Twelve healthy adult mixed-breed female dogs were included. Blood samples (6 mL) were collected at baseline (M0) for complete blood count (CBC), biochemical testing, prothrombin time (PT), activated partial thromboplastin time (PTT) and fibrinogen. After M0, dogs were allocated into two groups: midazolam-fentanyl-propofol group (MFG; n = 6), which received a continuous infusion of midazolam (0.5 mg/kg/h), fentanyl (10 µg/kg/h), and propofol (1

Comparison of propofol and propofol-ketamine anesthesia on optic nerve sheath diameter in endovascular cerebral aneurysm procedures: a prospective randomized study.

BACKGROUND: Endovascular treatment of unruptured intracranial aneurysms requires anesthetic strategies that maintain cerebral perfusion and hemodynamic stability while avoiding increases in intracranial pressure (ICP). Optic nerve sheath diameter (ONSD) measured by ultrasonography is commonly used as a noninvasive surrogate marker of ICP. This study compared propofol and propofol-ketamine anesthesia with respect to perioperative ONSD, cerebral oxygenation, and hemodynamic parameters during endovascular aneurysm procedures. METHODS: In this prospective, randomized, single-center, observer-blin

Development, Early Verification and Application of a North American Pediatric PBPK Model, Comparison With the North European Pediatric Population.

Physiologically based pharmacokinetic (PBPK) models are increasingly used in pediatric drug development and matching such models to the target population is an important consideration. A US pediatric PBPK model was developed incorporating four sub-groups describing physiology and enzymatic/transporter levels for White, African American, Hispanic/Latino and Asian subjects. Virtual populations could then be created reflecting the demographics of the clinical study design. Ten different drugs (14 studies) were used to verify the pediatric PBPK model performance. Predicted PK parameters showed rea

High-order brain interactions during ketamine-induced state changes: A functional marker of response in late-life treatment-resistant depression?

Ketamine is a fast-acting intervention for treatment-resistant depression (TRD), yet only a subset of patients show robust clinical response, and the underlying neural mechanisms remain unclear. High-order interactions (HOI) derived from multivariate information theory provide a framework for examining nonlinear dependencies among brain regions beyond pairwise connectivity. One such metric, the O-information, captures the balance between synergistic and redundant interactions across three or more variables. In this secondary analysis of a randomized, double-blind, midazolam-controlled trial (N
