Tumor size and vascular and perineural invasion predict mesenteric involvement in small-intestinal neuroendocrine tumors.

Small-intestinal neuroendocrine tumors (SI-NETs) frequently give rise to mesenteric masses with fibrotic stroma, yet the primary-tumor features that signal mesenteric involvement are not well defined. We performed a single-centre retrospective study of 70 resected, well-differentiated SI-NETs, dichotomized into a primary-only cohort without mesenteric lesions (n = 21) and a mesenteric-lesion cohort with at least one mesenteric mass (n = 49), using the endpoint “any mesenteric lesion”. Clinicopathologic variables, including tumor size, grade, multiplicity, vascular invasion, and perineural inva

Diagnostic Pearls and Pitfalls in the Evaluation of Small Biopsies From the Bile Duct and Ampulla.

CONTEXT.—: Histopathologic evaluation of bile duct and ampullary biopsies can be challenging. Biopsies from these sites are often tiny, scant, and/or fragmented. When assessing these biopsies, there is significant overlap between reactive atypia and malignancy, in situ precursor lesions can be misinterpreted as malignancy, and nonprimary tumors can mimic primary disease. OBJECTIVE.—: To provide diagnostic pearls and pitfalls in the evaluation of small biopsies from the biliary tract. DATA SOURCES.—: Literature review of published studies and the author's own observations. CONCLUSIONS.—: Bec

68 Ga-DOTATOC PET/CT of Glomus Tumor of the Small Bowel.

A 57-year-old woman had an incidental finding of a hypervascular small bowel lesion on a contrast-enhanced CT scan performed for a suspected renal graft infection. The morphologic features of the lesion were suggestive of a neuroendocrine tumor (NET), but serum chromogranin A and urinary 5-HIAA were negative. 68 Ga-DOTATOC PET/CT showed high SSTR-2 expression in the lesion, suggesting a NET. However, pathologic examination revealed diffuse alpha-smooth muscle actin positivity, consistent with a glomus tumor. This case highlights the utility of PET/CT for in vivo molecular tumor characterizatio

Beyond the Common: A Pictorial Review of Rare and Atypical Periampullary Lesions.

The periampullary region, defined as the area within 2 cm of the major papilla of the duodenum, is associated with various benign and malignant conditions. Although periampullary cancers are rare, they are the third leading cause of cancer-related deaths in the Western world. Tumors in this area often mimic benign lesions due to overlapping clinical presentations. Diagnostic imaging techniques, including computed tomography, magnetic resonance imaging, ultrasonography, and MR cholangiopancreatography, are commonly used in clinical practice and typically confirmed by pathological analysis. Benign focal small bowel lesions : a review of the features on multiphasic multidetector computed tomography.

The detection of small bowel lesions and their discrimination from normal bowel tissue are the most elementary and important factors in the computed tomography (CT) diagnosis of focal small bowel lesions. The detection and characterization of small bowel lesions have recently improved with advances in CT technology. Post-contrast multiphasic multidetector CT (MDCT) aids in the assessment of the vascular features of focal small bowel lesions. Understanding the typical multiphasic MDCT features of focal small bowel lesions is valuable because CT features overlap, and the severity and associated

Differentiation of non-ampullary duodenal carcinoma from intestinal-type ampullary carcinoma by tailored multidetector CT: utility of four imaging criteria.

OBJECTIVE: To evaluate the diagnostic accuracy of four imaging criteria obtained with a tailored duodenal MDCT to differentiate non-ampullary duodenal carcinoma (NADC) and intestinal-type ampullary carcinoma (ITAC). MATERIALS AND METHODS: In this retrospective single-center study, patients with resected ITAC and NADC who underwent preoperative contrast-enhanced MDCT using a tailored duodenal protocol were included. Three radiologists independently reviewed each MDCT, using four imaging criteria to differentiate the two cancer types: lesion shape, ductal cutoff, duodenopancreatic groove fixati

Atypical presentation of systemic lupus erythematosus flare with ileocecal ulceration and lymphadenopathy in a tuberculosis-endemic region.

RATIONALE: Systemic lupus erythematosus can involve the gastrointestinal tract, but lupus enteritis (LE) with ileocecal ulceration and generalized lymphadenopathy as an initial presentation is exceptionally rare. In regions where tuberculosis (TB) is endemic, overlapping clinical and imaging findings can lead to diagnostic delays and inappropriate treatment. This case highlights the challenges in differentiating lupus enteritis from abdominal TB and underscores the importance of considering autoimmune etiologies in atypical gastrointestinal presentations. PATIENT CONCERNS: A 20-year-old woman

A Systematic Approach to the Histologic Evaluation of Terminal Ileal Biopsies.

BackgroundIleal biopsies represent a critical diagnostic tool in gastroenterological practice, with particular relevance in the evaluation of Crohn's disease (CD). However, the ileal mucosa can harbor numerous nonneoplastic and neoplastic conditions that may clinically and histologically mimic CD. Accurate interpretation requires a nuanced understanding of normal ileal histology and its variants, alongside a structured diagnostic algorithm.ObjectivesTo delineate the spectrum of pathologic processes affecting the ileum and establish a systematic diagnostic approach, while highlighting potential

A case of Meckel's diverticulum misdiagnosed as gastrointestinal stromal tumor: case report and literature review.

Meckel's diverticulum (MD) is a common congenital gastrointestinal malformation often containing ectopic gastric mucosa. It is prone to ulceration and painless lower gastrointestinal bleeding, predominantly affecting children and adolescents. Gastrointestinal stromal tumors (GISTs), which arise from Cajal interstitial cells, are predominantly mesenchymal tumors that occur predominantly in middle-aged and elderly individuals. Both conditions may occur in the small intestine, presenting gastrointestinal bleeding and exhibiting overlapping imaging features, which pose challenges for clinical diff

Symptomatic ileal nodular lesion: a persisting diagnostic challenge.

This case report details the diagnostic and therapeutic approach of a young male presenting with an ileal nodular lesion of uncertain aetiology whose final diagnosis was gastric heterotopia within a Meckel's diverticulum. The case underscores the challenges of diagnosing ileal lesions due to the overlapping of clinical presentations of benign and malignant conditions. It highlights the importance of integrating advanced diagnostic tools and a multidisciplinary team approach to ensure accurate diagnosis and effective treatment.

Jejunal calcifying fibrous tumor: A jigsaw puzzle with a pool of diagnostic dilemma.

Calcifying fibrous tumor (CFT) is a very rare benign fibroblastic tumor featuring a wide anatomical distribution and mimicking various spindle cell tumors. CFT has been documented in soft tissues, gastrointestinal tract (GIT), neck, adrenal glands, and pleura. Within the GIT, it is reported in the stomach, small bowel, large intestine, stomach, esophagus, and appendix. The occurrence of CFTs in the gastrointestinal tract presents a diagnostic dilemma, firstly due to the rarity of the lesion and secondly, due to the occurrence of a variety of other stromal lesions in the gastrointestinal tract

Prominent black esophagus, Wischnewsky spots, and black duodenum in a fatal hypothermia case with an underlying diabetic ketoacidosis.

Hypothermia can occur in patients with diabetic ketoacidosis (DKA), and these two conditions can exacerbate each other. Fatal hypothermia and DKA have overlapping features and findings such as Wischnewsky spots (WS), black esophagus, basal subnuclear vacuolization in the renal tubule, dehydration, and increased acetone levels. Therefore, it may be challenging to differentiate or clarify the context of these two conditions. Herein, we report a case of a 49-year-old man with type 1 diabetes who was found lying in his house in mid-winter. He experienced cardiopulmonary arrest 10 h after the initi

Pancreaticoduodenal Groove: Spectrum of Disease and Imaging Features.

The pancreaticoduodenal groove (PDG) is a small space between the pancreatic head and duodenum where vital interactions between multiple organs and physiologic processes take place. Muscles, nerves, and hormones perform a coordinated dance, allowing bile and pancreatic enzymes to aid in digestion and absorption of critical nutrition. Given the multitude of organs and cells working together, a variety of benign and malignant entities can arise in or adjacent to this space. Management of lesions in this region is also complex and can involve observation, endoscopic resection, or challenging surg

Updates in interaction of gastroesophageal reflux disease and extragastroesophageal digestive diseases.

INTRODUCTION: Gastroesophageal reflux disease (GERD) is one of the common chronic diseases with prevalence increasing in the last decades. Because of its prevalence and chronicity, GERD affects the quality of life and increases health-care costs. Gastroesophageal diseases leading to GERD have been thoroughly studied, while extragastroesophageal digestive diseases (EGEDDs) may coexist with GERD and affect the occurrence and persistence of GERD symptoms and therapeutic effect. AREAS COVERED: In this review, we aim to summarize the EGEDDs correlated with GERD and explore the potential mechanisms

Malignant gastrointestinal neuroectodermal tumor presenting with small intestinal obstruction: A case report.

Malignant gastrointestinal neuroectodermal tumors (GNETs) are rare malignant mesenchymal neoplasms. To our knowledge, only 99 cases have been reported worldwide. The tumor has an aggressive malignancy, with a rapid progression. The histological features of GNET overlap with those of clear cell sarcoma, which contain Ewing sarcoma breakpoint region 1 mutation. GNETs lack melanocyte-specific markers, while clear cell sarcoma exhibits melanocytic differentiation. Various symptoms have been reported previously, and the most reported lesion is in the small bowel. The patient was a 69-year-old man w

The Role of Gut Microbiota in Duodenal-Jejunal Bypass Surgery-Induced Improvement of Hepatic Steatosis in HFD-Fed Rats.

Bariatric surgery including duodenal-jejunal bypass surgery (DJB) improves insulin sensitivity and reduces obesity-associated inflammation. However, the underlying mechanism for such an improvement is still incompletely understood. Our objective was to investigate the role of the gut microbiota in DJB-associated improvement of hepatic steatosis in high fat diet (HFD)-fed rats. To study this, hepatic steatosis was induced in male adult Sprague-Dawley rats by feeding them with a 60% HFD. At 8 weeks after HFD feeding, the rats were subjected to either DJB or sham operation. HFD was resumed 1 week

Adenomatous neoplasia in the papilla of Vater endoscopic and/or surgical resection?

BACKGROUND: Adenomatous neoplasia in the papilla of Vater needs to be resected in order not to progress. It can be challenging to distinguish between early ampullary malignant lesions and non-invasive adenomas, due to the overlap in symptoms and radiological findings. This retrospective study describes the different findings and treatment decisions taken prior to endoscopic and/or surgical resection of ampullary adenomatous lesions. MATERIALS AND METHODS: Patients treated with endoscopic and/or surgical resection for suspected or verified ampullary adenomatous neoplasia, between January 2006
