EJHF expert consensus statement on the diagnosis and management of hypertrophic cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) is the most prevalent genetic cardiac disease and a leading cause of heart failure, arrhythmia, and sudden cardiac death in both young and older adults. This consensus document was developed by a multidisciplinary panel of European and U.S. experts in HCM, imaging, electrophysiology, genetics, and heart failure. While it aligns with the 2023 ESC and 2024 AHA/ACC guidelines on HCM, the paper addresses areas where clinicians might require further guidance. Key sections include phenotypic classification, diagnostic strategies incorporating multimodal imaging and

Fatal overdose of antihypertensive and antidiabetic medications: A case report and a brief review of the literature.

Calcium channel blockers (CCBs) and hypoglycaemic medications are widely prescribed for the management of hypertension and diabetes. However, their overdose, either accidental or intentional, can lead to morbidity and mortality. This report details a case of combined ingestion of antihypertensive and antidiabetic medication overdose in a 36-year-old male, leading to a fatal outcome. The postmortem biochemical and toxicological analysis, along with histopathological findings, provided key insights into the fatal outcome, highlighting the forensic and toxicological significance of such overdoses

Proposed echocardiography-guided flowchart for calcium channel blocker overdose: distinguishing vasoplegia from cardiogenic shock.

BACKGROUND: Calcium channel blocker (CCB) overdose can present as vasoplegic or cardiogenic shock, but current guidance does not operationalize bedside echocardiography to direct therapy. OBJECTIVES: We propose an echocardiography-centered, phenotype-driven flowchart to standardize decisions. METHODS: Our proposed algorithm (1) prioritizes standard airway, breathing and circulatory management and an initial mean arterial blood pressure (MAP) target ≥ 65 mmHg, with higher targets (for example, 75–80 mmHg) considered in selected patients such as those with chronic hypertension; (2) embeds immedi

Challenges and Perspectives in Optimising the Treatment of Arterial Hypertension: Role of the Ramipril-Amlodipine-Hydrochlorothiazide Single-Pill Combination.

Globally, hypertension remains inadequately controlled, despite the availability of effective therapies and guideline recommendations. This narrative review synthesises current evidence on the clinical rationale, efficacy and implementation of single-pill combination (SPC) therapy to control blood pressure (BP), with a focus on an SPC containing ramipril, amlodipine and hydrochlorothiazide. These agents offer complementary mechanisms of action and a favourable tolerability profile, supporting their use in dual and triple SPCs to overcome therapeutic inertia (failure to intensify therapy when B

Calcium Signaling and Pathogenesis of Neurodegenerative Disorders: Potential Therapeutic Opportunities.

Alzheimer's disease and other neurodegenerative disorders are major health and economic problems. The main emphasis on treating these disorders has been based on the idea of targeting the amyloid pathway, but these efforts have not yielded effective disease-modifying therapies. In this article, I review arguments in favor of "calcium hypothesis of neurodegeneration" that offer additional targets that may potentially be explored. The main limitation in translation of this idea to the clinic has been concern about potential side effects of calcium channel blockers and inhibitors of calcium signa

Rational development of a drug-delivery device based on MOF UiO-66 for calcium-channel-blocker drugs.

The administration of drugs with low solubility or absorption, such as the calcium-channel-blocker (CCB) drugs amlodipine, nifedipine, and nimodipine, affects the management of complicated health conditions related to cardiovascular diseases. The development of a drug-delivery device based on MOFs is a rising topic. Thus, in this work, the use of MOF UiO-66 as a platform to develop a drug-delivery device was analyzed using CCB drugs through an empirical and theoretical approach. Our results demonstrate that UiO-66 adsorb and release the three studied drugs without altering the chemical or phys

Selective Cav1.3 inhibition promotes survival of transplanted dopaminergic neurons via the CaMKII-p65-p53 pathway.

Poor survival of transplanted midbrain dopaminergic neurons (mDANs) remains a key barrier for stem cell-based Parkinson's disease (PD) therapy. Cav1.3-containing L-type calcium channel blockers have been shown to prevent native mDAN loss in PD animal models and patients. Here, we aim to investigate whether they also enhance the post-transplant survival of stem cell-derived mDANs. We found that Cav1.3 is highly expressed in human embryonic stem cell-derived mDANs. Cav1.3 inhibitors including isradipine and cp-PYT significantly reduced apoptosis and improved mDAN survival in both interferon gamm

Class-Specific Antihypertensives and Alzheimer's Disease: Genotype- and Hypertension-Stratified Analysis.

Antihypertensive medications (AHMs) may modulate Alzheimer's disease (AD) pathogenesis via cerebrovascular or neuroinflammatory pathways, yet evidence remains conflicting. This study investigated causal associations between AHM use and AD risk, focusing on drug classes, blood pressure status, and apolipoprotein E epsilon 4 (APOE ε4) effects. We integrated genetic causal inference with longitudinal survival analyses in a dual-evidence framework. Mendelian randomization (MR) was used to estimate class-specific causal effects at the population level. To examine effect modification by genetic and

Molecular Mechanisms and Targeted Intervention Strategies of Calcium Overload in Ischemic Stroke.

Ischemic stroke is a leading cause of global neurological mortality and disability, characterized by a complex pathogenesis wherein calcium overload constitutes a pivotal mechanism in neuronal and glial cell death. Following ischemia and reperfusion, excitotoxicity triggered by excessive glutamate release activates NMDA receptors and voltage-dependent calcium channels, leading to a large accumulation of intracellular Ca2+. This calcium dyshomeostasis subsequently initiates a cascade of detrimental events, including mitochondrial dysfunction, endoplasmic reticulum stress, reactive oxygen specie

Pharmacokinetic Insights and Therapeutic Potential of Calcium Channel Blockers in Cardiovascular and Non-Cardiovascular Disorders.

Calcium channel blockers (CCBs) regulate calcium ion transport across cell membranes and are central to the management of hypertension, angina, arrhythmias, and cerebrovascular disorders, with emerging roles in neurological and oncological diseases. Most CCBs undergo significant first-pass metabolism mediated by cytochrome P450, which affects their interindividual variability, dosage, and bioavailability. Recent research highlights the potential of T-type CCBs in cancer treatment and the use of nanocarriers to overcome their low bioavailability and rapid metabolism. Since the introduction of v

Hexahydroquinoline Featuring Amide Functionality: A Promising Scaffold With Calcium Channel Blocking Activity.

Hexahydroquinoline (HHQ) is a widely recognized scaffold that has garnered considerable attention owing to its diverse pharmacological properties. The structure of HHQ includes a 1,4-dihydropyridine (DHP) ring, which serves as the pharmacophore for the predominant class of drugs known as calcium channel blockers. DHPs are frequently utilized in the management of cardiovascular diseases and also show potential for pain management. Since all DHPs on the market possess ester functionality, we aimed to employ bioisosteric replacement to observe if their amide-containing counterparts would still bl

Pharmacology and Pharmacogenomics of Anti-Migraine Drugs.

Migraine is the most prevalent neurological condition, which affects 1.16 million individuals globally, ranking first among adolescents and children and second in the case of adults aged under 60 years. Both genetic and environmental factors contribute to its pathophysiology, influencing treatment outcomes and drug responses. Pharmacogenomics has emerged as a critical field in understanding interindividual variability in migraine therapy. Genetic variants in pharmacokinetic and pharmacodynamic pathways impact the metabolism, efficacy, and adverse drug reactions of drugs used in migraine. Varia

Involvement of Cav3.2 T-Type Ca2+ Channels and the Role of Endogenous Estrogen in Pruritus: Evidence from a Fundamental Study and Cross-Sectional Analysis of Pharmacy Claims Data.

To clarify the roles of Cav3.2 T-type Ca2+ channels and endogenous estrogen in pruritus, we conducted a fundamental study employing mice and clinical cross-sectional analyses of pharmacy claims data. In mice, intradermal injection of sulfide (Na2S), a Cav3.2 enhancer, caused itch responses, an effect blocked by KTtp38, a T-type Ca2+ channel inhibitor, and deletion of Cav3.2 gene. KTtp38 also suppressed itch responses following intradermal histamine or chloroquine. The sulfide-induced itch responses in female mice decreased by ovariectomy and/or repeated treatment with letrozole, an aromatase i
