Autoimmune Primary Adrenal Insufficiency: Understanding the Past, Present, and Future.

INTRODUCTION: Primary adrenal insufficiency (PAI), or Addison's disease, results from adrenal gland dysgenesis or destruction, leading to impaired production of glucocorticoids, mineralocorticoids, and adrenal androgens. Our understanding of the etiology, pathophysiology, and clinical manifestations of PAI has significantly evolved since this condition was originally described. RESULTS: Over the past 3 decades, the epidemiology and demographics of PAI has shifted, with autoimmune PAI now recognized as the most common cause. This shift has been influenced by increasing awareness of autoimmunit

The Landmark Series: Evaluation and Management of Adrenal Incidentalomas.

Adrenal incidentalomas are adrenal masses ≥ 1 cm discovered on imaging studies for unrelated clinical conditions. The prevalence of adrenal incidentalomas has increased as a byproduct of the widespread use of cross-sectional imaging, particularly in older adults. The clinical significance of adrenal incidentalomas varies based on tumor size, hormonal activity, and imaging characteristics. While most adrenal incidentalomas are benign and asymptomatic, a significant minority are hormonally active or malignant, necessitating careful evaluation and management. Adrenal hormone secretion can have si

A case of mineralocorticoid intermediate-producing sarcomatoid adrenal cortical carcinoma: case report and review of literature.

Sarcomatoid adrenal cortical carcinoma (SACC) is an extremely rare histological subtype accounting for only 0.2% of all adrenal cortical carcinomas. Most reported cases of SACC are nonfunctional, showing a biphasic histological pattern with both epithelial adrenocortical carcinoma and sarcomatous components, which are often associated with poor prognosis. Herein, we report a unique case of SACC with characteristics distinct from those previously documented. A 66-year-old man presented with uncontrolled hypertension, night sweats, exertional dyspnea, and palpitations. Imaging revealed an 11 cm

How should glucocorticoid and mineralocorticoid replacement be optimised in a young patient with classic 21-hydroxylase congenital adrenal hyperplasia?

This article aims to provide a simple clinical approach to understanding and managing steroid supplementation in young adults and adult patients with classical congenital adrenal hyperplasia secondary to 21-hydroxylase deficiency (21OHD). It aims to overcome the clinical inertia that leads to glucocorticoid excess in these patients, particularly addressing the adult endocrinologist at the transition when they meet the patient with 21OHD with 'fresh eyes'.

A novel serum-based steroid-protein panels for differentiating ovarian cancer from non-malignant adnexal masses.

BACKGROUND: Ovarian cancer is the deadliest gynecological malignancy, largely due to the advanced stage at diagnosis in most patients. This study investigates whether systemic steroids can serve as biomarkers to distinguish malignant ovarian tumors from non-malignant adnexal masses. METHODS: This prospective, single-center observational study included 99 women with adnexal masses who underwent surgery between December 2021 and February 2025. Preoperative serum levels of 17 steroid hormones, including androgens, 11-oxyandrogens, glucocorticoids, and mineralocorticoids, were quantified using li

Distribution of androgen receptor immunoreactive cells in the adrenal glands of Hystrix javanica.

BACKGROUND: The adrenal glands play a vital role in regulating endocrine functions by producing corticosteroids, mineralocorticoids, and androgens. Adrenal androgens contribute to overall systemic androgen levels, primarily through interactions with androgen receptors (ARs) within the gland. Although the distribution of AR has been studied in various mammalian species, no research has yet focused on the Sunda porcupines (Hystrix javanica), a species of considerable ecological importance in Indonesia. AIM: This study aimed to investigate the endocrinal regulation of the adrenal gland of Sunda

An unusual case of remission of clinically overt autoimmune Addison's disease in a Pakistani girl.

Addison's disease is generally regarded as an autoimmune irreversible, progressive disease leading to the lifelong replacement of glucocorticoids and mineralocorticoids. We are reporting a rare case of a young girl diagnosed with clinically overt Addison's disease whose adrenal function recovered over time. A 10.5 years old girl, known case of celiac disease for five years with good compliance presented at 7.5 years with complaints of dark pigmentation, lethargy, nausea, and vomiting. Physical examination showed hyperpigmentation along with orthostatic hypotension. Biochemical tests confirmed

Postoperative adrenal cortical insufficiency following adrenal adenoma removal: a case report.

BACKGROUND: The adrenal gland is one of the important endocrine organs in the human body. With advancements in medical technology, the diagnosis rates of adrenal diseases, such as adrenal tumors and adrenal cortical hyperplasia, have been steadily increasing. Surgical intervention is the mainstream method for treating adrenal tumors; however, unilateral complete adrenalectomy may impair the patient's adrenal cortical function, leading to adrenal insufficiency (AI). AI is caused by a deficiency of glucocorticoids, which may be accompanied or not accompanied by reductions in mineralocorticoids a

Adrenalectomy Versus Medical Therapy in Primary Aldosteronism: A Meta-Analysis of Long-Term Cardiac Remodeling and Function: Medical versus Adrenalectomy Treatment Compared in Hyperaldosteronism (MATCH) Study.

BACKGROUND: Primary aldosteronism (PA) is a common curable cause of secondary hypertension that significantly increases left ventricular mass (LVM) and predisposes patients to adverse cardiovascular outcomes. Although adrenalectomy and medical therapy with mineralocorticoid receptor antagonists (MRAs) are both used to treat PA, their long-term comparative efficacy in reversing cardiac structural changes remains debated. METHODS: We systematically searched MEDLINE and Embase for prospective and retrospective clinical trials published up to November 22, 2024, with a minimum follow-up of 6 month

Endocrine disrupting chemicals and the adrenal gland.

Endocrine disrupting chemicals (EDCs) are exogenous compounds that have hormone-like effects inside the body. Endocrine disruptors can affect adrenal glands in humans and animals. Adrenocortical dysfunction related to chemical exposure has been reported in the literature. The human adrenal gland is crucial for the synthesis of steroid hormones (glucocorticoids, mineralocorticoids, and androgens) and amino acid-derived hormones (epinephrine and norepinephrine). All steps in the adrenocortical steroidogenesis pathway are potential targets for chemical inhibition, including the adrenocorticotropi

Adrenal-adipose tissue crosstalk in health and disease.

Adipose tissue (AT) closely interacts with the adrenal glands to regulate metabolism, energy balance, and stress responses. While the adrenal cortex secretes glucocorticoids and mineralocorticoids that influence AT distribution, lipid storage, and browning, the adrenal medulla releases catecholamines that acutely activate thermogenesis in brown and beige adipocytes. Under physiological conditions, this bidirectional crosstalk maintains energy homeostasis and cardiovascular stability. However, in adrenal diseases such as Cushing syndrome, primary aldosteronism, adrenocortical carcinoma, or pheo

Performance of Plasma Renin Assays in Guiding Mineralocorticoid Dosing in Children With Adrenal Insufficiency.

BACKGROUND: Plasma renin is measured in children with salt-wasting adrenal insufficiency (AI) as part of assessing the adequacy of mineralocorticoid (MC) replacement. It is recommended that plasma renin is used to monitor MC replacement. This can be measured as either plasma renin concentration (PRC) or activity (PRA); it is not known if one method is preferable over the other. METHODS: Samples from 129 patients less than 18-years old requested for PRC were selected randomly. PRC was measured using the IDS-iSYS chemiluminescence immunoassay, PRA was measured by LC-MS/MS. The PRC and PRA value

Adrenal failure as an antiphospholipid syndrome manifestation: case-based review.

Adrenal involvement in antiphospholipid syndrome (APS) is rare but potentially life-threatening. Early recognition reduces morbidity and mortality. We report a 56-year-old male with chronic thrombocytopenia and prolonged activated partial thromboplastin time who developed fatigue, anorexia, and weight loss. On admission, he presented with hypotension, hyperkalemia, thrombocytopenia, anemia, and elevated inflammatory markers. After hospitalization, he underwent an extensive workup study. PET-CT revealed bilateral adrenal uptake, and hormonal tests confirmed primary adrenal insufficiency (PAI).

Unveiling the role of mineralocorticoid receptor antagonists in epilepsy: the intersection of seizures, stress, and neuroinflammation.

INTRODUCTION: Epilepsy often presents numerous comorbidities, including anxiety, depression, and cognitive deficits. Epilepsy is not only restricted to GABA/glutamate imbalance, as researchers are delving more into finding the possible causal inference between other factors; one such recognized interplay is between seizures, stress, and inflammation. Stress-induced hypothalamic-pituitary-adrenal (HPA) axis activation increases corticosteroid release, which binds to mineralocorticoid receptors (MR), intensifies the release of neuroinflammatory markers, lowers seizure threshold, and worsens seiz

Metabolic dysfunction-associated steatotic liver disease in individuals with type 2 diabetes mellitus and diabetic foot ulcers: prevalence and associated factors.

BACKGROUND: Classical congenital adrenal hyperplasia (CAH) is characterized by a defect in cortisol and aldosterone secretion, adrenal hyperandrogenism, impaired adrenal medullary function and insulin insensitivity. The latter along with the increased tendency towards obesity raises questions whether other cardiovascular risk factors are altered in CAH. OBJECTIVE: To evaluate 24-h ambulatory blood pressure and obesity in patients with salt-wasting 21-hydroxylase deficiency diagnosed in the neonatal period and treated with hydrocortisone and 9alpha-fludrocortisone thereafter. [A rare case of autoimmune polyglandular syndrome type 2 in an elderly patient].

Autoimmune polyglandular syndrome type 2 (APS-2) is a rare and complex clinical entity, whose etiology and evolution are not fully understood. We present the case of a 78-year-old male, followed and treated for Addison's disease for 45 years, in whom macrocytosis with a hemoglobin level of 11.8 g/dL was detected during a routine evaluation. The immunological vitamin B12 deficiency led to the diagnosis of Biermer's disease, while the thyroid, diabetic and liver immunological evaluations were negative. Treatment was started with periodic injections of vitamin B12 and hormone replacement therapy

Neuroendocrinology meets addiction: Emerging pharmacotherapies on the horizon.

Alcohol and other substance use disorders (ASUDs) are prevalent and major contributors to global morbidity and mortality. Their impact extends beyond the individual, imposing significant burdens on families, communities, healthcare systems, and society at large. Treatments include psychosocial, behavioral, and pharmacological interventions. However, available pharmacological treatments remain limited, primarily targeting alcohol, tobacco, and opioid use disorders, with a lack of approved pharmacotherapies for other substance use disorders. This gap highlights a critical need to develop novel t

Effects of mineralocorticoids on α-Na+/K+ ATPase expression and parasite morphology in T. crassiceps WFU cysticerci.

Mineralocorticoids are corticosteroid hormones that, in vertebrates, regulate extracellular fluid volume and salt balance by modulating the expression and activity of Na+/K+ ATPase, the enzyme that generates and maintains Na+ and K+ gradients across plasma membranes. However, the function of mineralocorticoids in invertebrates has not been well studied. Cysticerci (larvae) of the parasite Taenia crassiceps WFU synthesize the corticosteroids 11-deoxycorticosterone (DOC) and corticosterone in vitro. Here, we searched the genome and transcriptome of T. crassiceps for the presence of α-Na+/K+ ATPa

Prevalence and main characteristics of primary aldosteronism in bilateral macronodular adrenal disease: a systematic review of the literature.

CONTEXT: Bilateral macronodular adrenal disease (BMAD) typically presents with bilateral benign adrenocortical macronodules and variable cortisol excess. Anecdotal evidence suggests oversecretion of other adrenal steroids, including mineralocorticoids. HYPOTHESIS: We hypothesized that primary aldosteronism (PA) can occur in BMAD, resulting in a distinct, more severe phenotype compared to BMAD with isolated cortisol hypersecretion or unilateral PA (uPA). OBJECTIVE: To assess the prevalence and characteristics of PA in BMAD. METHODS: We systematically reviewed case reports and series of BMAD

Effect of fludrocortisone in the management of hyponatremia after aneurysmal subarachnoid hemorrhage.

Aneurysmal subarachnoid hemorrhage (aSAH) is associated with significant morbidity, particularly related to delayed cerebral ischemia (DCI). Hyponatremia, defined as a serum sodium level < 135 mmol/l, is a common complication of aSAH and is associated with the development of DCI. Fludrocortisone is used to decrease natriuresis and improve hyponatremia, but has failed to demonstrate a reduction in the frequency of cerebral vasospasm or DCI. Using the ProReSHA database, we retrospectively analyzed the impact of fludrocortisone on the development of DCI in patients hospitalized in neurocritical c
