Revisiting the anatomy of the retroperitoneum and renal fascia. Aspects applied to radiology, laparoscopic and robotic surgery - a narrative review.

This review aims to provide a comprehensive analysis of the anatomy of the retroperitoneal and abdominal fascias. We analyzed papers published in the past 70 years in the databases of Pubmed, Embase and Scielo. We excluded case reports, editorials and opinions of specialists. Studies were excluded if they lacked clear anatomical descriptions of the fascias, if full-text access was unavailable, or if they were published in languages other than Portuguese, English, or Spanish. This narrative review addresses the anatomical integration, surgical techniques, and functional importance of the fascia

Case Report: Diagnostic pitfalls and laparoscopic repair of a delayed large right-sided traumatic diaphragmatic hernia.

BACKGROUND: Delayed right-sided traumatic diaphragmatic hernia (TDH) may be overlooked during polytrauma assessment because concomitant injuries dominate attention and the liver can obscure diaphragmatic discontinuity. Subsequent respiratory and gastrointestinal symptoms may remain nonspecific until obstruction or visceral compromise develops. CASE DESCRIPTION: A 50-year-old man presented with more than 20 h of nausea and vomiting 19 months after severe blunt trauma. Since the injury, he had experienced intermittent dyspnea, chest tightness, postprandial fullness, abdominal distension, and ab

Cholecystocolonic Fistula With Acute Cholangitis: A Case Report.

Cholecystocolonic fistula (CCF) is a rare type of cholecystoenteric fistula, which are sometimes diagnosed incidentally in patients undergoing cholecystectomy. They are usually a complication of chronic cholecystitis, often secondary to gallstones, and they are difficult to diagnose due to their rarity and vague clinical presentation. If untreated, they can lead to complications such as acute cholangitis, biliary peritonitis, and biliary cirrhosis. This highlights the importance of early recognition of CCF and timely treatment. While the generally accepted management is surgical, there are no

Concurrent follicular lymphoma and follicular dendritic cell sarcoma: composite neoplasm or transdifferentiation?

Follicular dendritic cell sarcoma (FDCS) is a rare neoplasm, and its synchronous occurrence with follicular lymphoma (FL) has not been reported. Whether composite tumors arise independently or through clonal evolution remains unclear. A 52-year-old woman presented with night sweats, weight loss, and an obstructing hepatic flexure colonic mass with mesenteric disease. Biopsy showed two tumor populations: an atypical B cell infiltrate positive for CD20, CD10, focal BCL6, and weak/focal MUM1, with kappa predominance and Ki-67 of 20-50%, consistent with follicular large B cell lymphoma/FL grade 3B

Prognostic factors of non-metastatic right colon adenocarcinoma: A multicentre retrospective analysis from the ATCCR database.

BACKGROUND: Right-sided colon cancer (RSCC) exhibits distinct characteristics, yet specific prognostic determinants remain insufficiently defined. This multicentre retrospective study evaluated clinicopathological and surgical factors influencing survival in a real-world cohort undergoing curative resection for non-metastatic RSCC. METHODS: We analysed 800 adults undergoing curative-intent resection for non-metastatic RSCC across 28 centres (2018-2024). The primary outcome was 3-year overall survival (OS). Multivariate Cox model identified prognostic factors. An exploratory, post hoc subgroup

Pancreaticocolonic fistula resolved through conservative management: a case that defies the norm.

Spontaneous fistulization of pancreatic collections into the colon is a rare complication of necrotizing acute pancreatitis, with an incidence of less than 3% and high morbidity and mortality. We present the case of a 47-year-old male with severe necrotizing acute pancreatitis who developed a 12 mm pancreaticocolonic fistula between an infrapancreatic retroperitoneal collection and the hepatic flexure of the colon. Given the patient's clinical stability, a conservative approach was adopted, consisting of broad-spectrum antibiotic therapy, parenteral nutrition, and close radiological monitoring

Temporary intraperitoneal breast implant spacer to facilitate safe stereotactic body radiotherapy for recurrent high-grade serous ovarian carcinoma.

BACKGROUND: Recurrent ovarian cancer involving the upper abdomen can be challenging to treat with radiation therapy due to the proximity of bowel. Spacer technologies have been used in other malignancies to displace radiosensitive organs, but they are not routinely employed in gynecologic oncology. CASE: We report the case of a 71-year-old woman with a germline BRCA1 mutation and recurrent high-grade serous carcinoma following secondary cytoreduction with right hepatectomy and diaphragmatic resection. Surveillance imaging demonstrated a right upper quadrant abdominal wall recurrence overlying

Exceptional long-term survival in a patient with recurrent pancreatic carcinoma on adjuvant tegafur-gimeracil-oteracil potassium: a case report.

BACKGROUND: Pancreatic carcinoma is a highly malignant tumor, and it is aggressive even in its early stage. It has a very poor prognosis and responds poorly to systemic therapy. It is common for resected patients to undergo postoperative adjuvant chemotherapy. However, survival remains low, with an average 5-year survival of 10-25% and median survival between 10 and 20 months. CASE PRESENTATION: A 50-year-old Malaysian Chinese gentleman with no past medical history was diagnosed with stage IIb pancreatic cancer. A computed tomography scan of the abdomen showed a tumor at the uncinate lobe of

Hepatic flexure: The end of the beginning, or the beginning of the end? Why pre-operative localisation remains inconsistent.

Precision colon cancer surgery increasingly aims to tailor the extent of resection to individual tumour biology and lymphatic drainage, guided by pre-operative assessment. However, at the hepatic flexure (HF), reproducible pre-operative localisation remains challenging because the HF is not a discrete anatomical point but a transition zone. Misclassification at the HF is not merely semantic: given non-trivial intermediate and central nodal metastasis rates in this region, inconsistent localisation may influence assumptions about regional nodal basins and the adequacy of mesenteric excision and

Signet-ring cell colon carcinoma in a Northern Ghanaian child: a case report.

INTRODUCTION AND IMPORTANCE: Colorectal cancer (CRC) in children represents less than 1% of pediatric malignancies worldwide. Signet-ring cell carcinoma (SC), a subtype associated with mucin-producing phenotypes and distinct molecular pathways, is infrequently encountered in this age group, particularly in low-resource settings. PRESENTATION OF CASE: A 10-year-old boy presented with 1 month of right lower quadrant pain, progressive abdominal distension, anorexia, constipation, and weight loss. Radiologic studies (plain abdominal X-ray, barium enema, and CT scan) showed right-sided colonic obs

Nomogram for preoperative prediction of lymph node metastasis in patients with early submucosal right-sided colon cancer: a dual-center retrospective cohort study.

BACKGROUND: Preoperative lymph node metastasis (LNM) risk stratification is crucial for selecting endoscopic resection or colectomy in T1 right-sided colon cancer (T1 RCC), yet validated tools integrating preoperative predictive indicators remain lacking. METHODS: This retrospective study included 628 patients with T1 RCC. Single-predictor and multi-predictor logistic regression models identified independent preoperative predictors of LNM to construct a nomogram. Model performance was assessed using area under the curve (AUC) and calibration metrics. RESULTS: Six preoperative factors constit

Adult ileocolic intussusception with lymphoma as the lead point: a case report.

Adult ileocolic intussusception is almost always associated with a pathological lead point. We present a 29-year-old male with autism spectrum disorder (ASD) who attended with a one-week history of colicky right iliac fossa pain and loose stools. Communication difficulties related to ASD delayed a clear clinical history. Abdominal ultrasonography demonstrated a 7.0 × 6.0 cm target sign in the subhepatic region. Contrast-enhanced computed tomography confirmed ileocolic intussusception extending to the hepatic flexure, with a heterogeneously enhancing 6.6 × 4 × 3.4 cm lead-point mass. Diagnostic

Primary omental torsion mimicking acute appendicitis in a child: a case report.

Omental torsion is a rare cause of acute abdomen and an uncommon entity in the pediatric population, often mimicking more common surgical conditions such as acute appendicitis. Preoperative diagnosis remains challenging due to nonspecific clinical and radiological findings. We report the case of an 8-year-old boy who presented with acute right-sided abdominal pain and ultrasound findings initially suggestive of early acute appendicitis, which later proved to be misleading. Diagnostic laparoscopy revealed a hemorrhagic, infarcted omental mass near the hepatic flexure, along with a mildly conges

Clip-Assisted Cyanoacrylate Injection for the Management of a Proximal Colonic Varix.

Colonic varices are a rare manifestation of portal hypertension and an uncommon cause of lower gastrointestinal bleeding, particularly when involving the proximal colon. We report a case of bleeding hepatic flexure varices in a 23-year-old male with extrahepatic portal vein obstruction (EHPVO) and prior splenectomy with lienorenal shunt. The patient presented with recurrent hematochezia and severe anemia. Upper gastrointestinal endoscopy was negative for a bleeding source, while colonoscopy revealed large tortuous varices at the hepatic flexure with stigmata of recent hemorrhage. Contrast-enha

Proximally dominant inflammation at colonoscopy predicts pan-colonic histological remission following oral vancomycin therapy in pediatric ulcerative colitis.

BACKGROUND: Oral vancomycin therapy (OVT) induces histological remission in children with ulcerative colitis (UC) showing primary sclerosing cholangitis (PSC)-associated features (proximally dominant or patchy inflammation, backwash ileitis, or rectal sparing). However, predictors of response and optimal treatment duration remain unclear. AIM: To evaluate segmental colonoscopy outcomes across various OVT durations. METHODS: Children with active UC receiving OVT for ≥3 months were retrospectively identified. Pre-post-OVT colonoscopies were available for 38 children, 4 had repeated examination

Cancer of Unknown Primary Most Suggestive of Colorectal Origin With Microsatellite Instability-high Status and High Tumor Mutation Burden Including NTRK Fusion.

BACKGROUND/AIM: Cancer of unknown primary (CUP) is associated with a poor prognosis. Recent advances in comprehensive genomic profiling (CGP) have made it possible to detect actionable mutations in CUP cases, which allows the inference of the primary tumor site. This report demonstrates CUP most suggestive of colorectal origin and highlights how CGP can significantly impact treatment decisions and outcomes. CASE REPORT: A 73-year-old woman presented with altered consciousness secondary to diabetic ketoacidosis. Imaging revealed a huge mass with abscess formation near the hepatic flexure, but

[Right-Sided Colon Cancer: Is Hepatic Flexure Location a Factor of Poor Oncologic Prognosis?].

UNLABELLED: Limited evidence exists regarding survival outcomes among different subsites of right-sided colon tumors, thus contributing to uncertainty regarding their prognostic implications. AIM: To assess the oncological impact of hepatic flexure tumor location compared with other subsites in patients with right-sided colon cancer undergoing conventional right colectomy with curative intent. METHODS: This retrospective study included patients with right-sided colon cancer stage I-III undergoing right colectomy with curative intent between January 2010 and December 2021. A Rare Case of Squamous Cell Carcinoma of the Colon.

Squamous cell carcinoma (SCC) of the colon is a rare malignancy. Unlike colonic adenocarcinomas, it lacks defined risk factors or established treatment guidelines. We describe a case of metastatic squamous cell carcinoma of the colon in a 77-year-old male with ulcerative colitis. The patient presented with abdominal pain and had labs that revealed transaminitis with conjugated hyperbilirubinemia. Imaging showed a large irregular mixed hyperechoic mass with fluid collection in the liver along with retroperitoneal lymphadenopathy concerning infection/abscess or malignancy. This was initially tre
